Description

1 The Gulf Coast Veterans Health Care System (GCVHCS), Network Contracting Office (NCO) 16, Biloxi, MS, is issuing a combined synopsis / solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice in conjunction with procedures in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation is being issued. The solicitation number 36C25620Q1533 is issued as a Request for Quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-03 effective January 14, 2021. The NAICS code is 621910 and the size standard is $16.5M. This requirement is for a Small Business set-aside and will be firm-fixed-price. The closing date for this notice is 1:00 PM CT August 19, 2021. The requirement is for the following: The contractor shall provide non-emergent ambulance and wheelchair transport services for the Mobile Outpatient Clinic, Mobile, AL. Services shall be provided in accordance with the terms and conditions of the contract, as needed. (See Performance Work Statement for additional detailed information.) Contractor to provide services in accordance with terms and conditions of the contract for the period of October 1, 2021 through September 30, 2022, to include four (4) twelve (12) month option periods. To be eligible for award of a contract, vendor must be actively registered in the SAM database (https://www.beta.sam.gov). Vendor is also required to complete representations and certifications found in the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items. Questions regarding this solicitation must be made in writing to the Contract Specialist, Danette Rene Impey at rene.impey@va.gov, and must include the solicitation number 36C25621Q1533 on the subject line. NO TELEPHONE REQUESTS OR INQUIRIES WILL BE ACCEPTED. Submissions of questions must be received by 12:00 PM CT on August 11, 2021. No additional questions will be received after this date. All responsible offerors are invited to submit an offer which shall be considered by the agency.
