- SUBJECT: PACS door replacement, Louis A Johnson VA Medical Center, Clarksburg, WV 26301 DISCLAIMER: This Sources Sought is issued solely for information and planning purposes and does not constitute a solicitation. Neither unsolicited proposals nor any other kind of offers will be considered in response to this Sources Sought. Responses to this notice are not offers and will not be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this Sources Sought. All information received in response to this Sources Sought that is marked Proprietary will be handled accordingly. Responses to the Sources Sought will not be returned. At this time, questions concerning the composition and requirements for a future RFP will not be entertained. SUBJECT: The Louis A Johnson VA Medical Center is conducting market research in order to provide PACS door replacement. OBJECTIVE: See Statement of Work below RESPONSE INSTRUCTIONS: Name and Address of Company Any small business designation as certified by the Small Business Administration A through description of experience in managing support requirements of this nature. All information regarding Capabilities Statements or any other proprietary information relative to this RFI shall be submitted via email to danfeng.lu@va.gov no later than 3:00 pm EST on Friday, August 6, 2021. Louis A. Johnson VAMC 1 Medical Center Drive Clarksburg, WV 26301 Statement of Work It is the general intention of the Louis A. Johnson VAMC located at 1 Medical Center Dr. Clarksburg WV 26301 to acquire contractor services for the purpose of installing door hardware and a PACS reader on the double entry doors of the old loading dock. The PACS reader is necessary to enhance security for VA Police and control access into the building. PACS EQUIPMENT 1 PACS Card Reader Multi-authentication style card reader that includes: Qty 1 LENEL MT15 style reader (no multi-authentication) Lot control equipment needed to support added card reader and magnetic locks 1 Door Hardware Equipment Door Hardware equipment that includes: Qty 2 Magnetic Door Locks Qty 1 Request to Exist Pushbutton station for free egress through the door Qty 1 PIR Motion detector allowing handsfree egress through the door Qty 1 MC25 Relay to interface with door control function 1 Lot Plenum rated cable needed to support new equipment TECHNICAL SERVICES INCLUDED IN THE ABOVE 1 Lot Technical Services Onsite Technical service to: Install new door hardware on existing door Install cable needed between new door location and existing system controller Install controller modules and power supplies as needed to support this added door Terminate equipment to cables, program new equipment into the existing system, test and certify proper operation 1 Lot Warranty One (1) Year factory sponsored equipment warranty One (1) year on-site labor warranty Scope of Work: Phase 1 Mechanical Installation Contractor shall review project with VAMC and schedule time for this work to be performed. Contractor shall utilize VAMC containment equipment as needed to perform this work. Work schedule and use of containment equipment (if needed) to be approved by VAMC in advance. Contractor shall furnish and install new cable and suspend cable within concealed areas, furnish and install surface raceway were cable may be exposed, mount new equipment to existing door frame, modify door frame as needed, terminate new cables to new equipment, terminate new cables to new control equipment, program, test and document proper operation of this new equipment on the existing system. New equipment bill of material is shown in PACS Equipment section. If more equipment is discovered or requested by Owner, contractor will prepare cost information to add equipment to this project for review/approval by Owner. All new cable will be labeled as needed. All work shall be performed in a workmanlike manner, all trash will be removed daily, technicians shall report in and out as they perform work. This work shall be performed during normal business hours per schedule approved in advance by Owner. Phase 2 Testing and Certification: Contractor shall program and test operation of new equipment on the existing system. Contractor shall provide testing documents to Owner. Contractor and Owner are to participate with walk-thru on the installation of new devices. All new equipment shall be covered under the existing Service Agreement
