This notice constitutes amendment no. 2�to the combined synopsis/solicitation for JSC Emergency Medical Evacuation Services, 80JSC021MEDEVAC4Q. �Companies shall acknowledge all amendments�in their quote. This notice serves as the official amendment to subject synopsis/solicitation and a written amendment will not be issued.� The purpose of this amendment 2�is to:� 1. Upload the submitted Questions and Answers document, as it did not hold during the posting of Amendment 1. The due date for receipt of offers is not extended.� Companies shall provide the information stated in the synopsis/solicitation.� This notice constitutes amendment no. 1 to the combined synopsis/solicitation for JSC Emergency Medical Evacuation Services, 80JSC021MEDEVAC4Q. �Companies shall acknowledge all amendment(s) in their quote. This notice serves as the official amendment to subject synopsis/solicitation and a written amendment will not be issued.� The purpose of this amendment 1 is to:� 1. Post submitted questions and answers 2. Update Statement of Work (SOW) Section 10 to specify the due date for the�one-time onsite field research trip. The due date for this trip is March 31, 2022. The�SOW below and the SOW in the Model Contract have been updated accordingly. The due date for receipt of offers is not extended.� Companies shall provide the information stated in the synopsis/solicitation.� � This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. This solicitation is issued as a Request for Quote (RFQ) for MEDEVAC 4. The Government intends to acquire commercial services using FAR Part 12 in conjunction with FAR Part 13 - Simplified Acquisition Procedures. The resulting contract will be a Firm Fixed Price (FFP) Purchase Order. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-05. The NAICS Code and the small business size standard for this procurement are 621910 and revenue of $15 million, respectively. The Offeror shall state in their offer their size status for this procurement. All responsible sources may submit an offer, which shall be considered by the agency. Relevant Attachments: 1.�Proposal Pricing Sheet 2.�SF1449 3.�Model Contract 4.�Russia and Kazakhstan Medical Facilities 5.�NASA Historical Travel Data � FY16 - FY20 6.�NASA Actual Usage Data - 2011 - June 2021 7.�NASA Internal Risk Assessment � Through August 2020 � Attachments 4 through 7 in the solicitation are critical documents to review to understand the actual usage of the previous contracts. Please review these attachments carefully to best understand the requirements of this procurement. Attachment 4, Russia and Kazakhstan Medical Facilities, describes the known hospital and medical facilities most likely to be used by NASA to support a medical situation during NASA operations in Russia and Kazakhstan. This does not preclude other solutions provided by the Contractor. Attachment 5, NASA Historical Travel Data � FY16-FY20, states the cumulative number of international travel days broken out by year from 2016 through June 2021. The total number of travel days has been approximately 46,309 and the total number of countries visited during this time has been approximately 51 countries. Attachment 6, NASA Actual Usage Data - 2011 - June 2021, shows how many medical incidents NASA/JSC reported to the Contractor over the past 10 years. During these 10 years, there were 11 relevant cases requiring contractor support. There were no Level 4 incidents, which required air ambulance transport. There were three Level 3 cases, which the contractor provided a commercial return flight to the US for the injured/sick traveler. There were two Level 2 cases that required professional medical care in a hospital/clinic, but treated locally without transport to the US. Finally, there were six Level 1 cases, which were considered minor incidents or only required the contractor to provide advisory services. Attachment 7, NASA Internal Risk Assessment - Through August 2020, depicts the results of a risk assessment performed for NASA internal use on the likelihood of a medical incident per travel day, based on actuals for completed trips between July 1, 2014 through August 31, 2020. This data should not be construed as NASA requirements, usage, or predictor of future NASA needs. This does not replace potential future Offeror�s own risk assessment. NASA is not requesting any political security advisory or evacuation services. MINIMUM REQUIREMENTS This notice is being issued as a Request for Quote (RFQ) for MEDEVAC 4. The minimum requirements for the resulting contract are as follows: I. Statement of Work (SOW) � 1.�The Contractor shall be responsible for the overall performance of the contract and provide all necessary resources to meet contract requirements. All work completed under this contract shall comply with contract requirements. All deliverables under this contract shall not be marked proprietary or confidential or with any other markings limiting the Government�s use, unless otherwise specified by the Government. 2.