- This Request for Offer (RFO) is for Helicopter charter service for Juneau, Alaska to various sites in Southeast Alaska to be completed as required in the Statement of Work that is located in Section C of the attached RFO#-� 697DCK-21-R-00202.� This acquisition is set-aside for small business The principal North American Industry ClassificationSystem (NAICS) code applicable to this effort is: 481211 ( Nonscheduled Chartered Passenger Air Transportation), with the size standard of 1,500 employees. The resulting award will be Firm-Fixed Price Contract with a base year plus four option years. No SITE VISITS are scheduled/required�at this time. Questions or comments pertaining to this RFO must be submitted in written form via email to Saman Sraon, Contracting Specialist at Samandeep.ctr.sraon@faa.gov no later than 1700 Pacific Time on Friday, 08/13/2021. Questions and responses that clarify the RFO may be provided to all Offerors; if this information is provided to all Offerors, the source of the questions will not be identified. Responses to all questions and any necessary amendments made to the RFO will be made by 1700 Pacific Time on Monday,�08/02/2021. All responses must reference the solicitation number (697DCK-21-R-00202) and be submitted to the following email address: Samandeep.ctr.sraon@faa.gov SUBMISSION REQUIREMENTS: EACH OFFEROR SHALL SUBMIT AS A VALID OFFER TO THIS REQUEST FOR QUOTES: 1. Cover letter stating that no exceptions are taken to any specifications requirements or contract terms and conditions, or a detailed summary of all exceptions taken 2. Signed Standard Form 33 - SOLICITATION, OFFER ,ANY AMENDMENT AND AWARD 3. Part I, Section B - PRICE SCHEDULE 4. Part IV, Section K - REPRESENTATIONS, NDAA , CERTIFICATIONS AND OTHER STATEMENTS 5. Congratulations 8(a) letter from the Small Business Administration (if applicable) 6. Past Experience containing at least two (2) projects of similar nature with Point of Contact information for confirmation of completion of work.� Contractors must be registered with the System for Award Management, www.sam.gov on the date established for receipt of offers (08/13/2021).
