- Page 3 of 3 This is an amendment to the combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The purpose of this amendment 0001 is to correct the closing date of this solicitation, please see below. The solicitation number 36C24821Q1565, 0001 is issued as a Request for Quotation (RFQ) in conjunction with FAR -- Part 13 Simplified Acquisition Procedures and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. The provisions and clauses in this document are in effect through the Federal Acquisition Circular 2021-04, January 19, 2021. This solicitation intends to award a contract to a SDVOSB/VOSB business associated with NAICS Code 334510 with a size standard of 1250 employees. The Department of Veterans Affairs (VA) seeks to make a procurement for a Brand Name or Equal to GE Venue Go R3 Ultrasound for the West Palm Beach VA Medical Center, West Palm Beach FL. Below are the standard items or equal necessary for the quote. H8041VD Venue Go R3 Focus Package 1 EA H40462LF L8-18i-RS Linear Array Probe 1 EA H48062AB L4-12t-RS Linear Array Probe 1 EA H40482LJ C1-5-RS Wideband Convex Array Probe (USA POC Only) 1 EA H45181PC Venue Go B&W Sony Printer UP-D898DC Kit 1 EA H8041VE Venue Go Cart 1 EA Offeror quotations must include credit for trade-in, if available for GE Logic E System. This must be reflected in the overall quoted amount. Required salient characteristics/standards are contained in the attached and listed below. Comparative evaluations will be performed after all quotes have been received. SALIENT CHARACTERISTICS: Equipment must have the physical characteristics to enable the government to perform the capabilities: The West Palm Beach VA Medical Center has a requirement for a Point of Care compact, phased, linear array ultrasound imaging device intended for use in the Pain Management and Rehab Clinic. The system must provide image generation in 2D, Color Doppler, M-Mode, Color M-Mode, PW, and Tissue Velocity images. The ultrasound must also have a touch screen interface, and be fully digital, allowing usage of all scanning modes throughout all operating frequencies. Imaging Modes 2D Gray Scale and 2D Color Flow Imaging M-Mode Gray Scale Imaging Color M-Mode Doppler Different Combination of the above methods Probes Linear Array Probe for imaging superficial and deep structures Linear Array Hockey Stick probe with high frequency for superficial and small structures Wideband Convex Array Probe for imaging mid to deep structures Mechanical Design Adjustable stand that allows the system to be placed on a flat horizontal surface to allow tilting the system within a range of 20 to 70 deg Cart Design that can fit into the confined spaces found in the Point of Care environment Printer Medical-grade black-and-white digital printer with ability to attach to ultrasound cart Storage Solid State Hard Drive (SSD) that is partitioned into 4 logical drives each with different operations Warranty 3-Years Warranty or more Offeror quotations must demonstrate meeting the salient physical, functional, or performance characteristic specified in this solicitation, otherwise it will be considered non-responsive. The information identified provided is intended to be descriptive, not restrictive and to indicate the quality that will be satisfactory. The interested parties bear full responsibility to ensure their submission demonstrates to the government that they can provide the supplies being requested. Only new equipment is acceptable; no remanufactured, used/refurbished or ""gray market"" items. No product in development shall be considered. All items must be covered by the manufacturer's warranty. At a minimum the quote shall contain the following: The name, address, and telephone number of the vendor. Tax Identification Number (TIN) DUNS Number / Cage Code Point of Contact for submission of orders if awarded a contract Authorized Distributor Letter certified by OEM.
