- Statement of Work as of 7/13/2021 Contract Title: Quarterly and Annual Ventilation Surveys at the Erie VAMC Background: This new service contract shall consist of a base year and four (4) option years. Current five (5) year ventilation Contract # VA244-16-P-7232 ends on 9/29/2021. Period of Performance: Base Year: 9/30/2021 9/209/2022 Option Year 1: 9/30/2022 9/29/2023 Option Year 2: 9/30/2023 9/29/2024 Option Year 3: 9/30/2024 9/29/2025 Option Year 4: 9/30/2025 9/29/2026 Specific Tasks: The contractor will perform a survey of the ventilation Quarterly for: Entire Sterile Processing Service (SPS) Decontamination Prep & Pack Steam maintenance Room ETO Sterilizer Room ETO Maintenance Room Clean Storage Sterile Storage Main Entrance/Lobby Bio Med Work Center Surgical Suites Endoscopy Room 1 Endoscopy Room 2 Operation Room 1 Operation Room 2 PACU 3 Laboratory Histology Room Morgue Room Pharmacy Antei-Room Clean Room Chemo IV Hazardous Room Isolation rooms: 6th Floor room 630 6th Floor room 631 5th Floor room 533 Urgent Care Center Exam Room 2 Exam Room 3 Exam Room 4 Exam Room 5 Exam Room 6 A floor plan of each area will be provided by the VAMC. The ventilation survey will provide the airflow measurements for: Supplies marked on the drawing as S with a circle around it. Returns marked on the drawings as R with a square around it. Exhaust marked on the drawing as E with a square around it. A report will be due along with the drawings, the columns will be labeled as: Room Name Use the room names provided above. Actual Air Exchanges Number of air changes per hour. Minimum Air Exchanges Number of minimum air changes per hour required. Room Pressure Room pressure relationship of the room being measured, with respect to the area outside of the room. It will either be negative or positive, with an arrow showing direction. Pass/Fail Check mark or other symbol for PASS and FAIL for any areas that do not pass. The SPS area consists of a decontamination room, a sterile prep and pack room and several storage and work areas for a total of 9 area s, which are located on the 1st floor. The Surgical area to be surveyed consists of 5 rooms, which are located on the 2nd floor. The Laboratory area consists of 2 rooms to be surveyed on the 1st floor. The Pharmacy consists of 3 rooms, an Antei-room, a Clean room, and Hazardous room. The Contractor will provide a preliminary report, during the time of the survey, to determine if the ventilation requirements are met in the areas being measured. The contractor will perform Testing Annually for: Waste Anesthetic Gas Monitoring, N2O Leak Detection, Vacuum Terminal Performance, Nitrous Bank Leak Detection Endoscopy Room 1 Endoscopy Room 2 Operating Room 1 Operating Room 2 Formaldehyde Monitoring On-Site Test Report, Hazardous Communications, Medical Surveillance & Removal, Results and Recommendations. Laboratory Histology Autopsy Suite / Morgue Medical Piping Systems Evaluation, Carbon Monoxide (CO) Test, Carbon Dioxide (CO2), Halogenated Hydrocarbons (HC), Methane, Dew Point. 1. Surgical Suit OR 1 & 2 2. UCC 1st Floor 3. 6th Floor Services will be performed between the hours of 7:00am and 4:30 pm, Monday through Friday, unless otherwise approved by the Engineering COR. No overtime will be paid by the Government for services performed outside of these times, unless authorized by the Engineering COR. The contractor is to confirm the scheduled inspection with Engineering COR at least 48 hours prior to the planned visit date. Delivery Schedule: The Contractors representative shall report to the Medical Center Police Office to sign-in and obtain contractor identification badge prior to performing any testing. After obtaining a contractor s identification badge the Contractor's representative shall notify the Engineering COR to inform them of their presences on station. Upon completion of the service call the Contractor's representative and inform the Engineering COR that the services requested were completed. The final report shall include: 1) Date of service 2) Results of the testing 3) Recommendations to correct any failures 4) Signature of authorized VAMC official 5) Copy of all pertinent data and test results. FREQUENCY OF INSPECTIONS: All the rooms listed will be surveyed four (4) times per contract year, during the months of September, December, March, and June. TRAINING: All contractor employees and subcontractor employees requiring access to VA information shall complete the following before being granted access to VA information. Successfully complete the VA Privacy and Information Security Awareness and Rules of Behavior course and annually complete required security training. Training certificates will be submitted to the COR before the contractor can perform any work on site. See attachment A VA_Privacy_Training to complete the training.
