Description
This is�Update No. 25�-�Updated as of 2�Aug 2021�(12:48�pm HST).� �Refer to underlined areas for the updates.��The following summarizes the update: Updates PHNSY & IMF indoor mask requirement in area of �substantial� or �high� community spread/transmission as designated by the Centers for Disease Control and Prevention (CDC) regardless of an individual�s vaccination status.� Furthermore, during periods of �low� or �moderate� community spread/transmission as designated by the CDC, fully vaccinated individuals are not required to wear face masks in PHNSY & IMF indoor setting while unvaccinated individuals will continue to wear face masks.� Notwithstanding, fully immunized individuals must continue to comply with other COVID-19 mitigation measures, as well as other Commanders' mask wearing requirements, where applicable, e.g., when boarding a ship or visiting another command that continue to impose a mask requirement. Updates criteria restrictions from entering DOD workplace (no. 3, 4, 6, & 7) where the contractor will be required to immediately notify their PHNSY & IMF sponsor who will notify COVID-19 Tracking Team and provide further instructions. Attaches �Daily Self-Screening Assessment for PHNSY & IMF Contractor Employee Entering DOD workplace Revision 13.�� The daily self-screening assessment is to be used by the employees to help them understand the requirements of entering DOD workplace.� This notice is to all prime contractors and their subcontractors with contracts that were awarded and/or are administered by Pearl Harbor Naval Shipyard and Intermediate Maintenance Facility (PHNSY & IMF).� PHNSY & IMF, hereby, provides further clarification pertaining to the State of Hawaii�s 10-day self-quarantine.� The State of Hawai�i has a mandatory 10-day self-quarantine for individuals traveling from out of�O�ahu. The State has exceptions to the 10-day self-quarantine, including Persons traveling to the State to perform critical infrastructure functions.� The State of Hawaii has provided waivers to individuals regarding the 10-day quarantine when arriving from out of state.� Inter-island travelers effective 15�June 2021. Effective 8 July 2021, travelers who are fully vaccinated in the United States or its Territories may enter Hawaii on domestic flights without pre-travel testing/quarantine after the completion of their vaccination.� The vaccination record document must be uploaded to State of Hawai�i Safe Travels�online system and printed out prior to departure and the traveler must have a hard copy in hand when arriving in Hawai?i.�The State of Hawai�i will also accept vaccine records that are digitally validated by Digital Health Pass Partners. �For more information visit�hawaiicovid19.com/travel�and�register�with the�State of Hawai�i Safe Travels�online system. Travelers who are not fully vaccinated will have to test negative for COVID-19 no more than 72 hours (calculated using the local time of the flight) before departing on their flight to State of Hawai�i�under the State of Hawai�i�s pre-travel testing program to qualify for an exemption from the state�s quarantine requirement.��The program requires travelers aged five and older to take a Nucleic Acid Amplification Test (NAAT) from a Clinical Laboratory Improvement Amendments (CLIA) certified laboratory within 72 hours from the final leg of departure and produce a negative result. Only test results from trusted testing partners approved by the Hawai�i Department of Health are being accepted. Travelers wishing to bypass the state�s 10-day mandatory quarantine must have their negative COVID-19 test results prior to departure for the State of Hawai�i.� For more information visit�hawaiicovid19.com/travel�and�register�with the�State of Hawai�i Safe Travels�online system. Although the State of Hawaii will exempt travelers from the 10-day quarantine as listed above, PHNSY & IMF guidance does not allow for travelers (including the workforce supporting the defense industrial base) coming from off-island to enter the DOD workplace for�10�days after arrival, UNLESS contractor employee meets the exception identified in below �CRITERIA RESTRICTIONS FROM ENTERING DOD WORKPLACE�� Section ��Criteria 1 and 2.� ************************************************************************************************************************************************** CRITERIA RESTRICTIONS FROM ENTERING DOD WORKPLACE Contractor employees meeting ANY of the seven (7)�criteria below are restricted from entering the Department of Defense (DOD) workplace:� 1.� Contractor employees traveling from outside of the island of O�ahu are restricted from entering the DOD workplace for�10�days after arrival regardless of the State of Hawai�i�s 10-day quarantine exemptions listed in above section UNLESS one of the following A, B, or C has been met: A.� Travelers obtained a Restriction of Movement (ROM) waiver from PHNSY & IMF.� Please speak to your PHNSY & IMF sponsor regarding the ROM form and process.� A ROM waiver form and additional documentation may be submitted though the Government chain of command and approved by Commander, U.S. Pacific Fleet.