- Page 2 of 2 General Requirements: Brand name or equal Locker is needed for the San Antonio, TX in accordance with the following requirements: ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 1.00 UN $000.0 $00.00 TL-10H-A - PERFORMANCE LOCKER 10' - HEAVY - ALPHA LOCAL STOCK NUMBER: TL-10H-A GRAND TOTAL $00.00 Salient Characteristics equal to Beaver Fit Locker 10 PERFORMANCE LOCKER TRAIN UP TO 60 PERSONNEL FREESTANDING WITH ADJUSTABLE FOOTPADS INTEGRATED, LOCKABLE STORAGE WEATHERPROOFING (HOT DIP GALVANIZED / POWDER COATED) SIDE MOUNTED BRACKETS 7 GAUGE STEEL 3X3 TUBING NATO CERTIFIED AIR PORTABLE STATEMENT OF WORK (LOCKER) GENERAL: The purpose of this contract is for the delivery and set-up of the Performance Locker. SERVICE: Description of work: Contractor will provide delivery and complete installation of all parts related to this item. Will arrange for any required heavy equipment to complete the installation. Installation and set-up is part of the total cost. Leaking from the circled area. Hours of Performance: Services to occur Monday through Friday between 8am and 4pm excluding holiday s and weekends. Work schedules shall be communicated with the government point of contact on a daily basis until services have been completed. Check in Requirements: Check-in with VA police and PTRP Program Manager Crystal Goudeau crystal.goudeau@va.gov. Period of Performance: Work to be completed within 5 working days from the date of item delivery. Place of Performance: Polytrauma Transitional Rehabilitation Program - 4949 Gus Eckert Rd, SA 78240, RESTRICTIONS: Information technology security requirements: No use of VA information systems will take place. Information Systems Officer, Information Protection: The contractor will NOT have access to VA Desktop computers and will NOT have access to online resources belonging to the government while conducting services. Privacy Officer: The Contractor will NOT have access to protected Patient Health Information (PHI) and will NOT have capability of accessing patient information during the services provided to the VA. If removal of equipment from the VA is required, any memory storage device will remain in VA control and will not be removed from VA custody. All research data available for Contractor analysis is de-identified. Records Manager Restrictions: There will be no federal records created, maintained, used or dispositioned with this contract; regardless of format (paper, electronic, etc.) or mode of transmission (e-mail, fax, etc.). Safety Requirements: In the performance of this contract, the Contractor shall take such safety precautions as the Contracting Officer may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contracting Officer shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when served to the Contractor or his representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work and hold the Contractor in default. 52.211-6 Brand Name or Equal. As prescribed in 11.107(a), insert the following provision: Brand Name or Equal (Aug 1999) (a) If an item in this solicitation is identified as brand name or equal, the purchase description reflects the characteristics and level of quality that will satisfy the Government s needs. The salient physical, functional, or performance characteristics that equal products must meet are specified in the solicitation. (b) To be considered for award, offers of equal products, including equal products of the brand name manufacturer, must- (1) Meet the salient physical, functional, or performance characteristic specified in this solicitation; (2) Clearly identify the item by- (i) Brand name, if any; and (ii) Make or model number; (3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and (4) Clearly describe any modifications the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modifications. (c) The Contracting Officer will evaluate equal products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. (d) Unless the offeror clearly indicates in its offer that the product being offered is an equal product, the offeror shall provide the brand name product referenced in the solicitation. (End of clause) VAAR 852.212-72 Gray Market Items: a) Gray market items are Original Equipment Manufacturers' (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical equipment only for VA Medical Centers. No remanufactures or gray market items will be acceptable. (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system, verified by an authorization letter or other documents from the OEM, such that the OEM's warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. (End of clause) FAR 52.212-2 Evaluation-Commercial Items: A copy of the authorized distributor letter from the contractor to verify that you are an authorized distributor of the products/services SHALL BE SUBMITTED WITH THE QUOTE AND IS MANDATORY.
