Amendment 0001: This amendment extends the offer due date�to September 01, 2021 at 11:59 PM (EST).� In addition, there will be a second set of Questions and Answers conducted. The second set of questions will be due August 29, 2021 at 11:59 PM (EST). The Period of Performance has been updated to account for the extended offer due date.�See the revised version of�Section B (attachment) for Period of Performance details.� Acknowledge Amendment 0001 to the solicitation within proposal submissions.� ______________________________________________________________________________________________________ (i)�� �This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued.� (ii)�� �Solicitation number 75H704121R00022 applies, and is issued as a Request for Proposal (RFP).� (iii)�� �This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-06.� (iv)�� �This requirement is a total Woman Owned Small Business set-aside and the associated NAICS is 541690 - Other Scientific and Technical Consulting Services, with a small business size standard of $16.5M. (v)�� �See the attached Additional Terms and Conditions � Section B, for the pricing structure. �� (vi)�� �This labor hour contract is to acquire the contractor support services for: ongoing software quality assurance and software quality control; software testing and installation; training coordination; user support; documentation remediation; and release management for the existing Resource and Patient Management System (RPMS) applications. See the attached Additional Terms and Conditions � Section C, for further details. � (vii)�� �Delivery and acceptance terms for this order, when applicable, is FOB Destination.� (viii)�� �The provision at 52.212-1, Instructions to Offerors � Commercial Items (Jul 2021) applies to this acquisition.� (ix)�� �52.212-2, Evaluation -- Commercial Items (Oct 2014) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1) Technical Capability; 2) Key Personnel & Management Approach; 3) Relevant Past Performance; and 4) Price. Technical Factors, when combined, are less important than price. All non-price factors are of equal importance. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party.
