- This is a solicitation for commercial items or services prepared in accordance with the format in FAR Subpart 12.207, as supplemented with additional information included in this notice. The solicitation number is 75H70721Q00035 and is issued as a Request for Quotes (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The associated North American Industrial Classification is 541910. The Government contemplates to award a single award will result from this solicitation. DESCRIPTION: The intent of HCAHPS (Hospital Consumer Assessment of Healthcare Providers and Systems) is to obtain patient perspective on the hospital's inpatient care and to provide a standardized survey instrument and data collection methodology for measuring patient's perspective. SOLICITATION REFERENCE: In accordance with FAR 15.201 Exchanges with Industry before receipt of proposals, please refer to solicitation # 75H70721Q00035 when requesting information regarding the solicitation via email to shannon.eldridge-shorty@ihs.gov, phone number: 505-256-6768. All responsible offertory that respond to this solicitation is due August 20,2021 at 12:00 pm MDT to email address to Shannon.eldridge-shorty@ihs.gov. Question Submission: Interested offers must submit any questions concerning the solicitation at the earliest time possible to enable the Buyer to respond by email. All questions not received within a reasonable time prior to close of the solicitation may not be considered. If you should have any questions, please submit by August 18, 2021 by 5:00pm. Failure to comply with the below terms and conditions may result in offer being determined as non-responsive. Quote MUST be good for 90 calendar days after close of solicitation.
