National Ave, Bldg. 111 Milwaukee, WI 53295-0001 Identify purchase request number: 695-21-1-969-0055 Brief Description of Supplies/ Services required and the intended use/Estimated Amount: Clement J. Zablocki VA Medical Center located in Milwaukee, Wisconsin has a sole source requirement for trios spine and trauma table, which will be awarded as a firm fixed price contract. This procurement is intended to be utilized by Operating Room in order to treat trauma patients that require orthopedic, radiolucent, and surgical procedures. Estimated Amount: $X.XX Unique characteristics that limit availability to only one brand, with the reason no other supplies or services can be used: Mizuho trios spine and trauma table allows the surgeon to maneuver patients in several various positions for added comfort. Some positions consist spinal surgery top with advanced control base, prone (Trendelenburg with leg boards), prone with leg sling, and patient rotation supine to prone (360 degrees). In addition, to the open frame design, ergonomic padding can be added to relieve pressure on the patient s chest, hips, and thighs. Furthermore, the open frame reduces vena cava compression, minimizes epidural venous bleeding, and improves visibility at the surgical site. While there are other brands with interchangeable tops, research using ECRI.org Operating Table comparison chart indicated that other brands did not meet all required characteristics, such as a patient weight capacity of 650lbs when using the imaging/radiolucent table top and spine surgical tops and a lateral tilt capability of 25 degrees. These characteristics are required in order to accommodate bariatric patients at the medical center. Description of market research conducted and results or statement why it was not conducted: Original Equipment Manufacturer (OEM) for this requirement is Mizuho Orthopedic Systems Inc. who is considered a small business (SB) per System for Award Management (SAM) Reps & Certs. Mizuho Orthopedic Systems Inc. sole authorized Service-Disabled Veteran-Owned Small Businesses (SDVOSB) is Veterans Healthcare Supply (VHSS), Inc. who is VIP verified. VIP query on July 16, 2021 keyword Mizuho revealed zero Service-Disabled/Veteran-Owned Small Businesses (SD/VOSB). Unable to locate part numbers on schedule; therefore, items will be considered open market. Sources sought notice 36C25221Q0384 was posted from 03/18/2021 to 03/24/2021 that rendered one interested party who is VIP verified as an SDVOSB, Veterans Healthcare Supply (VHSS), Inc. Intent to sole source notice 36C25221Q0957 was posted for a minimum of three days with no additional interested parties. Notice of intent is to negotiate a limited source contract with Veterans Healthcare Supply (VHSS), Inc. for Mizuho Trios Spine and Trauma Tables. Market research indicates that this procurement shall be sole sourced to Veterans Healthcare Supply (VHSS), Inc. who is VIP verified as an SDVOSB IAW FAR 6.302-5(b)(6), FAR 13.106-1(b)(1), FAR 19.1406, 13 CFR § 121.406(b)(1), and 38 U.S. Code § 8127(b).
