Description
This is a SOURCES SOUGHT only pertaining to item(s) for the Aleda E. Lutz VA Medical Center located in Saginaw, Michigan. Information collected during this Sources Sought may be used in a set aside. If a solicitation is issued, the Government will do so in accordance with Federal Acquisition Circular (FAC) 2021-06. The North American Industry Classification System (NAICS) number is 339115. The NAICS size is 1,000 employees. Any contractor that believes they are capable and desires to claim preference for small business status must be registered with the SBA at http://web.sba.gov/pro-net/ and meet the requirements of FAR 19.102. Any contractor that believes they are capable and desires to claim preference for veteran owned small business status must be registered with the VIP at https://www.vip.vetbiz.gov as an SDVOSB or VOSB. A Contractor that proposes to provide a supply that is not manufactured by their organization (nonmanufacturer) shall meet the requirements of 52.219-33. Contractors that deem themselves capable of meeting the requirement shall provide the below information to Kellie Konopinski, Contracting Officer, at matthew.durden@va.gov no-later-than Wednesday, July 21, 2021 at 5:30PM, EDT, referencing (36C25021Q0925). Responses shall include: (1) Business Name and Address (2) GSA Contract Number, if applicable (3) Point of Contact Name, Phone Number and E-mail Address (4) DUNS and NAICS code (5) Business Size SMALL or LARGE (6) Type of Business: service-disabled veteran owned, veteran owned small business, 8a, HUBZone, woman-owned, etc. (7) A concern that is not a manufacturer of the supply shall provide the manufacturers name and size standard. If the concern is the manufacture and the sole distributor, please provide documentation to that effect. (8) Contractor must confirm they can provide ALL items on the SOW. Contractor must be registered with https://www.sam.gov To be considered SDVOSB/VOSB, must be registered in VIP (https://www.vip.vetbiz.gov/). Supplies are to include only TAA compliant items. Items that are not TAA compliant must be clearly identified by the Contractor should the contractor choose to offer a quote if a RFQ is published. FOB is Destination. Delivery is to: Department of Veterans Affairs Aleda E. Lutz VA Medical Center 2830 McCarty Road Saginaw, MI 48603-2441 Requirement: General: Currently, the Aleda E. Lutz VA Medical Center (herein after referred to as Saginaw VAMC) is adding newly built Saginaw North VA Clinic. The new Optometry clinic which requires twelve (12) fully equipped eye lanes to perform comprehensive eye examinations and care for patients. Delivery Location: 2830 McCarty Road, Saginaw, MI 48603-2441. Delivery Times: The contractor shall be responsible for delivery and installation of all equipment provided. The current eye lanes will be provided back later within a year of delivery. Delivery times shall be Monday through Friday from 08:00 to 15:30 (8:00AM to 3:30PM). Delivery must be coordinated with the Leasing Management Service Jennifer Richardson( Jennifer.Richardson7@va.gov) to ensure the space and building is ready for equipment delivery and installation. Delivery truck must be equipped with a lift gate. Normal business hours for acceptance of deliveries are 8:00am 3:30, Monday through Friday excluding Federal Holidays. Deliveries must be coordinated in advance. All items included in the requirement shall be shipped and delivered to site at the same time. If deliveries must be broken into several shipments due to size, this must be coordinated in advance with the Leasing Manager. Failure to coordinate the shipment with the Leasing Manager may result in failure to deliver and cause re-scheduling of delivery/freight. Scope: The Contractor shall provide and be responsible for the technical assistance, development, and generation for final specifications. The Contractor shall provide delivery and installation will be required with dates. The Contractor provide maintenance manuals for all products specified. Coordinate and provide project management of all products and services to manage, design, order, ship, deliver and install new equipment from the manufacturer until final acceptance The Contractor will provide the necessary equipment and accompanying software to the Saginaw VAMC for the newly installed twelve (12) new fully equipped eye lanes at the Saginaw North VA Clinic. Brand name or equal is required for all equipment. The requirement is for a comprehensive and whole solution to the requirement. The Contractor is required to be an expert in the field of Optometry equipment and requirements and as such the Contractor is required to include any items not specifically listed. Examples include installation expenses, VA staff training, equipment mounts, display charts, and special equipment wiring requirements. The current eye lanes will be turned in within a year of delivery. Training for VA Employees: Comprehensive training for VA staff is required. Examples include eye technicians, optometrists, ophthalmologist, and biomedical engineers. 6. Security Requirements: The contractor will have no access to patient records.
