Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-06. This solicitation is set-aside for SDVOSB. The associated North American Industrial Classification System (NAICS) code for this procurement is 334290, with a small business size standard of 750 Employees. The FSC/PSC is 6515. The Network Contracting Office (NCO) 9, 1639 Medical Center Parkway, Suite 400, Murfreesboro, TN 37129 is seeking to purchase Replace & Upgrade Nurse Call System Brand Name Only Rauland Responder 5. All interested companies shall provide quotations for the following: Supplies/Services Line Item Description Quantity Unit of Measure Unit Price Total Price 0001 Rauland Responder 5 System 0002 Responder 5 Applications 0003 Training 0004 Warranty Statement of Work/Description of Services: Background: The VA Memphis has a requirement for continuation of replacing and upgrading Nurse Call System for Ward 4 and 5. Scope/Performance Expectation: The vendor shall provide all labor, materials and equipment and resources necessary to perform the installation of a complete Rauland Responder 5 system in patient wards 4 and 5, plus the Responder 5 Applications. The system will consist of an enhanced single patient station with staff assist and code blue in each of the patient rooms. The dual patient rooms will get the dual patient station with a separate staff assist/ code blue for each room. The enhanced pillow speaker will provide TV control capabilities (channel control and direct dialing for television channels) and toilet buttons for summoning staff directly. The pillow speaker shall be programmable and compatible with all normal healthcare television brands. The patient station shall be equipped with a quarter inch jack for equipment alarm, in addition provide a 2-jack station for extra alarm inputs. Provide a bath station for every bathroom, and a standard waterproof pull cord for shower locations. Specific Tasks Supporting Capability: to upgrade the Nurse Call system for the facility to one uniform system for an enterprise level system. This will allow for the Nurse Call systems to be upgraded to the latest technology and allow for a seamless communication for Nurse Call throughout the facility. This procurement will be an upgrade of existing systems, which shall reutilize existing infrastructure such as conduit, back boxes, raceways, low voltage lighting power supplies, control relays, cross room television control cable, local paging amplifier and speakers and AC power circuits where possible. Existing system must remain in place while installing the new system. GRAY MARKET AND COUNTERFEIT ITEMS (MAR 2020) (DEVIATION) (a) No used, refurbished, or remanufactured supplies or equipment/parts shall be provided. This procurement is for new Original Equipment Manufacturer (OEM) items only. No gray market items shall be provided. Gray market items are OEM goods intentionally or unintentionally sold outside an authorized sales territory or sold by non-authorized dealers in an authorized sales territory. (b) No counterfeit supplies or equipment/parts shall be provided. Counterfeit items include unlawful or unauthorized reproductions, substitutions, or alterations that have been mismarked, misidentified, or otherwise misrepresented to be an authentic, unmodified item from the original manufacturer, or a source with the express written authority of the original manufacturer or current design activity, including an authorized aftermarket manufacturer. Unlawful or unauthorized substitutions include used items represented as new, or the false identification of grade, serial number, lot number, date code, or performance characteristics. (c) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system, verified by an authorization letter or other documents from the OEM or by an intermediary distributor authorized by the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. Addendum to 52.212-2 EVALUATION SIMPLIEFIED ACQUISTION PROCEDURES Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. Basis for Award. The Government will issue a purchase order to the responsible quoter whose quotation conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following are the decision factors: price. The Government will evaluate the price by adding the total of all line item prices. The Total Evaluated Price will be that sum. Evaluation Approach. The Government will evaluate quotations using the comparative evaluation process outlined in FAR 13.106-2 (b) (3), where quotations will be compared to one another to determine which provides the best benefit to the Government. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation but is not required to do so. Each response must meet the minimum requirements of the solicitation. The Government is not requesting or accepting alternate quotations. The award will be made to the response most advantageous to the Government. Responses should contain your best terms, conditions.
