Description
NOTICE OF INTENT TO SOLE SOURCE Natus Electroencephalography machine, monitoring system with review station, and compatible supporting Natus Server with NeuroWorks 9 software. The Lebanon VA Medical Center, 1700 South Lincoln Ave, Lebanon, PA has a requirement for brand name Natus Electroencephalography (EEG) machine. The EEG machine is required by the medical center and must be compatible with the Natus Server with NeuroWorks 9 software. The Department of Veteran Affairs has only located one vendor (Natus Medical Inc) capable of providing a nationwide standard for Neurology equipment and this addition will be keeping in compliance with this standard. The Contracting office intends to issue a sole source contract to that vendor. This notice is being posted so that any unknown vendors who are capable of providing this item may come forward and express their interest and capability in providing the Natus EEG machine to the VA. If your company/organization is capable of providing the required item, please provide your entities capability statement to LaShawn Knight or Andrew Taylor, Contracting Officer by 15:00. Monday, August 6, 2021 via email to lashawn.knight@va.gov and andrew.taylor3@va.gov . In addition to your capability statement please also provide any applicable distribution agreements regarding Natus, the size and socioeconomic status of your company, and any other information pertinent to this requirement. DEPARTMENT OF VETERAN AFFAIRS STATEMENT OF NEED TITLE OF PROJECT: EEG System GENERAL INFORMATION Scope of Need: The vendor shall provide all resources necessary to accomplish the deliverables described in this statement of need (SON), except as may otherwise be specified. The Lebanon VA Medical Center has a requirement for a Natus Electroencephalography machine, monitoring system with review station, and compatible supporting Natus Server with NeuroWorks 9 software. TYPE OF CONTRACT: Firm Fixed Price. GENERAL REQUIREMENTS The contractor shall provide all labor, equipment, tools, materials, supervision, documentation, services, and other items necessary to deliver and install the requirements in this SON. The Lebanon VA Medical Center (LVAMC) will be conducting internal EEG testing services and will require a Natus EEG machine, monitoring system with review station and supporting Natus Server with Neuro Works 9 software. DELIVERY: Contractor will provide delivery of the EEG Machine, monitor station and server according to the schedule outlined by the COR on the Delivery Order to the specified location: Lebanon VA Medical Center 1700 South Lincoln Street, Lebanon, PA 17042 Delivery to the facilities shall be coordinated at least 72 hours prior to the required delivery date with the Contracting Officer Representative (COR). ASSEMBLY/INSTALLATION: Installation shall include: Delivery of and setup of the EEG Machine, Monitoring Station and Server TEST AND ACCEPTANCE: Contractor shall test all equipment and software after assembly. CONTRACTOR RESPONSIBILITIES: Contractor shall provide qualified personnel to deliver, install and setup EEG Machine, Monitoring Station and Server. Contractor is responsible for ensuring the proper disposal of all waste generated from assembly activities. Contractor is responsible for securing all materials, equipment and tools while on government property or in government facility. Government is not liable for any lost or stolen items that are not properly secured. GOVERNMENT RESPONSIBILITIES: The Government shall provide the following: Provide site access and escorts through as needed. Provide adequate space for delivery and assembly/installation. OTHER CONSIDERATIONS: CONTRACTING OFFICER REPRESENTATIVE (COR): The Contracting Officer Representative shall be the technical point of contact for all work-related requirements. The COR does not have authorization to change, alter, or remove any requirements stated in the performance work statement. The Contracting Officer is the only government authorized person to change the performance work statement or any resultant contract. PLACE OF PERFORMANCE: Installation of the equipment will occur at the site noted. WORK HOURS: Standard Operational Hours are 0730-1600, Monday-Friday