�The Contractor shall provide an �Air Ambulance Medical Evacuation� (medevac) service in response to a worldwide medical emergency for International Space Station (ISS), Johnson Space Center (JSC), or other travelers identified by NASA (referred to as �traveler(s)� throughout) on a pre-paid basis. �Air Ambulance Medical Evacuation� (medevac) service includes, but is not limited to, comprehensive air and surface medical transport services appropriate for the condition of a sick or injured traveler; providing medical personnel, medical escorts, equipment and transportation that are fully certified and licensed, in accordance with local laws and regulations where operations are performed, to ensure the sick or injured traveler is provided the proper services to be stabilized prior to and during air and surface medical transport. These services shall be provided worldwide, regardless of traveler�s home location or country of origin. 3.�The Contractor shall provide an air ambulance medical evacuation (medevac) to NASA�s hospital of choice, which may be in the United States (U.S.) or elsewhere, for the traveler who is experiencing a medical condition when any of the following conditions are met: a.�Traveler requires in-patient hospitalization or b.�Traveler requires out-patient definitive medical care or c.�Traveler requires a medical escort for the medical condition or d.�Traveler has a medical condition and cannot safely travel by commercial means. 4.�The Contractor shall transport the injured or ill traveler to nearest appropriate medical facility that practices U.S. standards of medical care. This is defined as facilities with treating physicians and nurses who are trained in the US/Europe or are US/Europe board certified. In the event that the medical facility does not meet the U.S. standards of medical care, the Contractor shall provide sufficient rationale to the NASA/JSC Chief Medical Officer and NASA Contracting Officer Representative (COR) for using the proposed facility. 5.�In cases where the injured or ill traveler requires a medevac, the Contractor shall provide transportation for three accompanying travelers specified by NASA on the same transport as the injured/ill individual.� In the event that the same transport cannot accommodate all three accompanying travelers, at least one accompanying traveler specified by NASA shall be accommodated on the same transport. The Contractor shall provide sufficient rationale to the NASA Contracting Officer Representative (COR) why it cannot transport all accompanying travelers and provide alternate transportation for the remaining accompanying travelers within 24 hours to the same location as the injured or ill traveler. This alternate transportation requirement is applicable to a maximum of five medevacs per year. 6.�The Contractor shall provide additional medical travel services for the travelers on worldwide travel. These services include the following: a.�Provide transportation of patient from site of injury to appropriate medical facilities; b.�Coordinate with medical providers to ensure timely access to medical care; c.�Provide pharmacy assistance; d.�Obtain medical monitoring; e.�Provide assistance obtaining medications, vaccines, blood transfers, and medical devices; f.�Obtain medical referrals and consultations with English-speaking doctors; g.�Provide patient advocacy; h.�Provide bed side case management services such as in-person interaction and care; i.�Provide medical translation and interpretation services to include telephonic language assistance with medical incidents and medical translations of medical records to the extent required to resolve the medical emergency; j.�Provide assistance with hospital admittance and discharge; k.�Facilitate hospital payments; l.�Coordinate insurance information between the patient, insurers, and� medical providers; m.�Provide guarantee of payment (GOP) services to ensure traveler receives required medical services without delay. After the Contractor provides the timely GOP, the Contractor and the traveler will directly organize the reimbursement of the GOP, without any involvement or responsibility of the Government. n.�Provide repatriation of mortal remains for travelers to the location specified by NASA; NASA will co-develop with the Contractor the notification process for timely communication and dissemination of confidential medical reports. 7.�The Contractor shall participate in NASA Contingency Table Top simulations of evacuation events to validate communication and operations plans and decision-making procedures (estimate twice per year). 8.�The Contractor shall report their operational readiness to provide services prior to every launch and/or landing based on current worldwide events and capabilities. NASA does not require assets to be physically present at the launch and/or landing site or country of launch and/or landing operations. 9.�The Contractor shall conduct a program status meeting each contract year with NASA management and JSC�s Chief Medical Officer, with the first meeting happening within 90 days of the start of the contract. 10.