- Page 1 of 3 The Department of Veterans Affairs Great Lakes Acquisition Center (GLAC) in North Chicago Il intends to negotiate a sole source contract with INSTRON GMBH 825 UNIVERSITY AVE NORWOOD MA. 02062 to Instron service for a retrofit Instron 8800 Universal test system. This service covers general maintenance and calibration of Instron equipment, including two 5 Kn load cells (force verification 100-22K lb tension & compression, ASTM E4 protocols), for the Jesse Brown VA Hospital campus located at 820 S Damen Ave Chicago Il 60612 This contract will be for a firm fixed price contract. This procurement is being conducted in accordance with FAR 6.302-1, Only One Responsible Source and no other supplies or services will satisfy the agency requirements. The contract will be firm fixed price and the NAICS code is 334519 This Notice of Intent is not a request for competitive proposals. No solicitation documents are available and telephone requests will not be honored. However, if a firm believes it can meet the requirements it must furnish information about its products, services and capabilities as well as references from other customers who are using these products and services provide service for Gate repair and replacement for the Jesse Brown Campus Federal Health Care Center to the contracting officer NLT 11:00AM CST, 16 August 2021. Supporting evidence must be submitted in sufficient detail to demonstrate the ability to comply with the above requirements and attached Statement of Work, to include access to OEM parts for repairs. Responses received will be evaluated; however a determination by the Government to not compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the GLAC will proceed with the sole source negotiation with INSTRON GMBH Statement of Work I. CONTRACT ADMINISTRATION DATA Contracting officer will complete this section II. SCHEDULE OF ITEMS/ SUPPLIES OR SERVICES PRICES/COSTS Instron service for a retrofit Instron 8800 Universal test system. This service covers general maintenance and calibration of Instron equipment, including two 5 Kn load cells (force verification 100-22K lb tension & compression, ASTM E4 protocols), an extensometer (strain verification, ASTM E83 protocols), a 1000 Lb load cell (force verification 100-22K lb tension & compression, ASTM E4 protocols), a 500 N load cell (force verification 0-99 lb tension & compression, ASTM E4 protocols), 11 K lb load cell (force verification 100-22K lb tension & compression, ASTM E4 protocols), servohydraulic Oil Analysis, and Instron 8800 Stroke Verification (10 Pt test, both directions). This service ensures the safety, operation, accuracy and reliability of the universal test system for Research & Development activities performed at the JBVAMC. III. STATEMENT OF WORK TEMPLATE A. GENERAL INFORMATION 1. Title of Project: Service Contract for JBVAMC retrofit Instron 8800 Universal test system. 2. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in the statement of work (SOW), except as may otherwise be specified. This contract ensures the safety, operation, accuracy and reliability of the retrofit Instron universal test system and its associated equipment for Research & Development activities performed at the JBVAMC. 3. Background: This equipment is essential to performing mechanical characterization data collection for Research & Development activities. The equipment included is required to be properly maintained by certified technicians. 4. Performance Period: The period of performance is _FY22___. Work at the government site shall not take place on Federal holidays or weekends unless directed by the Contracting Officer (CO). 5. Type of Contract: Firm-Fixed-Price. 6. Place of Performance: This will take place at the JBVAMC. B. CONTRACT AWARD MEETING (IF APPLICABLE) The contractor shall commence performance on the tasks in this SOW as soon as possible. No meeting is necessary. C. GENERAL REQUIREMENTS 1. For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task, together with associated sub-milestone dates. 2. All written deliverables will be phrased in layperson language. Statistical and other technical terminology will not be used without providing a glossary of terms. 3. Where a written milestone deliverable is required in draft form, the VBA will complete their review of the draft deliverable within _14 calendar days from date of receipt. The contractor shall have 14 calendar days to deliver the final deliverable from date of receipt of the government s comments. D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below within the performance period stated in Section A.4 of this SOW. Task One: The contractor shall perform maintenance and service of the retrofit Instron 8800 Universal test system. This service covers general maintenance and calibration of Instron equipment, including two 5 Kn load cells (force verification 100-22K lb tension & compression, ASTM E4 protocols), an extensometer (strain verification, ASTM E83 protocols), a 1000 Lb load cell (force verification 100-22K lb tension & compression, ASTM E4 protocols), a 500 N load cell (force verification 0-99 lb tension & compression, ASTM E4 protocols), 11 K lb load cell (force verification 100-22K lb tension & compression, ASTM E4 protocols), servohydraulic Oil Analysis, and Instron 8800 Stroke Verification (10 Pt test, both directions). Deliverable One: Calibration sheets. E. SCHEDULE FOR DELIVERABLES 1. The contractor shall notify the JBVAMC of the delivery date for the deliverable. 2. Unless otherwise specified, the number of draft copies of a delivery notice and the number of final copies shall be the same. 3. If for any reason the scheduled time for a deliverable cannot be met, the contractor is required to explain why (include the original deliverable due date) in writing to the CO, including a firm commitment of when the work shall be completed. This notice to the CO shall cite the reasons for the delay, and the impact on the overall project. The CO will then review the facts and issue a response in accordance with applicable regulations. F. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. G. REPORTING REQUIREMENTS 1. The contractor shall provide the Task Order Project Manager (TOPM) with calibration sheets at the end of the service to report results of the maintenance and calibration. H. TRAVEL Technicians are located locally. Travel and per diem shall be reimbursed in accordance with VA/Federal Travel Regulations. Travel must be pre-approved by the TOPM. Travel must be priced separately in the price schedule. I. GOVERNMENT RESPONSIBILITIES NA J. CONTRACTOR EXPERIENCE REQUIREMENTS KEY PERSONNEL The skilled professional and/or technical personnel who are experienced in maintaining, servicing, and calibrating retrofit Instron universal test systems and associated actuation and measurement equipment are essential for successful contractor accomplishment of the work to be performed under this contract. The contractor agrees that the key personnel shall not be removed, diverted, or replaced from work without approval of the CO and TOPM. Any personnel the contractor offers as substitutes shall have the ability and qualifications equal to or better than the key personnel being replaced. Requests to substitute personnel shall be approved by the TOPM and the CO. All requests for approval of substitutions in personnel shall be submitted to the TOPM and the CO within 30 calendar days prior to making any change in key personnel. The request shall be written and provide a detailed explanation of the circumstances necessitating the proposed substitution. The contractor shall submit a complete resume for the proposed substitute, any changes to the rate specified in the order (as applicable) and any other information requested by the CO needed to approve or disapprove the proposed substitution. The CO will evaluate such requests and promptly notify the contractor of approval or disapproval thereof in writing. K. SECURITY CONTRACTING WILL COMPLETE THIS SECTION
