- B.3 STATEMENT OF WORK (SOW) - VISN 12 BioFire Torch System Maintenance and Service 1. Scope of Procurement Contractor shall provide an extended warranty, to include equipment preventative maintenance and emergency repair services for Torch Systems at the following facilities: Edward Hines, Jr. VA Hospital, Hines, Illinois (Hines) Clement Zablocki VA Medical Center, Milwaukee, Wisconsin (Milwaukee) VA Illiana Health Care System, Danville, Illinois (Danville) The period of performance is one base year plus two one-year options to extend services, to be exercised at the Government s discretion. 2. General Requirements The Contractor shall provide an Extended Warranty for the Torch Systems at Hines, Milwaukee, and Danville. The Contractor is responsible for all shipping costs, including insurance, for preventive maintenance and equipment repair. Telephone support must be available seven days per week to troubleshoot equipment malfunctions. Equipment repair service must be available during regular business hours, Monday Friday, 8:30 AM to 4:30 PM. Certain circumstances may dictate the need for emergency equipment repair service to be conducted outside routine business hours. Service response time will be no more than 24 hours during the regular business week. Arrangements for repair service will be coordinated between the Contractor and the Government laboratory personnel. The Contractor shall furnish labor, parts, and/or replacement equipment necessary to repair operational or mechanical breakdowns of instrumentation and ancillary equipment (i.e. printers, computers, etc.) The Contractor shall use new equipment and maintenance parts, acquired directly from BioFire Diagnostics, LLC. A service report shall be furnished to the respective Government Laboratory upon completion of each repair or preventative maintenance service. The Contractor will reimburse the Government for pouches used for mini-validations if equipment is loaned to the Government. Contractor personnel must have documentation from BioFire that they have been trained, certified and authorized to provide repair, preventative maintenance, and technical support services on BioFire FilmArray Systems. The Contractor shall provide upgrades to both the equipment hardware and software to maintain the integrity of the system and the state-of-the-art technology at no additional charge to the Government. These must be provided as they become commercially available and at the same time as they are being provided to commercial customers. 3.
