- THIS IS A SOURCES SOUGHT NOTICE: This is NOT a request for proposals/quotes; this is an industry inquiry for Market Research. A solicitation shall not be issued at this time. The intent of the procurement is to locate potential contractors for solicitation. The below specification defines the effort required for the purchase and period of performance. This market research is for informational and planning purposes only to determine if a socio-economic set-aside, or full and open competition, or sole-source procurement is appropriate. The Government shall not pay any costs for responses submitted. This notice is published to conduct market research to determine if there is a sufficient number of certified Service-Disabled Veteran Owned Small Businesses (SDVOSB), Veteran Owned Small Businesses (VOSB), Small Businesses (SB), or Large Business capable of performing the requirements to warrant a socio-economic set-aside or full & open competition. Notice to all contractors: All contractors must have an active registration in the System for Award Management (SAM) at https://beta.sam.gov/ before responding to any Government inquiry. Additionally, all Service-Disabled Veteran-Owned Businesses or Veteran-Owned Small Businesses who plan to submit an offer must have an active registration with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. Interested Parties Shall Submit the Following Information at a Minimum: (a) Company name (b) Address (c) Point of contact (d) Phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of Small Business Concern, e.g. Services Disabled Veteran Owned Small Business, Veteran-owned small business, Women-Owned Small-Business and or Large Business. (i) Must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. NAICS Code: 238210- Electrical Contractors and Other Wiring Installation Contractors Average Annual Receipts $16.5 Million. PSC/FSC- N059-Installation of Equipment Electrical and Electronic Equipment Components The term of the contract is intended to be firm-fixed priced contract. Period of Performance: 60 Days After Receipt of Award The Phoenix VA Health Care System has a service requirement. The contractor shall provide all labor, travel, and parts and equipment to perform the following services: Replace thirty-three (33) new fire suppression sprinkler heads to Anti-Ligature fire suppression sprinkler heads. Sprinkler heads shall be tamper-resistant and unable to be used as an anchor point; the fusible element is designed to be breakaway. Installation of a Sally Port Door Interlocking System, to the existing locking system that must not allow for both doors to be unlocked and opened at the same time. This is to be installed on one (1) pairs of doors on the Mental Health Ward Floor. All inpatient units will be locked under normal operation. Entrances to inpatient units have a Sally Port (secure entry vestibule) designed with two sets of synchronized doors. Door synchronization is required to ensure only one set of doors may open at a given time. The sets of doors are to prevent patient elopement. These doors should have hardware with electronic locks to allow remote unlocking by staff, proximity card access by authorized staff and automatic unlocking in the event of an emergency. Secondary exits shall also have electronic hardware that allows these doors to be automatically unlocked in an emergency. The installation of the Sally Port Door, interlocking system is to be installed during normal hospital working hours (Monday-Friday 6:30 am 3:30pm) with the Mental Health Ward remaining operational. The Contractor is to be aware that they will be working in the confines of a Mental Health Ward. The Contractor is to secure all materials and tools during working hours and NEVER leave any of these items unattended too or create any situation that will allow patients or staff to hurt themselves. Contractor is to provide minimum of 14 calendar days notice for required utility shutdowns. All utility requests submitted for less than 14 days may not be approved. Contractor to advise, if the they can perform all the required services or if they will have to subcontract services. Place of Performance: Phoenix VA Health Care System Carl T. Indian School Road Phoenix, Arizona 85014 Important Information: The Government is not obligated to nor shall it pay for or reimburse any costs associated with responding to these sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. The VA is mandated by Public Law 109-461 to consider a total set-aside for Service-Disabled Veteran Owned Small Business set aside. However, if response by Service-Disabled Veteran Owned Small Business firms proves inadequate, an alternate set-aside or full and open competition may be determined.
