Combined Synopsis-Solicitation for Commercial Items Request for Quote Number 36C25621Q1324 HOPTEL Service for the Overton Brooks VA Medical Center Shreveport, LA VHAPM Part 813.106 SAP: Soliciting Competition & Evaluation of Quotations SOP Page 3 of 5 Original Date: 10/12/17 Revision 04 Date: 06/10/20 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, and FAR Part 13, as supplemented with additional information included in this notice. Evaluation of price and other factors will be performed in accordance with procedures outlined in FAR 13.106-3 This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This combined synopsis/solicitation is issued as a Request for Quote (RFQ) 36C25621Q1324. The government anticipates awarding a Firm Fixed Price, Contract resulting from this solicitation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular Number 2021-05 dated March 10, 2021. Set- Aside: 100% Service-Disabled Veteran-Owned Small Business (SDVOSB). Contract Type: Firm Fixed Price The associated North American Industrial Classification System (NAICS) code for this procurement is 721110, Hotels (except Casino Hotels) and Motel, with a small business size standard of $35.0 Million. The FSC/PSC is V231 (Transportation/travel/relocation- travel/lodging/recruitment). The Network Contracting Office (NCO) 16, Department of Veterans Affairs Medical Center in Shreveport, LA has a requirement for HOPTEL Service, temporary overnight lodging accommodations for eligible Veterans that travel long distance (50 miles or more) to early morning or late afternoon appointments for the Overton Brooks VA Medical Center, which is located at 510 E Stoner Avenue, Shreveport, LA. Network Contracting Office (NCO) 16 intends to award a single firm fixed price contract for HOPTEL Hotel Services for Eligible Veteran Beneficiaries with an expected performance period of October 1, 2021 September 30, 2025. INTRODUCTION: 1.1 The Government requires lodging services for Eligible Beneficiary Veterans of the Overton Brooks VA Medical Center. II. The Contractor shall provide all labor, equipment, tools, material, supervision and other items and services necessary to perform the work as defined in this Performance Work Statement. III. APPLICABLE DOCUMENTS: The following laws, regulations, policies, and procedures in effect on date of contract issuance and all subsequent changes or updates apply: FAR Federal Acquisition Regulation VAAR Veterans Administration Acquisition Regulation US Code ยง1708 Temporary Lodging CFR Part 60 Fisher Houses and Other Temporary Lodging VHA Directive 1107 VA Fisher Houses and Other Temporary Lodging Definitions: 2.1 Beneficiary: Means a person determined eligible for Veterans Health Administration (VHA) benefits. 2.2 Physician, physician assistant (PA), nurse practitioner (NP), register nurse (RN), case manager (CS), or social worker (SW) authorized to submit a request for lodging. 2.3 AOD: Administrative Officer of the Day 2.4 COR: Contracting Officer Representative 2.5 FAR: Federal Acquisition Regulation 2.6 VA: Department of Veterans Affairs 2.7 VHA: Veterans Health Administration 2.8 VA facility: Means VA medical center (VAMC), VA Outpatient Clinic (OPC), or VA Community Based Outpatient Clinic (CBOC) 2.9 CO: Contracting Officer 3.0 PWS: Performance Work Statement 3.1 OBVAMC: Overton Brooks VA Medical Center IV. PERFORMANCE WORK STATEMENT - SERVICE DESCRIPTION AND REQUIREMENTS 1) SPECIFICATIONS: Contractor shall furnish rooms to VA beneficiaries as referred by authorized personnel of the Shreveport VAMC. The contractor shall furnish all standard supplies/services included as minimum room requirements. Lodging shall be provided in accordance with HOPTEL standards to accommodate those Veterans receiving medical care who may have to travel long distances for evaluation the day before or the day after treatment received. Only medically stable patients who would normally be discharged to their home will be considered for lodging. 2) LOCATION: Lodging facilities must be located within ten (10) miles of the Shreveport VA Medical Center located at 510 E Stoner Avenue, Shreveport, LA. 3) ROOM REQUIREMENTS: a. Daily room rates shall include all fee and applicable taxes and any other no-charge services offered to guests such as shuttle service and continental breakfast. Rate quotes shall not include sales tax. The VAMC is a Federal institution and therefore exempt from paying sales tax. Room rates offered shall be Single Occupancy and shall provide a minimum of two (2) double beds. The VA patient shall be allowed one caregiver/driver/attendant to stay in the same room with the Veteran, at the single occupancy rate when pre-authorized by the VA. Any additional occupants are the responsibility of the veteran and shall not lodge at VA expense. b. Contractor shall reserve ten (10) rooms per night. An additional five (5) rooms may be reserved by the VA at the same rates based on a space available basis. Contractor will be reimbursed only for actual rooms nights used in accordance with this contract. c. The fully furnished hotel/motel rooms shall include contractor