� ROM waivers will be submitted and processed only when, at the Government�s sole discretion, it is absolutely necessary.� Also, the traveler must be exempt from the State of Hawai�i travel-related quarantine requirements. B.� Immunized Travelers whose travel originated from outside of the island of O�ahu, to include the United States as well as international locations.� Traveler is qualified as �Immunized Traveler� by meeting the following a and b: � � ��a.� A traveler is considered immunized if he/she has received all required doses for the specific COVID-19 vaccine AND at least 14 days have passed since the final vaccine dose BEFORE the start of the self-quarantine/restriction period. Vaccine Doses:� Pfizer � 2 doses, Moderna � 2 doses, Johnson & Johnson � 1 dose.� The PHNSY & IMF sponsor will verbally determine if contractor employees meet immunized traveler definition above.� � � � b.� The immunized traveler must be exempt from the State of Hawai�i travel-related quarantine requirements. C.� The traveler has fully recovered from a confirmed laboratory diagnosis of a COVID-19 infection within three months from the scheduled ROM start date.� Also the traveler must be exempt from the State of Hawai�i travel-related quarantine requirements.�� 2. A contractor employee who is�not�a close-contact or known to be exposed to anyone displaying COVID-19 symptoms; experiences symptom(s) similar to COVID-19 for 24 hours or less, then become asymptomatic is restricted from entering the DOD workplace until the contractor employee is asymptomatic for at least 24 hours without use of fever-reducing or other symptom-altering medicines (i.e. cough medicine, etc.). � As of 5 Jan 2021, CDC list of COVID-19 symptoms include fever (? 100.4�F)/chills, cough, shortness of breath/difficulty breathing, fatigue, unexplained muscle or body aches, unusual headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, and/or diarrhea.� CDC updates this list as they learn more about COVID-19.� For current CDC list of COVID-19 symptoms, please go to https://www.cdc.gov/coronavirus/2019-ncov/symptoms-testing/symptoms.html. 3. Contractor employees with COVID-19 symptoms AND tested negative for COVID-19 AND (a) had traveled from off-island in the last 10 days AND/OR (b) had close contact with a confirmed COVID-19 case are restricted from entering the DOD workplace.� Contractor shall immediately notify their PHNSY & IMF sponsor who will notify COVID-19 Tracking Team and provide further instructions.� The contractor employee may enter the DOD workplace IF at least 14 days have passed since COVID-19 symptoms first appeared AND the contractor employee has been asymptomatic for at least 24 hours, without the use of fever-reducing or other symptom altering medicines (i.e. cough medicine, etc.) excluding loss of taste/smell, OR confirmed in writing by a healthcare provider that the employee may return back to work.� A COVID-19 negative test result will not allow the contractor employee to enter back to the DOD workplace earlier than the 14-day requirement specified above.��� As of 5 Jan 2021, CDC list of COVID-19 symptoms include fever (? 100.4�F)/chills, cough, shortness of breath/difficulty breathing, fatigue, unexplained muscle or body aches, unusual headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, and/or diarrhea.� CDC updates this list as they learn more about COVID-19.� For current CDC list of COVID-19 symptoms, please go to https://www.cdc.gov/coronavirus/2019-ncov/symptoms-testing/symptoms.html.� 4.��Contractor employees who have other persons living/staying in their house/hotel room that identified as having COVID-19 are restricted from entering the DOD workplace for 14 days.� Contractor shall immediately notify their PHNSY & IMF sponsor who will notify COVID-19 Tracking Team and provide further instructions. The 14-day period starts from the day the contractor employee learns of the cohabitant�s positive testing for COVID-19.� A COVID-19 negative test result will not allow the contractor employee to enter back to the DOD workplace earlier than the specified 14-day requirement. 5.���Contractor employees testing positive or deemed presumptively positive for COVID-19 by a primary health provider or under the authority of a public health official are restricted from entering the DOD workplace. �Contractor shall immediately notify one of the following Contracting Officers:� a.� Vivian Fukumitsu, 808-368-0881 b.� Janet Baker, 808-347-8785 c.� Jackie McGlone, 808-368-0147 The contractor shall conduct the contact tracing in coordination with the Contracting Officer, and further coordination with Hawai`i Department of Health. �In addition, contractor shall ensure the impacted DOD workplace is cleaned and disinfected in accordance with Centers for Disease Control and Prevention (CDC) guidance for the safety of the personnel.� The Government may require the contractor to provide a list of individuals (both contractor and Government employees) that the contractor employee has been in close contact (as defined below) within the past�48�hours�prior to their first exhibiting COVID-19 symptoms and/or the date of testing positive or determined as presumptively positive for COVID-19, whichever occurred first.