Description
Please note that this is not a request for quotes or proposals. VHA Reginal Procurement Office (RPO) 2 is currently conducting a market survey for qualified firms capable of providing service Surgical Instrument Containers and baskets. The North American Industry Classification Systems (NAICS) is 337122. Responses to this notice must be submitted in writing (email or fax) and must be received no later than July,28 2021 @ 10AM EST. Interested parties MUST provide company/individual name, a capability statement, examples of same or similar work performed at other facilities, DUNS number and address, point of contact and social-economic. Interested parties who do not submit the above information on or before the response due date and time will not be considered for this procurement. STATEMENT OF WORK FOR DOMICILIARY SPECIALIZED WARDROBE FURNITURE Introduction and Background The VA New York Harbor Healthcare System is dedicated to providing quality health care to veterans using the abilities of all employees supported by our commitment to education and research. We are recognized locally, regionally, and nationally as a leader in quality patient care, positive customer service, medical/allied health education, health-related research and employment opportunities. We seek to be the provider of choice of veterans and the community by offering an efficient, integrated quality health care system capable of providing a full range of primary, specialty and chronic health care services in a system that is readily accessible and responsive to change. The Domiciliary Residential Rehabilitation Treatment Program has a mission to give at-risk veterans a chance by enabling them to take part in residential and rehabilitative therapeutic community activities. This is for purposes of enabling the veterans to achieve their optimum functionality levels and to return to independent living in their respective communities. The Domiciliary Care program is a long-term rehabilitation program which provides veterans a chance to live in the community while learning skills and avoiding a return to homelessness. Residents of the Domiciliary Care program take part in a wide range of rehabilitation services such as addiction counseling, spiritual, behavioral, physical, psychosocial, dietary, vocational and occupation therapy. Objective The VA NYHHS Domiciliary is seeking sources to provide the following specialized wardrobe furniture and services. CA HPL Three Drawer Wardrobe with Pull Out Desktop Dimension: 36 W x 24 D x 84 H Product Code: CA214-06(SP) (2) Half doors with metal door pulls (3) Drawers on lower half of unit with modern metal drawer pulls Pull-out desktop on full extension tracking Upper cubby on top of unit Laminate Finishes Furniture assembly and installation Assemble, and install required wardrobe furniture per VA Domiciliary requirements All wardrobe furniture are to be anchored or attached to rated framing. It is the responsibility of the Contractor to provide a safe support system for the wardrobe furniture. It is the sole responsibility of the Contractor to verify all dimensions and take field measurements to confirm furniture installation can be safely achieved. Test wardrobe furniture to ensure they are functioning effectively. Provide a furniture maintenance guide with all manufacturer specific operating and maintenance instructions for all furniture installed. Provide manual with basic system overview and instructions for routine use. Provide all labor, materials, transportation and equipment to complete wardrobe furniture delivery and installation. Removal & Disposal of existing furniture Disassemble/Un-install and remove old/existing furniture in accordance with safety regulations. Remove all hardware or any hardware in place that are part of the old or existing furniture. The vendor shall be dispose the old/existing furniture in accordance with safety regulations. The contractor shall perform cleanup at any time when work area becomes cluttered and/or restricts traffic. Nevertheless, work area must be cleaned at the end of the each work shift. After completions, re-install all removed items. Exceptional care must be taken to assure tidiness of work. In the performance of all work, the workers must be incompliance with safety regulations governing worker safety. Place of Delivery The furniture will be delivered to VA Medical Center Domiciliary Service 800 Poly Place Brooklyn, NY 11209 Product Installation/Assembly The vendor shall be responsible for determination of and compliance with federal and state or local code requirements, design data, and other factors necessary to deliver the items at the desired location. The new wardrobe furniture s will not require any new construction, space, special wirings etc. Recalls The Vendor shall immediately notify all affected VA Medical Centers and designated POC in the event of a product recall, removal, required adjustment, or required modification that is suggested or mandated by the vendor, distributor, manufacturer, or any regulatory or government agency. The following information shall be included in this notification: A complete item description and identification (to include lot and serial numbers where applicable); Administrative identification data including order numbers and order date; Reasons for recall, removal, adjustment, or modification; and Instructions for appropriate corrective action. Information Security and Privacy Training All contractor employees and subcontractors participating under this contract or order, to provide installation, training or service repairs are required to complete the VA's Privacy and Information security training requirement VA Privacy Training for Personnel without Access to VA Computer Systems  or Direct Access or Use to VA Sensitive Information Contractors must provide signed certifications of completion to the COR during each year of the contract.  This requirement is in addition to any other training that may be required of the contractor and subcontractor(s).
