Amendment 0001 to RFQ 36C25821Q0287: The due date for quotes has been extended to 12:00 PM PDT, August 20, 2021. The following addresses questions regarding solicitation: 36C25821Q0287. These questions and answers are not incorporated into the solicitation and are only to clarify the procurement. Phoenix VAHCS: After reviewing the inventory, I noticed that the 5-Year Load test for Phoenix for Elevator S1, S2, S3, S4, and S5 are due Nov 2023. However, I do not see a line item for this. Answer: Elevators S1 thru S5 require 5-year load tests in Nov 2023. Please see the following pages for an updated list of Contract Line Items that incorporates Line Item 2005 for 5-year testing, to include weight testing. Vendors shall provide quotes to this updated list. New Mexico VAHCS: There are no due dates for Elevators P3, P4, S1, S2, S3, S4, P1, P2, T1, and P1 bldg 85. Can we assume that these elevators are all due for the five-year load test? If possible, please list out all the elevators that are due for the five-year load test on the year that it is due. Answer: The load tests are included in the 5-year testing, Line Items 2004, 4004, 4005, and 4006. The load test for the one elevator in Bldg. 85 is due for a load test in 2024. All the other elevators will be due in 2026. For the Base year you have listed in the Price/Cost Schedule that 5 elevators need inspecting for CAT5 but then list the following 6 elevators: S6, S7, S8, T1, T3, T6. Also, in section D many of the elevators do not have a date listed for when they are due for the next CAT 5 inspection. Our question is for the Base year at the Phoenix facility: How many and what type of elevators are needing CAT 5 inspections in the Base year? Answer: Six elevators need the 5-year inspection for the Base year and they are cable elevators. S6, S7, S8, T1, T3 and T6 will be due their 5-year test in Apr 2022. For Option Year 1 you have listed in the Price/Cost Schedule that 5 elevators need inspecting for CAT5 but then list the following 6 elevators: S6, S7, S8, T1, T3, T6. Also, in section D many of the elevators do not have a date listed for when they are due for the next CAT 5 inspection. Our question is for Option Year 1 at the Phoenix facility: How many and what type of elevators are needing CAT 5 inspections in Option Year 1? Answer: Five elevators need the 5-year inspection for Option Year 1 and they are cable elevators. S1, S2, S3, S4 and S5 will be due a 5-year test in Nov 2023. For Option Year 3 in the Price/Cost Schedule you have listed that the CAT 5 hydraulic inspections are due. According to Section D many of the Hydraulic elevators are N/A for CAT 5 inspections. Our Question is for Option Year 3 at the New Mexico facility: How many of the hydraulic elevators need CAT 5 inspection and in what years do they need to be done? Answer: There is one hydraulic elevator which was installed and certified in August 2019; that elevator will be due for a CAT 5 inspection in 2024 (Option Year 2), all the remaining hydraulic elevators should be done on the current contract this year and will be due again in 2026 (Option Year 4). For Option Year 4 in the Price/Cost Schedule you have listed that the CAT 5 hydraulic inspections are due again. Also, in section D the due date for traction elevators for have the CAT5 inspections differ in years. Our question is for Option Year 4 at the New Mexico facility: How many of the traction elevators and hydraulic elevators need CAT 5 inspections in Option Year 4? Answer: All eleven of the traction elevators and the one dumbwaiter will require a CAT 5 inspection in 2026 (Option Year 4) along with all but one hydraulic elevator (a total of seven hydraulic elevators). This amendment updates the Contract Line Items below by incorporating Line Item 2005. This additional line item is for performing five-year tests at the Phoenix VA Health Care System and includes weight testing. Quotes must include pricing for this additional line item.
