Description

SOLICITATION INSTRUCTIONS W81K00-21-Q-0173 Attached is a solicitation for Zenith III 440 Thompson Pneumatic Hylo Chiropractic Adjusting table, Delivery, and Installation for General Leonard Wood Army Community Hospital, Fort Leonard Wood, Missouri, 65473. In order to be considered for the proposed contract the appropriate person must: 1. Fill out pricing as appropriate for each of the items listed in the CLIN structure: CLIN 0001 provide pricing for each unit and the total amount. CLIN 0002 provide pricing if this is applicable. CLIN 0003 provide pricing for each unit and the total amount. CLIN 0004 provide pricing for each unit and the total amount. � Please note there are special instructions in the 52.212-1 ADDENDUM. These must be adhered to or the vendor will not be considered for award. � Please read the 52.212-1 ADDENDUM and the 52.212-2 ADDENDUM carefully. 2. Proposed vendor MUST fill out block 17a with the company CAGE Code, company name and company address. 3. Proposed vendor MUST SIGN and DATE in blocks 30 a, b, and c. Please return the completed solicitation via email to fredicinda.d.jones.civ@mail.mil NLT 11 August 2021 by 10:00 a.m., Central Standard Time as noted in Block 8 of the solicitation. If you have any questions please contact via email ONLY at fredicinda.d.jones.civ@mail.mil. All questions must be submitted via email by 03 August 2021 at 10:00 a.m. CST to fredicinda.d.jones.civ@mail.mil; answers to all questions will be provided no later than 3:00 PM CST on 05 August 20201. Quotes must be received by 11 August 2021 at 10:00 a.m. Central Standard Time (CST). Untimely received quotes will NOT be considered for award.
