- Requirement Title: 169 ASOS Human Performance Program Services: Physical Therapist (PT)/Injury Manager (IM) Solicitation Number: W50S7T-21-R-0004 Classification Code Q518 � Medical Physical Medicine/Rehabilitation Set Aside: Total Small Business NAICS Code: 621340 The Illinois Air National Guard at Peoria Air National Guard Base intends to issue a Request for Proposal (RFP) to award a single firm fixed- price contract for services, non-personal, to provide all personnel, equipment, tools, materials, supervision, and quality control necessary to perform Human Performance Program (HPP) services. The Contractor shall perform injury prevention and mitigation, rehabilitation services, and human performance optimization services to personnel assigned to the 168 Air Support Operations Squadron (ASOS), 182 Airlift Wing (AW), Peoria, IL, USA. The Contractor shall furnish consultation and reports, similar to the range of services found in a commercial facility, as well as services specialized to meet the needs of Air Force Special Warfare operators. Performance shall be in accordance with the professional standards of the Accreditation Association for Ambulatory Health Care (AAAHC), and as defined in the Performance Work Statement. The North American Industry Classification System (NAICS) code for this work is 621340. The small business size standard is $8 million. This action is being procured on a total small business set-aside basis. Due to COVID-19 precautions and the nature of the services required, a site visit will not be conducted. All questions regarding this RFP shall be submitted only to the Contracting Officer, Melissa Grice, by 12:00pm CDT, August 9, 2021 via email to melissa.grice.2@us.af.mil.� Answers will be posted via an Amendment to the solicitation on Beta.Sam.gov on or about close of business on August 11, 2021. Questions sent to anyone other than the Contracting Officer will only delay an answer being posted.� Offers are due by the date and time listed in the Solicitation document W50S7T-21-R-0004.� The source selection process will be conducted in accordance with FAR Part 15.3 and DFARS Part 215.3, source selection procedures. Evaluation factors include Technical, Past Performance and Price. Award will be made to the offeror who represents the best value to the Government as outlined in accordance with the criteria set forth within the solicitation.�The best value will provide a comprehensive, coordinated, multidisciplinary �pre-habilitative� physical training program with focused strength and conditioning, performance nutrition, and physical therapy.� The Government intends to award without discussions.� ONLY EMAILED OR MAILED OFFERS WILL BE ACCEPTED.� NO FACSIMILE OFFERS WILL BE ACCEPTED. Funds are not presently available for this contract. Currently, the Government anticipates funds will become available prior to but no later than midnight 30 Sep 2021. This action has been identified on the National Guard priority list for award if funds become available and the prices received are within an awardable range.� Interested offerors must be registered in the System for Award Management (SAM).