� For current CDC list of COVID-19 symptoms, please go to https://www.cdc.gov/coronavirus/2019-ncov/symptoms-testing/symptoms.html.� Close contact is defined as someone who: � � �a. Was within 6 feet of a COVID-19 infected person for a cumulative total of 15 minutes or more over a 24-hour period* starting from 48 hours before illness onset (or, for asymptomatic COVID-19 infected individuals, 48 hours prior to positive specimen collection) until the time the COVID-19 positive person is isolated regardless of whether the individuals were wearing face coverings, and/or � � � �b. Had direct contact with secretions (being coughed on, sneezed on, kissing, sharing utensils, etc.) from a COVID-19 infected person.� *Individual exposures added together over a 24-hour period (e.g., three 5-minute exposures for a total of 15 minutes).� Contractor employee experiencing symptoms of fever (? 100.4�F), cough, chills, sore throat, shortness of breath/difficulty breathing, new loss of taste or smell, unexplained muscle pain/aches, and/or unusual headache and tests positive or deemed presumptively positive for COVID-19 shall not enter the DOD workplace until at least 14 days have passed since the date of the contractor employee�s last positive COVID-19 test, AND at least 24 hours have passed without the contractor employee experiencing or showing COVID-19 symptoms and without taking any symptom altering medications (e.g., Tylenol� for fever, cough medicine, etc.).��For current CDC list of COVID-19 symptoms, please go to https://www.cdc.gov/coronavirus/2019-ncov/symptoms-testing/symptoms.html.� If the contractor employee tests positive, but never develops any COVID-19 symptoms, the contractor employee may enter the DOD workplace after 14 days have passed since the date of the contractor employee�s last positive COVID-19 test. For current CDC list of COVID-19 symptoms, please go to https://www.cdc.gov/coronavirus/2019-ncov/symptoms-testing/symptoms.html.� �The contractor employee does NOT have to be tested negative before entering the DOD workplace after 14 days have passed.� A COVID-19 negative test result will not allow the contractor employee to enter back to the DOD workplace earlier than the 14-day requirement specified above. Any information related to COVID-19 cases shared by PHNSY & IMF with the contractor is not publicly releasable.�� 6.���Contractor employees who have been in close contact (as defined below) with an individual confirmed COVID-19 case within 48�hours prior to (a) the individual first exhibiting COVID-19 symptoms�and/or (b) the date the individual tested positive or was determined as presumptively positive for COVID-19, whichever occurred first, are restricted from entering the DOD workplace.� For current CDC list of COVID-19 symptoms, please go to https://www.cdc.gov/coronavirus/2019-ncov/symptoms-testing/symptoms.html.� Contractor shall immediately notify their PHNSY & IMF sponsor who will notify COVID-19 Tracking Team and provide further instructions.� Such contractor employees shall not return to the DOD workplace for 14 days from the date of last contact with the COVID-19 positive/presumptive positive test of the employee they came into contact with.� If the contractor employee who was in close contact with a confirmed COVID-19 case is also experiencing COVID-19 symptoms the contractor employee may enter the DOD workplace IF at least 14 days have passed since COVID-19 symptoms first appeared AND the contractor employee has been (a)� asymptomatic for at least 24 hours, without the use of fever-reducing or other symptom-altering medicines (i.e. cough medicine, etc.) excluding loss of taste/smell, OR (b) confirmed in writing by a healthcare provider that the employee may return back to work.� A COVID-19 negative test result will not allow the close contact individual to enter back to the DOD workplace earlier than the 14-day requirement specified above.� Close contact is defined as someone who: � � �a. Was within 6 feet of a COVID-19 infected person for a cumulative total of 15 minutes or more over a 24-hour period* starting from 48 hours before illness onset (or, for asymptomatic COVID-19 infected individuals, 48 hours prior to positive specimen collection) until the time the COVID-19 positive person is isolated regardless of whether the individuals were wearing face coverings, and/or � � � �b. Had direct contact with secretions (being coughed on, sneezed on, kissing, sharing utensils, etc.) from a COVID-19 infected person.� *Individual exposures added together over a 24-hour period (e.g., three 5-minute exposures for a total of 15 minutes).� Any information related to COVID-19 cases shared by PHNSY�& IMF�with the contractor is not publicly releasable.�� 7.��� Contractor employees who are Person Under Investigation (PUI) for COVID-19 (as defined below) are restricted from entering the DOD workplace.� Contractor shall immediately notify the Contracting Office with the number of PUI for COVID-19.�� Refer to �DAILY COVID-19 REPORT REQUIREMENT & GUIDELINES��below.