�The Contractor shall conduct a one-time onsite field research trip to Russia and Kazakhstan to vet medical facilities and airports identified by NASA. The specific goals of the trip will be determined jointly between the Contractor and NASA. The Contractor shall provide a written trip summary report and conduct a debrief with NASA upon completion of the trip.�This trip shall be concluded by March 31, 2022. The Contractor shall provide and update each contract year the facilities points of contact, capabilities, admittance and use process, whether facility accepts US Government employee medical insurance plans and use recommendations for the medical facilities identified by NASA.� 11.�The Contractor shall establish, within 60 days of contract start and maintain for the duration of the contract, the GOP advance service agreements with the specific medical facilities that NASA identifies as critical medical facilities in Russia and Kazakhstan. If the Contractor cannot establish a GOP advance service agreement at one of NASA�s identified medical facilities in Russia/Kazakhstan by the end of that 60-day period, then the Contractor shall notify the NASA COR and provide an alternate course of action to meet the intent of the GOP advance service agreement. Annually on December 15, the Contractor shall provide a written summary report on the status of GOP advance service agreements for the NASA identified hospitals. 12.�The Contractor shall provide a written medevac plan of action/response for locations that NASA frequently visits to include Moscow, Russia; Baikonur, Kazakhstan; Karaganda, Kazakhstan; Nordwijk, Netherlands; Tsukuba, Japan; and Tanegashima, Japan. The plan shall be updated annually for currency throughout the life of the contract and after any major changes. The plan shall include: a.�Specific points of contact the Contractor will utilize to access restricted airports or facilities; b.�Unique country and local specific requirements for medevac services and plans on how to address each; c.�Specific facilities, capabilities and transport services in country the Contractor will utilize for bed-to-bed medical transport; d.�Whether selected medical providers accept U.S Government employee medical insurance plans e.�List of data and documentation the Contractor requires NASA to provide to support medevac. 13.�The Contractor shall provide a secure electronic registration capability that: a.�Provides travelers with a description of the scope of Contractor�s services in accordance with this SOW. b.�Educates travelers on specific instructions to follow should a medical incident or emergency occur and traveler�s responsibilities in accordance with this SOW. c.�Captures critical information from travelers that expedites medical assistance and evacuation when necessary. d.�Meets industry standards for data protection, accessibility, storage and retention requirements of private medical and personal information (e.g. Personally Identifiable Information, Health Insurance, etc.) The Contractor shall provide NASA with up-to-date weekly access to this enrollment and critical traveler information. Travelers will not be required to be pre-enrolled or registered in a Contractor�s system in order for the Contractor to provide services covered under this contract. 14.�The Contractor provided services shall meet the Commission on Accreditation Medical Transport Services (CAMTS) and European Aero-Medical Institute (EURAMI) accreditation standards or equivalent for personnel and assets. Any deviations must be reported to the JSC Chief Medical Officer and Contracting Officer Representative (COR), and approved by NASA prior to implementation, unless otherwise specified by the Contracting Officer. 15.�The Contractor shall provide services globally without exception. In the event that the Contractor cannot provide a medevac in specific parts of the world, then the Contractor shall provide sufficient rationale to the NASA COR why it cannot and provide an alternate solution at no additional cost beyond the prepaid amount. 16.�Evacuation and Additional Medical Travel Services Cost Responsibility a.�If a medical event occurs, all costs for evacuation and additional medical travel services shall be the responsibility of the Contractor and no additional cost beyond the prepaid amount will be incurred by NASA. b.�The Contractor shall be responsible for all medevac and medical travel service costs. This excludes personal medical bills accrued by the traveler which are not arranged for or provided by Contractor are the responsibility of the injured or ill traveler. c. The Contractor shall collect the necessary insurance information from the traveler when required and coordinate payment of medical expenses directly with the injured or ill traveler and his or her insurer. d.�NASA will not provide arbitration or remediation to resolve disputes between the Contractor and traveler regarding reimbursements of uncovered expenses. 17.�Decision to Evacuate The decision to perform a medevac shall be made jointly (50/50) by NASA and the Contractor, who will apply U.S. standards of medical care. In the case of disagreement on medical evacuation between the NASA and the Contractor, an independent third party will be consulted. Upon contract award, this independent third party will be identified and mutually agreed upon.