provided utilities including heat, air conditioning, electricity, water and sewer, local telephone service (no deposit required), basic television cable service, and other specified services. The hotel/motel rooms shall comply with all city, county and state building codes and ordinances applicable to the ownership and operation of the building in which the rooms are located. Rooms shall comply with the applicable NFPA (Fire) 101, 2003 or latest edition, Chapters 28/29 specifications and American with Disabilities Act (ADA) guidelines. All rooms shall be located in a single hotel or motel. d. Vendor should have a certain amount of ADA rooms available upon request. e. Each room shall have a microwave and refrigerator. f. The rooms shall be smoke-free unless requested otherwise by the Veteran. g. On-site parking shall be available at no additional charge. h. Vendor shall allow the service dog / pet of the Veteran or Veteran s Caregiver to reside in the furnished room with the Veteran and/or Caregiver. Vendor shall not put Veteran or Veteran s Caregiver under any stress in relation to verifying service dog. Vendor shall not cancel or delay check-in of Veteran, Veteran s Caregiver and service dog due to attempting to verify service dog status. If Vendor has any questions, they should contact the on-call Social Worker. 4) ROOM RESERVATIONS: a. Room reservations, room occupancy and room payment will be on the following basis: (1) The Contractor shall allow the occupancy of the room covered by this contract only upon the specific individual referral by authorized personnel of the Overton VA Medical Center. The VA will inform the Contractor of each individual s name and the arrival date at the Contractor s motel/hotel. (2) Services shall be performed Sunday through Saturday for beneficiaries of the Overton VA Medical Center. The Contractor shall use its existing check in and checkout times. (3) Personnel of the Overton VA Medical Center who are authorized to provide referrals to the contractor will identified following award of the contract and shall be accessible to the contractor from 8 a.m. to 4:30 p.m. a. The authorized dates and length of stay will be documented on the hotel voucher issued by the authorized personnel and presented to the Contractor by the VA referred beneficiary. In the event that a VA referred beneficiary refuses to leave the room, the Contractor shall immediately notify authorized personnel. Also, in the event that a VA referred beneficiary checks out of the hotel/motel without prior notice from the VA the Contractor shall immediately notify authorized personnel at the VA. b. The Contractor shall submit an invoice to the VA for payment of actual meals, and room nights used monthly in arrears. Invoices are due by the 15th of the month following previous month s services. The invoice shall contain the contract number, name of beneficiary and dates of lodging. c. VAMC After Hours Instructions: If a Veteran presents to the hotel and there is no reservation for the Veteran, please call the our AOD (Administrative staff on duty in the ER) at 318-990-5516 or the On-call Social Worker at 318-230-3193. 5) MEALS: Breakfast and Dinner will be available for Veterans who are receiving Chemo, Radiation or who have been approved for 4 or more nights. Approval for meals will be noted on the Hoptel Voucher generated by Overton Brooks VA Medical Center, (OBVAMC) staff. In addition, approval for meals for Veterans who do not meet the qualifications above will be reviewed and approved/denied by designated staff in VA Social Work Service on a case by case basis. All meals provided by OBVAMC must be clearly listed on the voucher with the correct dates of each meal. The hotel cannot accept emails or faxes or phone calls for additional meals. Meals are not authorized for caregivers. The Lodging Hotel will review the Hoptel Voucher form generated by OBVAMC staff for meal approval and if approved, Hotel Lodging staff will provide the Veteran with a breakfast and dinner coupon at check-in. The hotel will clearly print the Veteran s name, room number and date that the meal coupon is valid on the coupon prior to giving to the Veteran. OBVAMC, will only pay for meals provided to approved Veterans. The coupons will be for one breakfast and one dinner for the Veteran for each day of lodging. Veterans will only receive a breakfast coupon on the day of check out. In addition to the Veterans signature obtained when purchasing a meal, the hotel employee accepting it is also required to sign the coupon. The hotel will submit all signed redeemed coupons OBVAMC with their monthly invoice. Breakfast buffet items can/will include: scrambled eggs, bacon or sausage, hash brown potatoes or grits or biscuits and gravy, toast, cereal, assorted Danishes, muffins, assorted fruit, yogurt, milk, coffee, water, and juice. Specialty items on specific days: chicken and waffles, shrimp and grits. Not all items listed will be out on the buffet at all times. Some items will rotate/replace each other throughout the week. BREAKFAST will include coffee, soda or water. DINNER: Veteran selects dinner combo off Lodging Hotels VAMC menu . Dinner will include tea, soda or water. The cost to the VAMC Shreveport will be $7.50 per Veteran for breakfast and $13.00 per Veteran for dinner. 