� Furthermore, Contractor shall immediately notify their PHNSY & IMF sponsor who will notify COVID-19 Tracking Team and provide further instructions.� PUI is defined as an individual:� (a)� who has been directed/ordered by a healthcare professional to get tested for COVID-19 or (b)� currently being tested for COVID-19 as directed/ordered by a healthcare professional and/or waiting for his/her test results. If contractor employee tested negative for COVID-19, but reported experiencing COVID-19 symptoms, the contractor employee shall be asymptomatic for at least 24 hours, without the use of fever-reducing or other symptom altering medicines (i.e. cough medicine, etc.) AND at least 14 days have passed since symptoms first appeared to enter DOD workplace.��For current CDC list of COVID-19 symptoms, please go to https://www.cdc.gov/coronavirus/2019-ncov/symptoms-testing/symptoms.html.� If contractor employee tested negative for COVID-19 and had close contact* with a COVID-19 positive/presumptive positive person, the contractor employee shall not return to the DOD workplace for 14 days from the date of last contact with the COVID-19 positive/presumptive positive person they came into contact with. If a contractor employee is NOT experiencing COVID-19/flu-like symptoms AND has NO KNOWN close contact* with a COVID-19 positive case and is being tested for COVID-19 for other reasons (e.g. employee request, pre-surgery testing, employer�s company-wide/random testing for prevention measure, etc.), the contractor employee is NOT restricted from entering the DOD workplace while awaiting test results. The contractor employee may continue to report to the DOD workplace as normal.� Contractors DO NOT have to report employees as �Person Under Investigation� or �PUI� under this category to the Contracting Office.� *Close contact is defined as someone who: a. Was within 6 feet of a COVID-19 infected person for a cumulative total of 15 minutes or more over a 24-hour period** starting from 48 hours before illness onset (or, for asymptomatic COVID-19 infected individuals, 48 hours prior to positive specimen collection) until the time the COVID-19 positive person is isolated regardless of whether the individuals were wearing face coverings, and/or � b. Had direct contact with secretions (being coughed on, sneezed on, kissing, sharing utensils, etc.) from a COVID-19 infected person.� **Individual exposures added together over a 24-hour period (e.g., three 5-minute exposures for a total of 15 minutes).� ***************************************************************************************************************************************************** TEMPERATURE MONITORING Whenever practicable, PHNSY & IMF will conduct temperature monitoring of contractor employees working in PHNSY & IMF buildings (including offices and shops) and vicinity to ensure the health and safety of all personnel.��The temperature monitoring will be conducted at the beginning of each shift, or soon as practicable thereafter, on a daily basis, or at the start of training, meeting or similar event.� Separately, all ships (i.e. Surface Ships, Submarines, and Barges) will continue their screening for COVID-19 at the brow prior to boarding.�The temperature monitoring is very simple and your full cooperation is expected.� Some important information about the screenings include the following: The temperature monitoring uses non-contact infrared thermometers that DO NOT touch you. If a contractor employee has a temperature reading greater than 100.4�F, the PHNSY & IMF personnel assigned to conduct the temperature monitoring will immediately contact the Contracting Officer or Contracting Officer Representative who will then contact the company�s point of contact.� In addition, PHNSY & IMF will record your CAC/badge number.� The contractor employee will be directed to leave the shipyard.� Refer to �CRITERIA RESTRICTIONS FROM ENTERING DOD WORKPLACE� Section� for contractor employees experiencing COVID-19 symptoms.� Body temperatures greater than�100.4�F�is an indication that the body is fighting a disease. Not all COVID-19 cases have an associated fever. Additionally, a fever can be associated with several other causes to include other medical conditions, severe trauma or injury, and medications.�� **************************************************************************************************************************************************** MASK WEARING REQUIREMENT In locations designated as �substantial� or �high� area of community transmission as defined by the CDC, all individuals are required to wear a face mask regardless of vaccination status while in an indoor setting on installations and in other facilities owned or leased or otherwise controlled by the DoD, to include government vehicles.� Thus, PHNSY & IMF face mask use requirements will correspond with the current community transmission data indicated by the CDC.� Data on levels of community transmission for Hawaii and specifically Honolulu County (Island of O�ahu) can be viewed on the CDC COVID-19 Data Tracker website found at https://covid.cdc.gov/covid-data-tracker/.� Instructions are as follows: once on the site under �Your Community� click �County View.