- FINAL Amendment 0003 - �FINAL PWS 7-19-2021 have been uploaded and attached. FINAL ATTACHMENT A - 7-19-2021 has been attached and CLIN 0001 and CLIN0002 for all years has been changed to state��appropriate regulator to include a conserving device if clinically indicated� to match PWS.� Extension has been granted due to wording change on these CLINS. Proposals shall be due August 6, 2021 4:30pm. ******************************************************************************************************************************** Amendment 0002 July 19, 2021 Question/Answers uploaded. Proposal Due Date Extended to August 2, 2021. Attachment A has been updated to allow for additional extension.� Amendment 0001 June 23, 2021. Changes have been made to the original solicitation 36C24421R0068/36C24421R0068_1. Changes include PWS updates, Proposal Instructions, Evaluation Criteria and Basis for Contract Award.�Attachment D-Present and Past Performance Questionnare has also been updated. Due to these changes in Attachment D, an extension has been granted and this solicitation will close on July 26, 2021 at 4:30pm. An SF30 has been attached to include an updated RFP, and attachments�for proposals.� The updated RFP and attachments shall supersede the previous solicitation posted June 1, 2021.