6) PATIENT EMERGENCIES: In the event a VA referred beneficiary is in an emergency situation of any kind, the Contractor shall contact the 911 emergency medical services to respond to the emergency. Once the emergency has ended the Contractor shall notify the authorized personnel at the VA of the situation. 7) ROOM MAINTENANCE: Daily room maintenance shall include cleaning of baseboards and corners, all wall edges shall be free of dirt, dust, and debris. Glass and mirrors shall have no traces of smudges or water. Carpets shall not be torn and free of dirt debris litter and other foreign matter and shall be securely installed to avoid tripping hazards. Restrooms shall be cleaned daily and disinfected. All linen shall be changed and cleaned each day. 8) Pest Management: There shall be no observation of roaches, ants, or any other pest/vermin that will cause un unhealthy environment. 9) GENERAL BUILDING REQUIREMENTS AND SPECIFICATIONS: a. Heating and Cooling Requirements shall be in compliance with ADA and/or State of LA Laws. (1) All cooling and heating devices and appliances, including but not limited to furnaces, fireplaces, electrical baseboard heaters and water heaters, shall be an approved type in good and safe working order and shall meet all installation and safety codes. (b) Telephone Service Requirements: (1) Contractor shall provide local area, touch tone, telephone service (no deposit required) for each room. The telephone service shall be capable of long-distance telephone service when long distance calls are placed using a telephone calling card or other telephone charge method. c. Fire Safety: (1) Buildings in which space is offered shall be evaluated in accordance with latest editions of the NFPA 101, LIFE SAFETY CODE and AMERICAN DISABILITIES (2) As required by code, smoke detectors/alarms shall be installed in each room, office, storage area, hallways and other common areas. a. Wheelchair Accessibility: Common areas such as hallways shall provide for accessible path of travel for wheelchairs. b. Elevators: If elevators are a part of the room complex, they shall conform to the latest requirements of the American National Standard A171 and A171a Safety Code for Elevators, except for elevator emergency recall and shall be inspected and maintained in accordance with American National Standard A17.1, Inspector s Manual for Elevators or equivalent local code. The elevators shall also comply with local codes and ordinances. c. To be eligible for award under this solicitation, the Contractor must propose for use a facility that is compliant with the requirements of the Hotel and Motel Fire Safety Act of 1990 (Public Law 101-391) (15 U.S.C. 2201 et.seq.) and is listed on the US Fire Administration Hotel and Motel National Master List found at http://www.usfa.fema.gov/hotel/search.cfm and the System for Award Management (SAM) database found at https://www.sam.gov/portal/public/SAM/ or by calling 1-866-606-8220. Offers proposing facilities that are not listed will be rejected as non-responsive. In addition, the Contractor facility must be compliant with the requirements of the Americans with Disabilities Act (Public Law 101-336) (42 U.S.C. 1210 et.seq.). 10) INSPECTION AND ACCEPTANCE: The Government reserves the right to fully inspect the Contractor lodging facility by a team of VA contract and safety team members at any time. Evaluation team will complete an inspection prior to the award of the contract. 11) STATEMENT: The C&A requirements do not apply, and that a Security Accreditation Package is not required. 12) UNAUTHORIZED STAYS/LODGING: Any stays without prior authorization from VA authorized personnel will not be paid. The contract period of performance for the base year is intended from October 1, 2021 to September 30, 2022 and three additional option years from October 1, 2022 through September 30, 2025. The Overton Brooks VAMC will take occupancy of the rooms no later than October 1, 2021 and rooms must be in full compliance with all requirements of the contract by that date. As used in this clause (1) Contract financing payment has the meaning given in FAR 32.001; (2) Designated agency office means the office designated by the purchase order, agreement, or contract to first receive and review invoices. Facsimile, email, and scanned documents are not acceptable electronic forms for submission of payment requests; (4) Invoice payment has the meaning given in FAR 32.001; and (5) Payment request means any request for contract financing payment or invoice payment submitted by the contractor under this contract. (b) Electronic payment requests. Except as provided in paragraph (e) of this clause, the contractor shall submit payment requests in electronic form. Purchases paid with a Government-wide commercial purchase card are considered to be an electronic transaction for purposes of this rule, and therefore no additional electronic invoice submission