Description
1. Introduction: A. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Any questions or concerns regarding this solicitation should be forwarded in writing via e-mail to the Point of Contact listed below. B. This solicitation is not restricted to any particular business size requirement (i.e., businesses of all sizes are eligible to compete for this opportunity). The applicable NAICS code is 541380 and the PSC is H966. C. This solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular 2021-06. The full text of the provisions and clauses can be obtained from the following websites: http://www.acquisition.gov/far/index.html and http://www.va.gov/oal/library/vaar/. 2. Date & time quotes are due: August 30, 2021 at 11:00 AM Eastern. Late submissions shall be handled in accordance with the provision at FAR 52.212-1(f). 3. Responding to this solicitation: Offeror must submit its quote via Email to James.Whisner@va.gov. Offeror must provide the following information with its quote: A. Legal entity name, address, DUNS of your company. B. Information that demonstrates your company has the resources to perform the instrument calibration services itself in-house, within a 15-day turnaround time and in accordance with industry standards. E.g.,-- (1) Copy of your company s radioactive materials license. (2) Sample calibration certificate with your company s name on it. (3) Description of your company s facilities, equipment, radioactive isotope sources, industry/ISO certifications, and standards used for testing and calibrations. C. Your company s shipping instructions and instrument return procedures that customers must follow when sending/returning instruments for calibration and repairs. D. Completed Price Schedule for CLINs 0001 through 4006 (see the Price Schedule attachment). Enter the unit prices and amounts (Amount = Quantity X Unit Price) for each CLIN and the Grand Total on the accompanying Price Schedule document. For the instrument return shipping and instrument repair CLINs, please provide information explaining how your company will determine the instrument return shipping costs and instrument repair costs (e.g., price lists). E. A statement regarding the terms and conditions herein as follows: (1) The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition; or, (2) The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following: (offeror shall list exception(s) and rationale for the exception(s), if any.) 4. Evaluation-Simplified Acquisition Procedures: The Government will evaluate quotes using the comparative evaluation process outlined in FAR 13.106-2 (b)(3) and price reasonableness basis outlined in FAR 13.106-3(a), where quotes will be compared to one another to determine which provides the best benefit to the Government. The Government reserves the right to consider a quote other than the lowest price that provides additional benefit(s). A quote may exceed minimum requirements of the solicitation. The Government reserves the right to select a quote that provides benefit to the Government that exceeds the minimum requirements of the solicitation but is not required to do so. Each response must meet the minimum requirements of the solicitation. The evaluation will consider the following: A. A quote will be evaluated to the extent to which it can meet and/or exceed the Government s requirements as outlined in the work statement and based on the information the offeror submits in response to paragraph 3 of this solicitation. B. The Government will evaluate quotes by adding the total price for all options to the total price for the basic requirement. The Government may determine that a quote is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). C. Veterans Involvement per VAAR 852.215-70. 5. Description of Work: A. VHA Network Contracting Office 5 is seeking to procure a 5-year contract for radiation survey instrument calibration and repair services for the VHA Office of Emergency Management (OEM) Medical Emergency Radiological Response Team (MERRT). Contractor shall provide all resources (e.g., personnel, equipment, supplies, facilities, tools, materials, supervision, and other items and non-personal services) necessary to perform annual radiation survey instrument calibration services within a 15-day turnaround time, and repair services as needed, on these types of radiation survey instruments: Ludlum Model 2241-3 or 3001-3 & probes; Ludlum Model 2224-1 & probes; Ludlum Model 2401-P or other Geiger-Mueller survey instrument; Ludlum Model 52-5-1; and Polimaster PM1703-1 or other Personal Radiation Dosimeter. (See the attached Work Statement for additional details.) B. Contract Line Item Numbers, quantities, unit of measure: (see the Price Schedule attachment). 6. The provision at 52.212-1, Instructions to Offerors Commercial (JUL 2021), applies to this acquisition, including the following addenda: A. 52.204-7, System for Award Management (OCT 2018); B. 52.204-16, Commercial and Government Entity Code Reporting (AUG 2020); and C. 52.204-24, Representation Regarding Certain Telecommunications and Video Surveillance Services or Equipment (OCT 2020). 7. Offeror Representations and Certifications: Offeror must complete annual representations and certifications electronically via the System for Award Management (SAM) website in accordance with FAR 52.212-3. Offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certification electronically in the System for Award Management (SAM) accessed through https://www.sam.gov. If the offeror has not completed the annual representations and certifications electronically, the offeror shall complete only paragraphs (c) through (u) of this provision and submit it with its quote. 8. The provision at 52.222-48, Exemption from Application of the Service Contract Labor Standards to Contracts for Maintenance, Calibration, or Repair of Certain Equipment-Certification (May 2014). Offeror shall check does or does not certify that it meets the exemption criteria. 9. If the offeror does certify to the conditions in paragraph (a) of provision 52.222-48 or comparable provision at 52.212-3(k)(1) and the Contracting Officer determines in accordance with FAR 22.1003-4(c)(3) that the Service Contract Labor Standards statute will not apply to the offeror, then the Service Contract Labor Standards clause in this solicitation will not be included in any resultant contract. Rather, 52.222-51 will be included in the resultant contract.
