- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation;� proposals are being requested and a written solicitation will not be issued. � The solicitation number for this requirement is RFQ- 2021-55761-EJH and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified� Acquisition� Procedures. The Centers for Disease Control and Prevention intends to award a fi rm� fixed-price contract for the following requirement, please either fill in the dollar amounts or provide a separate excel spreadsheet containing the same information including a list of line item number(s) and items, quantities, units of measure, and options, if applicable: CLIN 0001: Provide Design for Three (3) Products in Accordance with the Attached Statement of Work TOTAL PRICE: Your quote shall include the following: Total Price for the CLIN 0001 Capabilities in Accordance with the Statement of Work and Evaluation Factors Partial Payments will be disbursed in accordance with the Statement of Work Period of Performance: to commence no later than one month after NIOSH delivers the first training needs document. All asks are to be completed within 10 months of the start date. QUOTERS ARE ADVISED AN AWARD MAY BE MADE WITHOUT DISCUSSION OR ANY CONTACT CONCERNING THE QUOTES RECEIVED. ��Quoters should not assume they will be contacted, or afforded the opportunity to qualify, discuss, or revise their proposals. Therefore, the initial quote should contain the quoter's best terms from a technical and price standpoint. ��All quotes must include the full name, address, and DUNS number of the business to be named on the purchase order as well as the full name, phone number, and email address of the person who can accept the final purchase order. This is a total small business set-aside. The North American Classification System (NAICS) code for this procurement is 611430-Professional and Management Development Training. Quotes are due on Thursday, August 19, 2021 by 3:00 PM EST and should be sent to the attention of Elaine Hamilton via email at EHamilton@cdc.gov.� �Evaluation and award of the purchase order will be performed in accordance with FAR 13.106-2 Evaluation of Quotations or Offers and FAR 13.106-3 Award and Documentation. The Offeror must have an active registration with the System for Account Management (SAM), �prior to award of the purchase order in accordance with FAR Clause 52.204-7. QUOTES MUST BE SUBMITTED IN TWO PARTS: A TECHNICAL PROPOSAL AND A PRICING PROPOSAL. After evaluation of the following technical factors, award will be made to the lowest priced technically acceptable offer. Each offer will be evaluated on a pass/fail basis, so offerors are cautioned that they should submit responses in sufficient detail to enable the government to evaluate their capabilities and experience. Evaluation Criteria Safely Transitioning Miners to New Positions - Instructional Design and Mine Safety Training Expert Period of Performance: 2021-2022 Minimum Qualifications / Evaluation Factors Technical Evaluation Provide information reflecting experience, including a detailed resume, biography, or curriculum vitae (CV). Highlight how all required and preferred expertise was obtained. A letter of interest is optional but welcome. Required Contractor Expertise: 1. Minimum 5 years experience delivering training to workers in an industrial occupational setting required.. �2. Minimum 2 years experience in training design for adult learners required; 3 or more years preferred. 3. Experience in evaluating training effectiveness and revising training required. 4. Familiarity with mine safety and health training requirements required. �5. Strong knowledge of scenario-based instruction preferred 6.
