Description

Revised to add a list of questions and answers. Revised to correct referenced clauses. This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice.� This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.� The solicitation is issued as Request for Quotes (RFQ) 246-21-Q-0093. This procurement is unrestricted under NAICS code 561621 with a size standard in millions of dollars $22.0. Contractor shall provide a firm, fixed-price for fire monitoring, inspections, maintenance, emergency service calls and technical support at the Lawton Indian Hospital, Lawton, Oklahoma in accordance with the Statement of Work. The period of performance for this requirement is 8/1/2021 � 7/31/2022. The closing date for receipt of quotes is July 26 2021 at 6pm CST.� Offers shall be submitted to the Oklahoma Area Indian Health Service, 701 Market Drive, Oklahoma City, Oklahoma 73114 or emailed to jeffrey.earnest@ihs.gov . The offer shall be addressed to Jeffrey Earnest, Contract Specialist, showing the solicitation number, and your name and address.� No phone inquiries! CLAUSES INCORPORATED BY FULL TEXT: Please note that due to character limitation, full text of the Federal Acquisition Regulation (FAR) and Health and Human Services Acquisition Regulations (HHSAR) can be accessed on the Internet at http://www.aquisition.gov� The provisions of 52.212-1, Instructions to Offerors-Commercial Items (Jan 2017), applies to this acquisition. In addition to the information required in FAR 52.212-1, Contractors shall provide the following: (1) Firm, fixed-price quote, (2) Specifications that are being quoted and (3) past-performance.� Award decision shall be based on lowest technical accepted price. Vendor is still required to meet all specifications in the Scope of Work The provision at FAR 52.212-2, Evaluation�Commercial Items (Oct 2014), applies to this acquisition. The contract shall be awarded to the Lowest Priced Technically Acceptable (LPTA) offeror. The technical evaluation shall be a determination based on information furnished by the vendor in the submitted proposals. The Government is not responsible for locating or securing any information which is not identified in the offer. The Government reserves the right to make an award without discussions. The clause at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (Feb 2021), applies to this acquisition.