�� Under the �State� drop down menu, select �Hawaii� and under the �County or metro area� drop down menu, select �Honolulu County.�� The applicable community transmission level (low, moderate, substantial, and high) and corresponding color (blue, yellow, orange and red) will appear for Honolulu County. This is the only location applicable to PHNSY &IMF. � Community transmission data updates are weekly, typically covering the period of Wednesday through the following Tuesday on the CDC website. If there is no update on a Tuesday, the community transmission indicated on the site is the most current. �Additional community transmission information is available through Shipyard employee notification/information phone number at (808) 473-9000.� Contractors are responsible for ensuring contractor employees including subcontractor employees are aware of the most current community transmission spread designations (low, moderate, substantial and high) by the CDC as applicable to PHNSY &IMF. �Additionally, CDC guidance on the definitions of low, moderate, substantial, and high spread transmission levels can be found at: https://www.cdc.gov/mmwr/volumes/70/wr/pdfs/mm7030e2-H.pdf. Each Commanding Officer maintains the right to implement command-specific COVID-19 mitigations including mask wearing to maintain a safe work place. When aboard a ship or submarine or visiting another command that requires masks, contractor employees shall comply with that command�s specific policy.� Please note that there may be policy differences between the various commands.� In addition, if a contractor�s policy requires that masks to be worn at PHNSY & IMF, contractor employees should comply with their company�s mask policy. � Individuals are expected to comply with all federal, state, local, and CDC face mask guidance when not at PHNSY & IMF facilities. Masks recommended by the CDC include non-medical disposable masks, masks made with breathable fabric (such as cotton), masks made with tightly woven fabric (i.e., fabrics that do not let light pass through when held up to a light source), masks with two or three layers, and masks with inner filter pockets. Novelty or non-protective masks, masks with ventilation valves, or face shields are not authorized as a substitute for masks. The below guidelines shall be followed regarding cloth face coverings: Masks shall fit snugly against the side of the face, be secured with ties or ear loops, allow for breathing without restriction, and be able to be laundered and machine dried.� They should be comfortable as they may need to be worn for prolonged periods of time.� All masks must have a minimum of two layers of tightly woven material, must fit snugly over the bridge of the nose and around the chin/neck area, and must fully cover the mouth and nose to prevent droplets from escaping.� Further, bandana-type coverings are prohibited because a snug fit over the chin/neck area cannot be achieved.� Bandana type coverings are prohibited even if they are made from two layers of cloth material.� Single layer neck gaiters worn by themselves are prohibited as they do not contain moisture in exhaled breath, but they may be worn on top of cloth face coverings as required (NAVADMIN 298/20).� Consistent with NAVADMIN 194/20, multi-layered neck gaiters may be worn in lieu of cloth face coverings, but only in particular circumstances where ear straps or neck straps on face coverings interfere with, or are otherwise not suitable for, the operational environment, e.g., work settings where ear and neck straps present safety concerns.� Masks shall not be shared.� Masks shall be in good taste and not offensive in appearance. Masks coverings are not a substitute for any job-specific personal protection equipment. IMPORTANT: Cloth masks shall be washed daily. Per CDC guidance, used cloth face coverings should be washed at the end of each day on a gentle cycle and dried at medium heat. �Refer to CDC Guidance at https://www.cdc.gov/coronavirus/2019-ncov/prevent-getting-sick/diy-cloth-face-coverings.html. IMPORTANT: At the request of security personnel, you shall lower your masks covering to verify your identity at base access points, CIA gates, or any other occasion that may require showing your full face for identification purposes. Face masks and coverings with one-way valves embedded in them allow the wearers� respiratory particulates to escape through the valves, thus defeating the purpose of capturing particulates and droplets when wearers breathe, talk, cough, etc. �For this reason, face masks and coverings of any kind that have one-way valves embedded in them are prohibited for wear in the DOD spaces. Masks or commercial disposable masks are required aboard ships and submarines at PHNSY & IMF unless deviations are specifically authorized for tasks where wearing masks are impractical (e.g. work required the use of Personal Protective Equipment such as respirators). during periods of low or moderate spread of community transmission for Honolulu County per the CDC COVID-19 Data Tracker, individuals who are fully immunized (i.e., 14 calendar days have passed since administration of the final dose of COVID-19 immunization series) are not required to wear face masks indoors at PHNSY &IMF facilities.