� 18.�The Contractor shall provide group membership as specified in Clause 5 Firm Fixed Price (FFP) Group Membership. 19.�The Contractor shall provide a medical evacuation using a Portable Medical Isolation Unit (e.g. infectious disease) upon request by NASA. This requirement is priced in accordance with Tables 1b and 2b in Clause 5 Firm Fixed Price (FFP) Group Membership. 20.�The Government does not assume liability that results from claims that arise from their direct performance of this contract. The Contractor shall indemnify the Government for any occurrence, action, or claim that might trigger the Government's liability; unless the Government is found to be liable. The rights and obligations of the parties under this clause shall survive the termination, expiration, or completion of this contract. (End of SOW) II. Period of Performance The period of performance for the contract base of the resulting purchase order is October 1, 2021 � September 30, 2023 Option 1 The period of performance for Option 1 of the resulting purchase order is October 1, 2023 � September 30, 2024. Option 2 The period of performance for Option 1 of the resulting purchase order is October 1, 2024 � September 30, 2025. Option 3 The period of performance for Option 1 of the resulting purchase order is October 1, 2025 � September 30, 2026. The overall period of performance (including Option 1, Option 2, and Option 3) for the resulting purchase order is October 1, 2021 � September 30, 2026. III. Instructions to Offerors Selection and award will be made to the Offeror whose offer will be most advantageous to the Government, with consideration given to the factors associated with proposed technical capability and price. In providing these instructions, the Government�s intention is to solicit information that will permit a competitive evaluation of the Offeror�s proposal. The information solicited will demonstrate the Offeror�s competence and understanding of requirements and the capability to successfully complete the requirements specified in the MEDEVAC SOW. All responsible sources may submit an offer, which shall be considered by the Agency. The Government intends to make a single award for all items on an ""all or none"" basis; therefore, Offerors must provide a proposal that responds to all items below. Partial quotes will not be considered responsible to this solicitation and therefore not accepted. The Government intends to award the resulting Purchase Order, if any, without discussions. A. Technical Capability Technical Capability will be determined by review of information submitted by the Offeror to the following Sections: 1. Technical Capability Questions/Responses 2. Scenarios Offerors must provide responses to these areas in sufficient detail to demonstrate responsiveness to the solicitation. 1. Technical Capability Questions/Responses Responses shall be submitted in a Microsoft Word document in Times New Roman, size 12 font. a. Describe the offeror�s approach, teaming arrangements, resources, processes and procedures to provide worldwide medical evacuation and additional medical travel services capabilities per the requirements of the Statement of Work (SOW). The information provided should be in sufficient detail to provide NASA technical and medical experts with evidence that demonstrates the Offeror technical understanding of NASA�s requirements and demonstrates the Offeror has the necessary resources, experience and qualifications to perform all NASA's requirements described in the SOW. The Offeror shall explain its capabilities and expected operational timelines/durations with medical extractions for potential life or death circumstances with a point of origin in a non-western country similar in size, scope, and complexity to the requirements in the SOW and scenarios. The Offeror shall explain how it flows down the contract terms and conditions to its subcontractors and teaming partners. b. Describe the air and ground transportation assets (type/design/age/capabilities/locations) your company uses to provide worldwide medical evacuations per the requirements of the Statement of Work (SOW). What types of certifications do these assets hold and where are they certificated (US, other country)? Does the Offeror maintain a fleet of its own fixed wing and rotary aircraft required for medical transport or does it subcontract aircraft from other companies (If other companies, which ones)? To what standards are the aircraft maintained (FAA, foreign equivalent, other) and is the maintenance provider FAA Part 145 (or equivalent) certified? Does the aircraft provider have a Safety Management System (SMS) in place (For example International Standard for Business Aircraft Operations (IS-BAO))? c. With regard to pilots used by Offeror for local and long-distance transport, does the Offeror maintain their own pilot cadre or does it subcontract pilots from other companies (If other companies, which ones)? Where, how, and to what standards are the pilots certified (For example FAA Commercial/Airline Transport Pilot (ATP), international equivalent)? To which medical standards are the pilots held (i.e. FAA, Class I/II, other)? What are the minimum experience levels of the pilots (flight time, international operations, IFR operations, etc. / i.e. minimum 1500 hours turbine/multi-engine etc.)