- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6 and using the procedures of FAR Part 13--Simplified Acquisition Procedures, as supplemented with additional information included in this notice. The solicitation number is W81K00-21- Q-0250 and it is issued as a Request for Quote (RFQ). This announcement constitutes the only RFQ that will be posted. A separate written solicitation document will be not be issued. This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-05 and the Defense Federal Acquisition Regulation Supplement Publication Notice (DPN) 20210224. This solicitation is to obtain one (1) Semen Analyzer for Raymond W. Bliss Army Health Center, Fort Huachuca, Arizona The North American Industrial Classification System (NAICS) code for this procurement is 334516 Analytical Laboratory Instrument Manufacturing with a size standard of 1,000 employees. This acquisition is 100% Small Business SET ASIDE. Solicitation incorporates by reference FAR 52.212-1, 52.212-2, 52.212-3, 52.212-4 & 52.212-5 and Addendums are attached. The Point of Contact (POC) for this RFQ is Emerita Torres. Offers and Questions shall be submitted to the POCs via e-mail at emerita.torres.civ@mail.mil. Quoters shall reference the following FAR Provisions and their Addendums to prepare and submit their offer: FAR Provision 52.212-1-Instructions to Offerors�Commercial Items and its Addendum; and, FAR Provision 52.212-2 Evaluation�Commercial Items and its Addendum.
