- Description
- The USPFO for Puerto Rico, located at Fort Buchanan, Puerto Rico, intends to issue a Request for Quote (RFQ) to award a single firm fixed-price contract for Laboratory Services. The contractor shall provide all personnel, equipment, tools, materials, supervision, and quality control necessary to perform laboratory services as defined in the PWS.� The contractor shall provide services between the hours of 0600-1600 on Monday through Sunday, except on recognized US holidays or when the Government facility/installation is closed due to local or national emergencies, administrative closings, or similar Government-directed facility/installation closings. Performance shall be at Building 589A, Soldier Readiness Processing Site Camp Santiago Joint Maneuver Site, Salinas, Puerto Rico. The contractor shall at all times maintain an adequate work force for the uninterrupted performance of all tasks defined within this PWS when the Government facility/installation is not closed for the above reasons. When hiring personnel, the contractor shall keep in mind that the stability and continuity of the work force are essential. The North American Industry Classification System (NAICS) code for this work is 621511. This is a total small business set- aside. The small business size standard is $35 million average annual revenue for the previous three years. Non-Personal Laboratory Services: The contractor must provide all qualified personnel supervision, and transportation to execute the contract requirements and material to conduct collection, handling and analysis of laboratory specimens. Provide a description of the work to be completed and the required outcomes. The contractor shall provide qualified personnel to perform the services, and /or OSHA, HIPPA,ADA, and Privacy Act Compliant services. The contractor shall conduct and perform services being requested this PWS no later than 7:00 am on the day of event. Blood samples results shall be delivered no later than 12:30pm (noon time) same day of testing. Hematology (CBC, HCT, HGB) Urinalysis (U/A) Chemistry (Fasting Blood Sugar (FBS) Lipid Panel (CHOL, LDL, HDL, Triglycerides) Blood Type Sickle Cell Test Hemo Fractionation (SCT Positive SM) Glucose-6-phosphate Dehydrogenase (G6PD) Prostate Specific Antigen (PSA) TSH/T4 (FDME Flight Duty Medical Examination) URINE HCG (Pregnancy Test) Comprehensive Metabolic Panel Period of performance: Base Period: 15 September 2021 to 15 May 2022 Option 1: 15 May 2022 to 14 May 2023 Option 2: 15 May 2023 to 14 May 2024 Option 3: 15 May 2024 to 14 May 2025 Option 4: 15 May 2025 to 14 May 2026 The solicitation closing date is scheduled for 2:00 PM (EST) 3 September 2021. Offeror shall make a statement of his/her the avilibty to comply with this requirement. If the offer does not have the statement, the Goverment will evaluate the offer as �Does not meet requirement�.
