Description

RFQ Amendment 0001, effective August 9, 2021 hereby incorporate the following: 1: Extend the closing date from August 12, 2021 to August 13, 2021. Time remains unchanged.� 2. Incorporate Questions and Answers as an RFQ Attachment.� ------------------------------------------------------------------------------------------------------------------------------------------------------------------------------ This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, �Streamlined Procedures for Evaluation and Solicitation for Commercial Items,� as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. SCOPE OVERVIEW: Contractor shall furnish including but not limited to all labor, materials, parts, equipment, tools, transportation, and supervision to provide the Southeast Louisiana Veterans Health Care System (SLVHCS), 2400 Canal St, New Orleans, LA 70119 with one hundred-(100) Stryker LIFEPAK CR2 defibrillator with LIFELINKcentral AED Program Manager (No. 99512-001262) and operational manual (name brand or equal). The equipment, all its accessories, and supplies that come with the equipment shall be brand new. See attchment for complete solicitation.
