- Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC_2021-05 3-10-2021. This solicitation is set-aside for: Small Businesses The associated North American Industrial Classification System (NAICS) code for this procurement is 332913, with a small business size standard of 1,000 employees. The FSC/PSC is 4510. The Northport VA Medical Center is seeking to purchase brand name Whitehall Anti-Ligature Bathroom Fixtures. All interested companies shall provide quotations for the following: Supplies: See attached Required items. Delivery shall be provided no later than 30 days after receipt of order (ARO). All quotations, shall be sent to Dayna Dickson-Miller Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. The following are the decision factors: Award shall be made to the quoter, whose quotation, offers the best value to the government, considering technical capability, past performance, and price. The government will evaluate information based on the following evaluation criteria: (1) technical capability factor meeting or exceeding the requirement, (2) past performance, and (3) price The award will be made to the response most advantageous to the Government. Responses should contain your best terms, conditions. Creating a ligature-resistant health care setting is important to preventing patient suicide and self-harm. The highest risk areas are those where the patient is alone and unsupervised such as a bathroom. Units 21/22 at the Northport VA Medical Center are undergoing renovations which will include new ADA compliant, ligature-resistance fixtures to improve the overall behavioral health space for the patient and comply with VA and patient safety regulations. This justification is for other than full and open competition for the procurement of brand name Whitehall Manufacturing ligature resistant bathroom fixtures.
