Description
The Department of Veteran Affairs, New England Veterans Healthcare System has a requirement to purchase two Sterile Medical Cabinets for Providence VAMC. This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of potential qualified sources and their size classification and socio-economic categories (service-disabled veteran owned small business, veteran owned small business, hub zone, 8(a), small disadvantage business, woman-owned, small business, or large business). The North American Industry Classification System (NAICS) Code assigned for this requirement is 423450 Medical, Dental and Hospital Equipment and Supplies Merchant Wholesalers with a small business size standard. Note the non-manufacturer rule (NMR) applies to the resulting procurement and is not waived at this time. If a small business is interested in this procurement, they must cover at least 50 percent of the cost of manufacturing the products or supply products it did not manufacture as long as those products come from another small business. This only applies to small businesses. The intent of the procurement is to award an Firm Fixed Contract. Reponses must be complete and sufficiently detailed to address the specific information. The documentation shall address, at a minimum the following: Company Profile to include: Company name and address: Affiliate information: parent company, joint venture partners, and potential teaming partners Point of contact (names, titles, phone numbers and email addresses) DUNS number, as registered in the System for Award Management (SAM) at https://beta.sam.gov/ (Must correlate with SAM registration) Socio-economic status. Service Disabled Veteran Owned Small Businesses and Veteran Owned Small Businesses must be verified in the VA s VetBiz Vendor Information Portal at https://www.vip.vetbiz.va.gov/, and must submit documentation that the firm is capable of providing 50% of the work by its own forces or by that of another SDVOSB/VOSB in accordance with Public Law 109-461. Additionally, small businesses must submit documentation of compliance with the non-manufacturer rule. Qualifications statement detailing experience/ability to provide the Sterile Medical Cabinet. INSTALLATION - NA. 3. TRAINING NA 4. INSPECTION AND ACCEPTANCE The Clinical Engineer on site will inspect upon delivery of equipment. The Clinical Engineer on site shall ensure all work is completed satisfactorily prior to acceptance. Disputes shall be resolved by the Contracting Officer. 5. WARRANTY The contractor shall provide a one-year manufacturer s warranty on all parts and labor. The warranty shall include all travel and shipping costs associated with any warranty repair.
