Description
This request for information (RFI) is being conducted to identify potential sources to provide immunization with influenza vaccine (flu shot) to personnel located at Tobyhanna Army Depot, 11 Hap Arnold Blvd. Tobyhanna, PA 18466. The influenza vaccinations will be supplied and administered at no cost to Tobyhanna Army Depot. This RFI is issued solely for planning purposes and to identify potential sources that would like an opportunity to administer influenza vaccinations at Tobyhanna Army Depot and does not constitute a solicitation. Responses will be treated as information only and will not be used as a proposal.� The information submitted in response to this RFI may be used to help the Government further define its requirements. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. The Government does not intend to make an award on the basis of this RFI.� Companies responding to this RFI are advised their response does not ensure participation in future solicitations or contract awards.� The government will not reimburse participants for any expenses associated with their participation in this RFI. All information received in response to this RFI that is marked proprietary or confidential will be handled accordingly.� Responses to this RFI that indicate that the information therein is proprietary, or represents confidential business information, will be received and held in confidence. Proprietary information submitted, as part of responses to the RFI, should be clearly identified and it will be treated as such. The Government reserves the right to follow up on a one-on-one basis with RFI respondents to gain additional information or clarity about information submitted. Specific Requirements This request for information (RFI) is being conducted to identify potential sources to provide immunization with influenza vaccine to personnel located at Tobyhanna Army Depot, 11 Hap Arnold Blvd. Tobyhanna, PA 18466. The influenza vaccinations will be supplied and administered at no cost to Tobyhanna Army Depot (TYAD). This announcement is only open to companies/providers in the local commuting area (within one hundred (100) miles of TYAD). Potential sources will be required to provide immunization with influenza vaccine (flu shot) to personnel located at Tobyhanna Army Depot. Flu shots will be administered one to two days per week with an anticipated start date the week of October 18, 2021 continuing for up to 2 weeks. Flu Shots will be administered to personnel at no cost to Tobyhanna Army Depot. No contract award will be made, this RFI is solely for scheduling purposes. Tobyhanna Army depot will be free of any and all liability involved with administering flu shots. �Providers will be required to sign a release of liability, releasing Tobyhanna Army Depot from any liability associated with the services being provided prior to being scheduled to administer flu shots. Providers will be responsible for providing their Companies� release of liability forms. Providers must accept all federal health plans and Tricare. Providers will bill the employee�s medical insurance directly for all costs associated with the flu shot and administration fees. No cost will be billed to Tobyhanna Army Depot for any reason. All costs are the responsibility of the Employee�s insurance provider and the Employee. Individuals administering the vaccine must be licensed to do so in the state of Pennsylvania. At a minimum, three individuals licensed to administer flu shots should be present and three support personnel should be present to process paper work and keep long lines from forming. There will be no exceptions to the number of personnel required to be present. � An area to administer the vaccine at Tobyhanna Army Depot will be provided along with tables and chairs. Providers will supply their own storage for the vaccine. Providers will also provide a way to safely dispose of and remove all medical waste. All medical waste will be disposed of off TYAD premises. Providers must be able to submit proof of liability insurance covering the services being provided. Any Provider that accepts a scheduled Flu Clinic day will be required to sign a Memorandum of Understanding (MOU) between Tobyhanna Army Depot and the Provider. The MOU will highlight the responsibilities of both parties involved, such as schedules, insurance coverages, personnel coverage, liability and compliance with TYAD COVID-19 mitigation measures. This MOU must be signed by the provider within two days of receipt or the providers scheduled days will be forfeit to another provider. Flu shot clinics will be thoroughly advertised throughout the Tobyhanna Army Depot. However, there are no guarantees as to the number of customers who may appear on any scheduled day, which can range from zero to well over one hundred. Providers must return for all of their schedules dates regardless of previous turnout. Tobyhanna Army Depot reserves the right to limit the number of providers who will be scheduled to administer flu vaccines to two, based on facilities limitations and administrative requirements such as background checks. Any non-credential carrying visitor to Tobyhanna Army Depot must submit an ELTY 648-C for that will be provided and pass a background check prior to admittance to Tobyhanna Army Depot.
