- (i) This is a Combined Synopsis/Solicitation for Brand Name CFPM 400 Rectangular Top Imaging Table as prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price. The solicitation number is 36C2442Q1129 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2021-05 dated 03/10/2021. This solicitation is set aside 100% Total Small Business and the associated NAICS code 339112 has a small business size standard of 1,000 employees. Schedule of Requirement/CLINS Statement of Need and Salient Characteristics This requirement is BRAND NAME OAKWORKS Bronchoscopy Procedure Table STATEMENT OF WORK Background: The Lebanon VA Medical Center (LVAMC) has a BRAND NAME requirement for one (1) CFPM 400-Rectangular Top Imaging Table with 2-inch Pewter Pad and Foot Control, Carbon Fiber Arm Boards, 2-inch replacement pad. The facility currently has 2 tables such as this utilized in the Pain Procedure Clinic, however, an additional table is needed in the Endoscopy Suite for a new type of Bronchoscopy Procedure that requires a non-metal table. Scope of Work: The Oakworks Medical Imaging Table, model CFPM 400-Rectangluar Top, is the table that will provide all of the characteristics required by the facility and providers in this field. A non-metal procedure table is needed to safely utilize the new Super Dimension Bronchoscopy procedure and this table has all the needed characteristics. The table will also be utilized in the Endoscopy Suite. Weight Capacity Multi-Function Hand Control Foot Pedal 2 Radiolucent Comfort Foam Tabletop Pad 7.5 Wide Arm Boards 4 Locking Casters Patient Safety Straps FDA Listed CE Marked cETLus approved CB Certificate Additional Information: The Oakwood Table has been researched and compared to other tables, and found to be, by the subject matter experts, to be the table that will provide the best functionality, durability, and most importantly, safety for providers and patients. This is also the table we currently utilize in other areas and will provide no further training for use for the provider, and continuity of care for the Veteran regardless which location they are cared for. Delivery: Lebanon VA Medical Center, 1700 S. Lincoln Ave., Attn; Warehouse, Lebanon, PA 17042-7529. NOTE: BioMedical Engineering will need to perform a safety check and enter the equipment into the system for annual Preventive Maintenance (PM) checks. Placement in the LVAMC clinic shall be coordinated through the Bio-Medical Engineer, and Medicine Care Line Business Manager. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufacturers or gray market items will be acceptable. Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty, and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right to enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