is required. (c) Data transmission. A contractor must ensure that the data transmission method and format are through one of the following: (1) VA s Electronic Invoice Presentment and Payment System at the current website address provided in the contract. (2) Any system that conforms to the X12 electronic data interchange (EDI) formats established by the Accredited Standards Center (ASC) and chartered by the American National Standards Institute (ANSI). (d) Invoice requirements. Invoices shall comply with FAR 32.905. (e) Exceptions. If, based on one of the circumstances in this paragraph (e), the Contracting Officer directs that payment requests be made by mail, the Contractor shall submit payment requests by mail through the United States Postal Service to the designated agency office. Submission of payment requests by mail may be required for (1) Awards made to foreign vendors for work performed outside the United States; (2) Classified contracts or purchases when electronic submission and processing of payment requests could compromise the safeguarding of classified or privacy information; (3) Contracts awarded by contracting officers in the conduct of emergency operations, such as responses to national emergencies; (4) Solicitations or contracts in which the designated agency office is a VA entity other than the VA Financial Services Center in Austin, Texas; or (5) Solicitations or contracts in which the VA designated agency office does not have electronic invoicing capability as described above. (End of Clause) VAAR 852.219-74 LIMITATIONS ON SUBCONTRACTING MONITORING AND COMPLIANCE (JUL 2018) (a) This solicitation includes VAAR 852.219-10 VA Notice of Total Service- Disabled Veteran-Owned Small Business Set-Aside. (b) Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6, or the limitations on subcontracting requirements in the FAR clause, as applicable. The Contractor is advised that in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) retained by VA to assist in assessing the Contractors compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to Contractors offices where the Contractors business records or other proprietary data are retained and to review such business records regarding the Contractors compliance with this requirement. (c) All support contractors conducting this review on behalf of VA will be required to sign an Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement to ensure the Contractors business records or other proprietary data reviewed or obtained in the course of assisting the Contracting Officer in assessing the Contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. (d) Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the Contractor to protect proprietary information as required by FAR 9.505-4, Obtaining access to proprietary information, paragraph (b). The Contractor is required to cooperate fully and make available any records as may be required to enable the Contracting Officer to assess the Contractors compliance with the limitations on subcontracting or percentage of work performance requirement. (End of Clause) VAAR 852.270-1 REPRESENTATIVES OF CONTRACTING OFFICERS (JAN 2008) The contracting officer reserves the right to designate representatives to act for him/her in furnishing technical guidance and advice or generally monitor the work to be performed under this contract. Such designation will be in writing and will define the scope and limitation of the designees authority. A copy of the designation shall be furnished to the contractor. BASIS OF AWARD This Request for Quote (RFQ) is issued in accordance with FAR 12 and in conjunction with Simplified Acquisition Procedures FAR 13.5. Evaluation of price and other factors will be performed in accordance with procedures outlined in FAR 13.106-3. A single award will be made to the offeror who represents the Best Value on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors; considering price, technical approach, and past performance. Price: The contracting officer will evaluate the offered price to determine whether it is fair and reasonable in accordance with FAR 13.106-3. The total evaluated price is the quoter s price to include all options. Options will be evaluated at time of award. Quoters complete and return with quote Price/Cost Schedule on Pages 3-5 of this solicitation. Technical Capability: The technical evaluator will evaluate the offerors quote to fulfill this requirement. The offeror will show its understanding of the requirement and the viability of its quote by demonstrating its plan of action to provide this service, demonstration of knowledge of the service and ability to perform by providing a detailed plan of equipment and personnel required to fulfill the need and the procedures you will use while executing the requirement. The quoter should include necessary information regarding the quoter s ability to perform all requirements outlined in the solicitation. In no case, shall statements such as we comply with the requirements of the contract or its equivalent be acceptable to meet the requirements of this request for quote.
