- This is a Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Background: The Department of Veterans Affairs Medical Center, NCO 9 Network Contracting Office, on behalf of the Nashville and York VA Medical Center is conducting market research through this sources sought notice to seek sources to identify potential experienced businesses that are capable of meeting the requirements per the Statement of Work (SOW) below. Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. NAICS code: 238350 and sized standard is $16.5 Million. STATEMENT OF WORK GENERAL INFORMATION Title: Refinish Cabinets - Fisher House 2021 2. Scope of Work: Provide new services for the above-named requirement at the Fisher House on the historic York Campus of the VA Tennessee Valley Healthcare System, 3400 Lebanon Pike, Murfreesboro, TN 37129 in accordance with the SOW, specifications, drawings and manufacturer recommendations. Contractor shall furnish all labor, material, equipment, and supervision necessary to completely refinish cabinets at the Fisher House. Work specialty includes, but not limited to, carpentry. 3. Performance Period: Complete the work required under this SOW in 45 calendar days from date of Notice to Proceed, unless otherwise stated in any written modifications required under the resulting contract and signed by the Contracting Officer (CO). B. GENERAL REQUIREMENTS 1. All written deliverables shall be phrased in layperson language. Where a written milestone deliverable is required in draft form, the VA will complete their review of the draft deliverable within _4_ business days from the date of receipt. The contractor shall have _4_ business days to deliver the final deliverable from date of receipt of the Government s comments. It is the intent of the Department of Veterans Affairs to have the contractor fully survey, investigate, and document the existing site in order to gain the knowledge and insight necessary to prepare quality, accurate documents, and perform the repairs / refurbishment in a professional and competent manner. The Information Technology Certification and Accreditation requirements do not apply, and a Security Accreditation Package is not required. Contractor will coordinate all work through the VA COR. The contractor superintendent will be on site at all times during the performance of work associated with this contract. All labor, materials, equipment to perform the job; When furnish , provide , install or similar term is used, it shall mean a complete installation, ready for use. The contractor shall identify all employees that will be working under the awarded contract. Contractor personnel shall include employees, sub-contractors, suppliers and delivery personnel entering the medical center. Those employees shall bring photo ID to obtain a VA Contractor identification badge that shall always be worn above the belt while on medical center property. Contractor s employees shall return all I.D. badges to designated personnel at the end of the period of performance of the contract. Lost badges will result in a surcharge. The contractor s safety manager is responsible for safety inspections of all contract operations. All VA and OSHA safety regulations shall be observed during the performance of work identified on this contract. Provide signage and barricades, as needed, to maintain safety and protection around project work site. The Contractor does not have assigned parking and is responsible for parking in authorized locations only. Drivers should be particularly concerned with Pedestrian traffic, yield to pedestrians in crosswalks. Seat Belt use is mandatory on all VA grounds. The Contractor shall always keep the work area, including any areas where materials/equipment is being stored, free from accumulations of waste materials. Before completing the work, the Contractor shall remove from the work and premises any rubbish, tools, scaffolding, equipment, and materials that are not the property of the Government. Normal duty hours will be 8:00 am - 4:30 pm Monday through Friday. No work shall be performed on federal holidays unless approved by COR. This project may require work to be performed outside of normal duty hours to meet stringent time requirements and minimize impact on patients and Medical Center operations. Scope of Work The contractor shall provide the specific deliverables described below within the time specified in the approved schedule. Perform work in accordance with manufacturer requirements and all applicable codes and standards, including all closeout documentation, as-built drawings, certifications, tests reports, training, warranties, manuals, etc. specified in contract documents for work at front entrance. All work must be scheduled and coordinated through the Contracting Officers Representative (COR). After work is completed all affected areas will be completely cleaned. Final inspection shall be requested from COR. All work shall be warranted for a period of 1 year. Unless otherwise stated in specifications. Specific Tasks include: Remove and clean all cabinet hardware. Clean, repair blemishes ,refinish and match existing cabinets finish, doors (68 ea.), drawers (12 ea.), refrigerator panels (84SF), laundry room doors (17 ea.), laundry room drawers ( 7 ea.), end panels (60SF), crown molding ( 85LF), flat panels (75SF) and toe base (140 LF). Clean, repair and refinish counters to like new condition (Approximately 200SF). Reinstall cabinet doors and all hardware. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written modifications to the contract by the CO. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. E. REPORTING REQUIREMENTS 1. The contractor shall provide a Daily Work Report (DWR). Report must contain forces, equipment, work performed, location of work performed, and any issues encountered. Each DWR is due the following business day. DWR s shall be submitted electronically in a format agreed upon by the contractor and COR. F. CONTRACTOR EXPERIENCE REQUIREMENTS The contractor must inform the COR and the CO when personnel are removed from the contract for any reason. Contractor must provide demonstratable documentation that cabinet refinishers have a minimum of 5 years task specific experience. If a key person becomes unavailable to complete the contract, proposed substitutions of key personnel shall be made by the contractor to the COR and CO. The COR may request contractor or contractor personnel to be removed from the site for violating VA Medical Center rules and regulations. The Contractor will provide a corrective action write-up on personnel removed that shall be approved by CO. Corrective action may include permanent removal of personnel from the premises. G. CONFIDENTIALITY AND NONDISCLOSURE 1. The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this contract, are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the contract. 2. The CO will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this contract. No information shall be released by the contractor. Any request for information relating to this contract, presented to the contractor, shall be submitted to the CO for response. 3. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO. NOTE: Respondent claiming SDVOSB and VOSB status must be registered and CVE verified in VetBiz Registry www.vetbiz.gov. Anticipated period of performance: 08/20/2021 thru 10/05/2021. Capability statement /information sought: Response to this announcement shall not to exceed 10 pages and should include the following information: (1) A tailored capability statement indicating the firm s ability to provide a product and related services; (2) The respondents DUNS number, organization name, address, point of contact, and size and type of business (e.g., SDVOSB/VOSB, etc.) pursuant to the applicable NAICS code; (3) Information should be provided electronically in a Microsoft Word or Adobe PDF format.