? What recurrent training and minimum proficiency requirements are mandated for the pilots (i.e. FAA Part 121 or Part 135 recurring training requirements)? Does the source of the pilot cadre have a Safety Management System (SMS) in place (for example IS-BAO)? d. Provide evidence showing your company is capable of providing each of the following services: 1. Advanced airway management including ventilator support/oxygenation 2. Advanced medication management in flight (anti-seizure meds, IV meds, etc.) 3. Advanced Cardiac Life Support 4. Medical Escort Services 5. Medevac Using Portable Medical Isolation Unit (e.g. infectious disease medevac) e. Describe how your company will apply US standards of medical care for travelers under this contract? How is this applied to your subcontractors and teaming partners? f. Provide evidence that demonstrates how your quality assurance program ensures that only appropriately trained, certified, and credentialed medical staff, equipment and transports are used to provide services within your company�s limits. Please provide similar evidence for any subcontractors. Additionally, please submit proof of CAMTS and/or EURAMI certification or equivalent. For equivalent certification, please provide narrative on applicability. g. Provide all steps required to execute a Contractor-provided medical evacuation under this contract. Please specify all steps required by the Government with rationale to support the medical evacuation. 2. Scenarios Offerors shall describe how they would respond to (4) four medical scenarios provided below assuming they occur at the time of the solicitation. Responses shall include action plans; decision points; operational timelines; specific resources, personnel, equipment, transport and other assets used at each stage; travel route from origin of incident to hospital of choice; permits, licenses, certifications, etc. required to execute the offeror�s response; interfaces with the Government; and any other information that will illustrate your company�s capabilities to perform the requirements in the Statement of Work. Offerors must demonstrate their ability to apply US standards of medical care to its patients during a MEDEVAC scenario. Offerors must also demonstrate their ability to include NASA ISS officials in the medical transportation decision process as well as demonstrate its quality assurance program, which ensures that only appropriately trained, certificated and credentialed medical staff, equipment and transportation are provided for medical care within the company�s limits. Offerors must also demonstrate their ability to provide a GOP and state the steps involved. The response for each scenario shall not exceed two pages each. Responses shall be submitted in a Microsoft Word document in Times New Roman, size 12 font. A) A 37-year old female NASA sponsored traveler is at Baikonur Cosmodrome, Kazakhstan. Early in the day, her husband launched aboard a Soyuz headed to the space station. She is 22 weeks pregnant and she develops abdominal pain and vaginal spotting. Local medical capability cannot provide appropriate care for her. NASA has a physician onsite supporting her. Her 12-year old daughter is also present at Baikonur. NASA contacts you requesting urgent medical evacuation. The patient is requesting transportation to the University of Texas Medical Branch Hospital in Galveston, Texas where her OBGYN doctor will see her. She also insists on having her daughter and NASA flight surgeon return on the same aircraft with her. B) A 48-year old NASA astronaut is performing a Soyuz landing training session at a remote field site 50 miles due east of Dzhezkazgan, Kazakhstan. During this training, the astronaut is noted to have atrial fibrillation. The astronaut is placed on rate control medication and anticoagulant. The accompanying NASA flight surgeon determines the astronaut is stable but requires return to the Cleveland Clinic in Cleveland, Ohio for appropriate care. At this point NASA notifies the contractor with the request along with the latitude and longitude of the astronaut and NASA flight surgeon. C) A 64-year old NASA male scientist becomes ill while on travel with NASA. He is in Tanegashima, Japan. The local medical facility has treated him to their maximum ability, and they recommend medical evacuation to a higher level of care. At the time NASA contacts you, he is on IV hydration and pain control. The presumptive diagnosis is acute pancreatitis though the local facility cannot confirm. He is from Las Cruces, New Mexico and requests travel to Memorial Medical Center for hospitalization and further evaluation. D) A previously healthy 51-year old female NASA employee is on NASA travel on the Kwajalein Atoll in the Marshall Islands. She collapses during a meeting. NASA contacts the contractor requesting urgent assistance with admittance to a local hospital with an Intensive Care Unit. She is diagnosed with a brain tumor, advanced, requires ventilator support and anti-seizure medications. Otherwise, she is stable and non-operative. The hospital will not discharge her until the hospital fees are paid, but she is traveling solo and does not have her wallet with her. The patient is non-communicative, but the next of kin indicates the desire to have the patient transferred to Los Angeles, California and mentions �Cedars Sinai Cancer Center� near their home.