The requesting facility is in VISN 6, Salem Veterans Affairs Medical Center, Salem Virginia Background: Previously procured by Salisbury VAMC via 36C24620P1557 on 9/17/2020. Nature and/or Description of the Action Being Processed: This is a request for a new contract at a firm-fixed price. (1) Purchase of new equipment, software and maintenance services, specifically the AvaSure Telesitter Solution system, for the Salem VAMC Community Living Center (2) This is a request for brand name and Vendor Specific Sole Source Justification (3) We are requesting the purchase of the following: 12 AvaSure Guardian Mobile Hardware Devices 12 AvaSure Software - Standard 1 Year - Licensed Per Device o Server Software o Client Software o AvaNET Support & Warranty o ORNA Reporting Tool o Clinical Program Materials o eLearning Modules o Pre-recorded Languages 12 US Power Cord 10 Medical 12 Shipping and Handling 1 Monitor Station PC and 40 Monitor 1 Spare Parts Kit with 10ft power cord Services: - AvaSure Clinical Services - AvaSure Install Services-Phase 1 Remote - AvaSure Project Management Services Miscellaneous install by Greenbrier Government Solutions Inc. to provide all materials and labor to install 25 Category 6A cable runs. AvaSure TeleSitter Solution is a sole source product: designed, manufactured, sold, and distributed exclusively in the United States by AvaSure, LLC. AvaSure TeleSitter Solution is a highly specialized system of continuous virtual monitoring of multiple patients who are at a high risk of falls or self-harm. The system can slow/prevent/reduce adverse events (falls, elopements, suicides, violent events, medical tube/line dislodgement) for patients w/agitation, confusion or impaired judgement (r/t dementia, delirium, terminal restlessness, or other medical conditions) by providing safe visualization and communication with patient/staff/family in bedrooms via mobile audio/video system. From a remote location, trained hospital staff members use video to watch multiple patient rooms simultaneously, intervening via audio or alerts when needed to keep patients/family/staff safe from harm. AvaSure TeleSitter Solution offers a high return on investment as it potentially reduces costs for injuries sustained from a fall (it is more cost effective to prevent a fall than to pay for the resulting injury/impairment after a fall). Additionally, the system can provide an alternative, fiscally responsible method of monitoring at-risk patients by conserving nursing resources and reducing the use of 1:1 sitters. The system provides data analytics/benchmarking of system utilization, clinical response times, and monitoring and effectiveness of staff interventions through ORNA (Online Reporting of Nursing Analytics). The TeleSitter uses an infrared video camera equipped with 2-way audio that can be deployed portably or permanently. The camera has a 360-degree pan, tilt, zoom, and a night vision IR light for optimal viewing. The system offer HIPPA compliance and patient privacy protection via a privacy mode with no recording of audio or video. No other company makes this type of product. Accordingly, AvaSure, LLC is the only firm capable of providing the supplies and services described in Section III above without the Veteran s Health Administration experiencing substantial duplication of cost that could not be expected to be recovered through competition and unacceptable delays in fulfilling its requirements. Description of Efforts Made to ensure that offers are solicited from as many potential sources as deemed practicable: AvaSure TeleSitter Solution is a sole source product: designed, manufactured, sold, and distributed exclusively in the United States by AvaSure, LLC. Citation 10 USC 2304(c)(1), Only one responsible source , since it is this survey of the market place that confirms our assumptions regarding the capability of industry to meet our needs. (See FAR 6.303-2(b)(6)). Determination by the CO that the Anticipated Cost to the Government will be Fair and Reasonable: The pricing is consistant with like VA purchases. Description of the Market Research Conducted and the Results, or a Statement of the Reasons Market Research Was Not Conducted: See Market Research document. Any Other Facts Supporting the Use of Other than Full and Open Competition: AvaSure LLC (SB) is the OEM of the Telesitter system. Listing of Sources that Expressed, in Writing, an Interest in the Acquisition: AvaSure LLC A Statement of the Actions, if any, the Agency May Take to Remove or Overcome any Barriers to Competition before Making subsequent acquisitions for the supplies or services required: Multiple meetings between Salem VAMC stakeholders in this project and acquisition specialists to ensure required forms are completed accurately.
