- Sole Source Justification PM Part 813.5 SAP for Certain Commercial Items Page 1 of 9 Original Date: 08/30/17 Revision 03 Date: 02/11/2021 DEPARTMENT OF VETERANS AFFAIRS SOLE SOURCE JUSTIFICATION UNDER SIMPLIFIED PROCEDURES FOR CERTAIN COMMERCIAL ITEMS IN ACCORDANCE WITH FAR 13.5 Acquisition Plan Action ID: 36C246-21-AP-3497 Contracting Activity: Department of Veterans Affairs, VISN 6, Salem VA Medical Center, Nature and/or Description of the Action Being Processed: This procurement is for surgical tables with accessories of 4 each STERIS 5095 SURGICAL TABLES. These specifically made OR tables are for operating room surgical procedures that will be able to serve multiple specialties such as orthopedics, general surgery, ENT, Plastics, GYN, Vascular, Thoracic, etc. that can easily be put in many different positions needed for the procedures such as Lateral, Lithotomy, Fowler s position, Trendelenburg, prone, and supine. Description of Supplies/Services Required to Meet the Agency s Needs: Equipment - Steris 5095 model surgical tables : a. STERIS 5095 Surgical Tables, 120V Stainless steel base, featherweight Leg section, Mushroom Top attachment for mattress pads, 2 thick PRS pads. 4 each, part # ST015221022. b. Standard Accessory Package with 2 Anesthesia Armboard Pad and Extra Long Restriant Strap 4 each, part # BF425. c. STERIS ShroudGuard Column Protector for 5000-Series Surgical Tables 4 each, part # BF069. Description - STERIS ShroudGuard Column Protector for 5000-Series Surgical Tables. d. Power Lift Stirrups (Pairs),- 2 pairs, part # BF453. e. Power- lift Stirrup Storage Cart 2 each, part # BF46, $414.87 each Description - This cart provides stable, lightweight, portable storage for your surgical stirrups, table clamps and other leg holder accessories; the pre-configured handles allow safe and easy transport. The cart was designed with an integral set of support prongs that accommodate either the Power-lift Stirrups or the Bariatric Power-lift Stirrups. f. Split leg Package 1 pair to fit the 5095 surgical tables attachment mechanism, part # BF537. g. Powerlift Beach Chair (for 4085 and 5085 Series Steris surgical tables, - 1 each, part # BF590. h. Shoulder Chair Dolly to transport the powerlift beach chair from room to room 1 each, part # BF046. Statutory Authority Permitting Restricted Competition: FAR 13.5 Simplified Procedures for Certain Commercial Items. The statutory authority for applying the Simplified Procedures for Commercial Items of FAR 13.5 is 41 U.S.C. § 1901 and is implemented by FAR 13.106-1(b)(2) for restricting competition on this procurement. Competition is restricted on this procurement for the reason below: ( x ) Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements. ( ) Unusual and Compelling Urgency ( ) Industrial Mobilization, Engineering, Developmental or Research Capability or Expert Services ( ) International Agreement ( ) Authorized or Required by Statute ( ) National Security ( ) Public Interest Demonstration that the Contractor s Unique Qualifications or Nature of the Acquisition Requires the Use of the Authority Cited Above: This specific model of surgical tables and accressories has unique features needed for this procurement to meet the needs of the Operating Room. This model table has an Auto Limit Sensor designed specifically by Steris that is a collision avoidance safety system. This features prevents collisions during articulation of the tabletop and correction to the intended articulation operated by the hand controls. The universal hand control of the table is intuitive with easy to read LCD display, communicates instantaneous to the table positioning data without having to lift the sterile drape. The hand controls buttons are also color coded to help staff find the right desired operational button in a hurry and the controller has a backlite screen for the user to see in a dark room when procedures require dark light. This model table has seamless side rails without exposed screws and has mushroom cap technology instead of Velcro strips to keep mattress pads from moving during patient positioning. This feature makes cleaning much easier, minimizing fluid invasion, and faster cleaning for in between patient surgical procedures. This allows for a much cleaner, safer environment for the patient minimizing infection and faster turn-over time increasing efficiency in the operating room. This specific model Steris 5095 has an option that no other manufacturer can provide, of the ShroudGuard Column Protectors which as a fabricated durable stainless steel, installed by the factory, to protect the OR table Column Shroud from other equipment interference. We have had OR tables from other manufacturers and experienced columns to collapse with a patient on the table due to staff putting small pieces of equipment on the base of the tables, this is a safety hazard. The model 5095 table is made to hold a patient weight capacity up to 1200lbs that will accommodate our larger patient population. The table also has a wide range of C-arm access to the surgical team via a narrow base column, radiolucent tabletop, and a slide that provides the largest imaging window on the market. The tables have a large clinical work area of an image area of 83 total body imaging window. The table is a 4 section motorized table-top with a powered radiolucent kidney elevator for lateral positions such as kidney removal surgery. This model table has a height adjustment from down to 23 from the floor and still raise to 42.5 tall, this allows the patient to be moved to the OR table with ease from a standing position or from a wheelchair. The table can then be positioned to a height that will eliminatie the need for the surgon to bend down to the surgical site decreasing fagigue and potental neck and back injury for the surgon. The 5095 table has removable leg section without protruding pins allowing the patient s legs in stirrups or lithotomy position for perineal rectal surgical procedures providing unrestricted perineal access - this space is tight to work in. The leg section is also light weighing 14 lbs instead of over 25 pounds with other manufacturer tables, keeping staff safety in mind from back injuries when needing to remove leg section when patient is on the table For the given reasons and features of this specific model, the operating room at Salem VA feel that this specific equipment meets the needs of the staff and will provide better healthcare for our veterans. Description of Efforts Made to ensure that offers are solicited from as many potential sources as deemed practicable: This is a brand name requirement and all authorized distributors will be contacted until the rule of two can be meet. Determination by the CO that the Anticipated Cost to the Government will be Fair and Reasonable: The required tables are on a GSA contract 36F79719D0018. Description of the Market Research Conducted and the Results, or a Statement of the Reasons Market Research Was Not Conducted: Any Other Facts Supporting the Use of Other than Full and Open Competition: VAMC Salem has other tables for the last 10-12 years and their experience with them has not been good. The service for repairs is too long, about 2-3 months, when a table is broken, thereby, shutting down an operating room. VAMC Salem has also had issues getting parts for biomed to do minor repairs. VAMC Salem expiernced existing table columns to collapse with the patient breaking the column, causing a major repair, not to mention a safety concern for patients. VAMC Salem has reliability issues with a table that has been down for repairs several times. This is very concening since the table is only 8 years old. VAMC Salem biomedical has not been able to get sericve in a timely manner from this company. Service and repairs also play a part in this decision. The service response from Steris is prompt for the OR lights and other Steris equipment in the hospital. Typical turn around for repair is 2-3 days. This is very important to keep the OR rooms up and operational. For these reasons VAMC Salem finds the Steris model 5095 as the only acceptable surgical table. VAMC Salem choose this model table for the weight compacity, the mushroom top mattresses which other vendor manufacturers do not have, and the shroud column option installed on tables at the factory, as well as, the wide area of access for the C-arm Flouro capability, without having to rearrange the table sections for different procedures. VAMC Salem feels this will saving time and reduce issues during surgery. A Statement of the Actions, if any, the Agency May Take to Remove or Overcome any Barriers to Competition before Making subsequent acquisitions for the supplies or services required: No action is considered at this time.
