Description
Please note that this is NOT a request for quotes or proposals, the Government is seeking information for market research purposes only. VHA Reginal Procurement Office (RPO) 2 is currently conducting a market survey for qualified firms capable of providing GI Electrosurgery units. The North American Industry Classification Systems (NAICS) is 339112. Responses to this notice must be submitted in writing (email or fax) and must be received no later than July 30 ,2021 @ 10AM EST. Email: LATEISHA.ROBINSON@VA.GOV and Fax: 718-741-4722. No telephone inquiries will be accepted. Interested parties MUST provide company/individual name, a capability statement, examples of same or similar work performed at other facilities, DUNS number and address, point of contact and social-economic. Interested parties who do not submit the above information on or before the response due date and time will not be considered for this procurement. Please review the attached Statement of Work attached herein. (gi4000 Electrosurgery Unit) Objectives Purchase of additional gi4000 Electrosurgery Unit for the new GI suite at the Brooklyn Campus Item/Quantity Description Reference# Item Description Technical Specification (Detailed description) QTY UM G1110001 gi4000 Electrosurgery Unit gi4000 electrosurgery unit* (Includes: submersible footswitch, power cord, lavage bottle holder, positional accessory clips & manual). Designed specifically for flexible endoscopy with Argon, Monopolar, Bipolar and Lavage functions all in one, compact unit Microprocessor controlled outputs in every mode Argon beam that has robust arc length without sacrificing limited tissue depth 3 EA Installation Vendor will be responsible for assembly and installation. Place of Performance / Delivery Delivery to the Brooklyn VA: Department of GI (4-123) NYHHCS Brooklyn Campus Warehouse: 800 Poly Place Brooklyn, NY 11209 Travel/Parking The contractor will be responsible for their travel. As stated earlier, due to other construction projects and limited parking available at the site, contractor must coordinate any parking needs with the assigned COR and the Police Service. Details of any vehicles entering the site must be provided in advance. (Driver s Full Name, Driver s License Number, Vehicle Description, Vehicle License Plate Number) Government-Furnished Property Contractor will not need any government furnished equipment or property. VA Service Point of Contacts VA Patient Privacy and Network Infrastructure Any contractors that will come on site for install will not need access to VA network. All contractors must comply with VA regulation and complete the following training: VA Privacy Training for Personnel without Access to VA Computer Systems or Direct Access or use to VA Sensitive Information. Reference the attached Handbook 6500.6 Appendix A - Checklist for Information Security in the initiation phase of acquisitions. The system does not transmit or maintain electronic Protected Health Information. A Manufacturer Disclosure Statement for Medical Device Security (MDS2) form in not applicable. A VA Directive 6550 Pre-Procurement Assessment form is not applicable. Recalls The Vendor shall immediately notify all affected VA Medical Centers and designated POC in the event of a device recall, removal, required adjustment, or required modification that is suggested or mandated by the Vendor, distributor, manufacturer, or any regulatory or government agency. The following information shall be included in this notification: A complete item description and identification (to include lot and serial numbers where applicable); Administrative identification data including order numbers and order date; Reasons for recall, removal, adjustment, or modification; and Instructions for appropriate corrective action. Defective Items/Warranty 1 year warranty.
