Description
This is a SOURCES SOUGHT only pertaining to item(s) for the Aleda E. Lutz VA Medical Center located in Saginaw, MI. Information collected during this Sources Sought may be used in a set aside. If a solicitation is issued, the Government will do so in accordance with Federal Acquisition Circular (FAC) 2005-83. The North American Industry Classification System (NAICS) number is 337214. The NAICS size is 1250 employees. Any contractor that believes they are capable and desires to claim preference for small business status must be registered with the SBA at http://web.sba.gov/pro-net/ and meet the requirements of FAR 19.102. Any contractor that believes they are capable and desires to claim preference for veteran owned small business (VOSB) status must be registered with the VIP at https://www.vip.vetbiz.gov as an Service Disabled VOSB or VOSB. A Contractor that proposes to provide a supply that is not manufactured by their organization (nonmanufacturer) shall meet the requirements of FAR 52.219-33. Lutz VA Medical Center 2830 McCarty Rd Saginaw, MI 48602 New Building for the Audiology & Optometry Center of the Aleda E. Lutz VAMC Saginaw, Michigan Modular Clinical Casework Package June 11, 2021 STATEMENT OF WORK The government is in the process of a new construction building requiring new eye clinic exam specialty modular clinical casework. This scope of work will be performed at the New Building for the Audiology & Optometry Center of the Aleda E. Lutz VAMC, 2830 Mc Carty Road, Saginaw, Michigan 48603. This is a specialty casework requirement.  It is essential that the casework required be designed, delivered, staged/moved, and installed concurrently with the activation phases in order to coordinate the timely delivery and installation of casework essential for government operational use no later than the expected dates of occupancy of the project.  Construction completion anticipated December 2021.  Casework shall be required to be installed once construction is completed and can begin installation December 13, 2021 and shall be completed no later than December 17, 2021.  In the event of a construction delay, it is expected that the vendor shall begin delivery and installation within 2 weeks (10 business days) of completion notification and complete delivery and installation within 15 working days following the first delivery and installation date. SUBMISSION OF QUOTATION Quote Submission: The contractors shall submit complete emailed electronic copy portable document of: Volume 1: (technical) drawings, (floorplans, isometrics, shop drawings, typicals etc.) complete manufacturer s product specifications Bill of Materials tagged per CLIN/ floorplan department and room etc. quantity of man hours for labor/installation project management plan product literature project staffing plan physical finish samples sustainability certification documents complete testing results / certificates mock up samples (list any needed here) warranty. Self-certifying statement confirming ability to meet project deadlines Volume 2: (pricing) price quote with separate line for labor/installation Bill of Materials with subtotals and tagged per CLIN/ floorplan Pricing must be within the established IDIQ Pricing. VA will review the submissions to determine that these meet all requirements. Upon award, the VA will require that the bill of materials is tagged by room for ease of installation. A project manager and or installation foreman must be onsite during installation. Upon completion of the project, as-built drawings in AutoCAD format must be produced for VA use. 1. Industrial design services related to new construction casework requirements in accordance with new construction space requirements. Participation in progress meetings is required as necessary to ensure a successful casework project solution. These meetings will occur with the VA Interior Designer to assure the requirements are met. 2. Delivery, staging, and installation/configuration of casework for the new construction (including all fasteners and fixtures required for assembly), and warranty service in accordance with new construction space requirements that will result in a turn-key casework solution in accordance with the final casework design (subject to final working/construction drawings). Installation must be performed by a licensed and bonded installation company, including any proprietary certifications necessary to maintain product warranties. 3. All finishes, material selections, final casework layouts shall be selected and approved by VA Interior Designer. This includes, but is not limited to, the following: Trucks cannot be left unattended and must be removed from unloading area after unloading. Drop shipping is not acceptable. All deliveries must be received and handled by installers. Aleda E. Lutz VAMC will not provide on-site storage for casework requirements. Necessary storage must be included in proposal. Any damage to the Aleda E. Lutz VAMC facility incurred through the installation process must be repaired within 30 days at the vendor s expense. Vendor is responsible for notifying VA Interior Design contact in advance of arrival and departure, providing review of all product and site conditions before sign off. Work is to be performed between 8:00am and 4:30pm weekdays unless specifically coordinated otherwise.