� B. Price Offerors shall provide pricing on the attached Attachment�1, Proposal Pricing Sheet. As this procurement will result in a firm-fixed-price purchase order, Offerors shall provide firm-fixed pricing that is all-inclusive (including profit and fees) for the required services on the Proposal Pricing Sheet. Additional pricing terms will not be accepted. The Proposal Pricing Sheet contains two sections: 1. Pre-paid group membership price 2. NASA identified independent traveler membership pricing Each section has an �a� and �b� pricing table. The �a� table represents the baseline pricing for the period and terms specified in the title of the table. The �b� table represents the optional delta price for Medevac Using Portable Medical Isolation Unit (e.g. infectious disease) only. If the Government orders Medevac Using Portable Medical Isolation Unit (e.g. infectious disease) coverage for the stated periods, the sum of tables �a� and �b� shall represent the total price of services.� The Offeror shall submit a completed Proposal Pricing Sheet with their proposal. Travel Days The values in the Number of Travel Days columns in Tables 1a and 1b represent the cumulative number of travel days covered across all travelers in the contract period specified in that row. For example, 3 travelers attending meetings in Moscow, Russia for 5 days would equal a total 15 travel days. These travel days do not include the travel of NASA identified independent travelers covered by Tables 2a. and 2b. Accordingly, the Contractor is expected to cover 22,500 travel days for the Base period of performance under the resulting contract, and 15,000 travel days per year for each of the remaining Option periods, if exercised. C. Contact Information and Response Time All responsible sources may submit an offer, which shall be considered by the agency. The Government intends to make a single award for all items on an ""all or none"" basis; therefore, Offerors must propose all items. Partial quotes will not be accepted. All contractual and technical questions must be emailed to Portia Keyes (portia.s.keyes@nasa.gov) no later than July 30, 2021, at 6:00PM CDT. Telephone questions will not be accepted. Offers for the items(s) described above are due by 6:00PM CDT on August 13, 2021, to portia.s.keyes@nasa.gov and must include, solicitation number, FOB destination to the Johnson Space Center, discount/payment terms, warranty duration, taxpayer identification number (TIN), identification of any special commercial terms, and be signed by an authorized company representative. Offerors are encouraged to use the Standard Form 1449, Solicitation/Contract/Order for Commercial Items, found as an attachment to this solicitation. D. Applicable Provisions, Clauses, Terms and Conditions Offerors shall provide the information required by FAR 52.212-1 (JUL 2021), Instructions to Offerors-Commercial Items, which is incorporated by reference. Proposals received in response to this solicitation will be evaluated using the procedures specified in FAR 52.212-2 (OCT 2014), Evaluation-Commercial Items. Paragraph (a) of provision FAR 52.212-2 is updated as follows: (a)�The Government will award a contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) Price. The Government will evaluate the offerors submission of the completed Proposal Pricing Sheet. The Government will determine the final evaluated price by adding the total price in tables 1a and 1b to the adjusted proposed prices in tables 2a and 2b. The proposed prices in tables 2a and 2b will be adjusted, for evaluation purposes only, as follows: The Government will calculate the average price of the Membership Types proposed in tables 2a and 2b. Each of these averages will be multiplied by 1,000. Table 2a Price Adjustment Example: 2.a Pre-Paid Membership Price for NASA Identified Independent Traveler (excluding Portable Medical Isolation Unit Evacuations) Coverage for 14 days per year�$ 100 Coverage for 30 days per year�$ 200 Coverage for 90 days per year�$ 300 Coverage for 365 days per year�$ 400 Average Price = ($100+$200+$300+$400)/4 = $250 Adjusted Price = $250 *1,000 = $250,000 (ii) Technical Capability For the purpose of proposal evaluations, Price is significantly more important than Technical Capability. � Offerors shall include a completed copy of the provision at FAR 52.212-3 (FEB 2021), Offeror Representations and Certifications-Commercial Items, with its offer. These representations and certifications will be incorporated by reference in any resultant contract. � FAR 52.212-4 (OCT 2018), Contract Terms and Conditions-Commercial Items, applies to this acquisition. Addenda to FAR 52.212-4 are provided in attachment 3. Model Contract.