� If a contractor�s policy requires that masks to be worn at PHNSY & IMF, individuals should comply with their company�s mask policy. �Individuals shall continue to adhere to federal, state, local and CDC face mask guidance when not on DoD installations. �Notwithstanding, fully immunized individuals must continue to comply with other COVID-19 mitigation measures, as well as other Commanders' mask wearing requirements, where applicable, e.g., when boarding a ship or visiting another command that continue to impose a mask requirement. �Individuals who are not fully immunized will continue to wear the face masks indoors regardless of the Hawaii and Honolulu County community transmission indicated by the CDC. Fully immunized individuals can continue to wear face masks at their discretion. Contractor employees going shipboard without a mask will be turned away by ship�s force. �Furthermore, all contractor employees are required to wear masks or disposable masks outdoors when 6 feet of physical (social) distancing is unlikely or difficult to maintain, except as provided in the exceptions listed below. Exceptions: At PHNSY & IMF facilities, individuals who are FULLY IMMUNIZED (14 days past final dose of COVID-19 vaccination) are no longer required to wear masks indoors or outdoors.� Each Commanding Officer maintains the right to implement command-specific COVID-19 mitigations including mask wearing to maintain a safe work place. When aboard a ship or submarine or visiting another command that requires masks, contractor employees shall comply with that command�s specific policy.� Please note that there may be policy differences between the various commands.� In addition, if a contractor�s policy requires that masks to be worn at PHNSY & IMF, contractor employees should comply with their company�s mask policy. �The State of Hawai'i and City and County of Honolulu still require that masks be worn�indoors�regardless of immunization status. � Please comply with all state and local COVID-19 requirements including mask wearing guidance when not on PHNSY & IMF facilities. When working alone in an office with floor-to-ceiling walls with a closed door; For brief periods of time when eating or drinking while maintaining six foot physical (social) distancing between you and others in accordance with CDC guidelines; When smoking in a designated smoking location while maintaining six foot physical (social) distancing between you and others in accordance with CDC guidelines; When face masks are required to be lowered briefly for identification or security purposes; When necessary to reasonably accommodate an individual with a disability; when underlying health conditions exist that prohibit mask wearing, and the individual is in possession of medical documentation from a military or licensed civilian healthcare provider outlining the condition and the restriction it places on mask wearing; When outdoors, including for physical fitness while maintaining appropriate six foot physical (social) distancing between you and others in accordance with CDC guidelines; When clear or unrestricted visualization of verbal communication is required for safe and effective operations (e.g., emergency dispatch, police/fire/emergency services); other examples include: (a) When working outdoors such as crane teamwork, building staging, forklift operations, or other command and control type environments where efficient, clear communication is needed for safety and efficiency of response and (b) When operating a forklift during work that requires the operator to repeatedly ingress and egress between the interior and exterior of a building where physical distancing can be maintained by the forklift operator. When an individual is unconscious incapacitated, or otherwise unable to remove the mask without assistance; When conducting training during which face mask wearing is not feasible, such as swim qualification, amphibious, or aquatic training events; or where mask wearing impedes safety, such as live-fire, chemical, biological, radiological and nuclear, or other high risk training events where masks create a hazard. When necessary to reasonably accommodate participation in religious service.� ************************************************************************************************************************************************ DAILY COVID-19 REPORT REQUIREMENT & GUIDELINES Implementation of the daily COVID-19 report requirement and guidelines below will begin 13 April 2020.��This requirement is only for contractors with contractor employees who access DOD workplace and/or PHNSY employees who access the contractor�s facility.��Prime contractors shall report the following information for all contractor personnel (including subcontractor personnel):� 1.� Total number of Persons Under Investigation (PUI) for COVID-19.� Refer below for definition.� 2.� Total number of Positive COVID-19 Cases.� Refer below for definition.� �Further breakdown of the following information to be provided for the Positive COVID-19 Cases:� � a.
