- Page 13 of 13 This is a SOURCES SOUGHT ANNOUNCEMENT ONLY The intent of this Sources Sought Announcement is to assist in our decision-making process for services procurement. This is not a solicitation, nor request for quotes or proposals. Through the receipt of responses, this will enable a more precise type of procurement process. The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award of a contract, nor does it restrict the Government to a particular acquisition approach. Any inquiries are to be made in writing by email to the point of contact, Larry Facio, Contracting Officer. Responses to this notice shall be sent by email to larry.facio@va.gov by the due date and time of 08/30//2021 by 12:00 p.m. (Pacific Time). This is for a Base Year Plus Three Option Years contract, possibly Base Year Plus Four Option Years. Background/Introduction: The Contractor shall provide a mobile actively shielded 16 slice CT Scanner at the Community Based Outpatient Clinic, Eureka, CA. This will include the transportation, set up/removal, Full-time Diagnostic Radiology Technologist, and inspection/certification by a health physicist. The date of lease will be from January 01, 2021 through December 31, 2022 plus three option years. The scanner will include supporting equipment to provide the doctors and radiologists a like system which will provide the same level of care our patients are presently receiving. Services shall be provided for the Department of Veterans Affairs San Francisco VA Health Care System. Place of Performance: VA Eureka Community Base Outpatient Clinic 930 W Harris Eureka CA 95503 Requirements: Equipment and Services: Contractor will provide a vendor owned portable CT scanner (minimum features seen below) in a trailer not to exceed 50 in length including transportation to and from the location and PACS/phone/ connectivity/hook up. Trailer needs to connect successfully to already built entry ramp with specified floor height of the ramp being 45 and width of ramp doorway being 96 . Power cable 50 extension must be available if initial power cable isn t sufficient to reach power junction Russell Stoll connection. Possible use of a short cab should be available for maneuvering in and out of use destination due to tight parking requirements if necessary. Vendor will perform a site visit prior to delivery of the CT trailer. Have the resources available to switch to a smaller truck (Yard Goat Semi-Trailer) to position the rental CT trailer in its final position, if necessary. 16 slice Scanner (preferred) With Minimum Features: 16 slice detector preferred XR-29 compliant Interleaved Volume Recon Sure View Dose Map Fast Planning Iterative Reconstruction Fast Positioning 456 lb. table weight limit Fast KV 0.48 High Speed Rotation Fast Wizard Low Dose Lung Scanning Dual CT Injector Contractor will have 24-hour customer telephone support Scanner entrance on scanner side Most current version of software Compatible with Windows 7 Operating System Contractor will provide Diagnostic Radiology Technologist (DRT) that shall meet or exceed recognized national standards established by the Joint Commission (JC) and certified by both the American Registry of Radiologic Technology (ARRT) and the California Registry of Radiology Technologist (CRRT). Meets the qualification standards for the GS 8 as defined in VA Handbook 5005, Part II, Appendix G Diagnostic Radiologic Technologist Qualification Standard . All continuing education courses required for maintaining certification must be kept up to date always. Documentation verifying current certification shall be provided by the Contractor to the VA COR. Contracted radiology technologist(s) will be responsible for the performance and coordination of Computerized Tomography (CT) examinations, which include examinations of the skull, skeletal spine, abdomen, chest, skeletal bone, and all extremities of the skeleton. Determines technical factors (i.e., calculates the correct exposure factors of time, distance, kilovoltage, and milliamperage), sets, controls and operates radiographic equipment ensuring that the portion of the body specified receives the correct calculated exposure. Sets up, adjusts accessory equipment required and makes exposures necessary for requested procedures. Contracted DRT is responsible to the radiologists and supervisors for the quality and quantity of computerized tomography exams performed to assure a high level of acceptability of radiographs while minimizing patient radiation dosage and reducing costs. Hours of Coverage: Normal hours of coverage are (Monday through Friday) from 8:00am to 4:30pm, excluding holidays. Work performed outside the normal hours of coverage at the request of Radiology would be billed at the government rate. A separate purchase order will be issued to cover the cost(s) associated with any after-hours call back service as applicable. Competency of Personnel Servicing Equipment: A. Each respondent must have an established business, with an office and full-time staff. The staff includes a ""fully qualified"" FSE and a ""fully qualified"" FSE who will serve as the backup. B. Fully Qualified"" is based upon training and on experience in the field. For training, the FSE(s) shall have successfully completed a formalized training program, for the equipment identified in Section B. For field experience, the FSE(s) shall have a minimum of two years of experience, with respect to scheduled and unscheduled preventive and remedial maintenance on equipment identified in Section B. C. The FSE(s) shall be authorized by the contractor to perform the maintenance services as required. All work shall be performed by ""Fully Qualified"" competent FSEs. The contractor shall provide written assurance of the competency of their personnel and a list of credentials of approved FSEs for each make and model the contractor services at the VAOPC. The CO may authenticate the training requirements, request copies of training certificates or credentials from the contractor at any time for any personnel who are servicing or installing any VAOPC equipment. The CO and/or the COR specifically reserve the right to reject any of the contractor's personnel and refuse them permission to work on the VAOPC equipment. Response Method: The Government requests capability statements and comments from interested businesses regarding the requirements described above. Responsible sources are encouraged to submit a response to this notice with a statement of interest on company letterhead. When responding, in Subject line insert: Sources Sought Eureka CT Scan At a minimum, the following information shall be provided: 1. Company Name; 2. Company Mailing Address; 3. Point(s) of Contact including telephone number(s) & email address(es); 4. Socio-Economic (i.e. Small/Large Business, HUBZone, Service Disabled Veteran Owned, 8(a), etc.) as it relates to NAICS Code 621512 ($16.5 Million). 5. Provide a summary of the type of services performed and experience as it relates to Equipment Rental of CT Scanning Equipment and Temporary Service Providers for said equipment. 6. DUNS Number 7. Additional information and/or comments. Veterans First Contracting Program and the VA Rule of Two (Kingdomware v. United States) 38 U.S.C. 8127 - 8128: Service-disabled veteran owned small business (SDVOSB) or Veteran owned small business (VOSB) concern must be registered and verified in VA's Vendor Information Pages (VIP) database at www.vip.vetbiz.gov to be eligible for award as a SDVOSB or VOSB if/when a solicitation is issued for this requirement as a SDVOSB or VOSB set aside. In addition, this requirement has been determined to be set aside as SDVOSB or VOSB acquisition, only SDVOSB or VOSB that respond to this specific notice with in the above stated due date will be eligible for award. As this is a service acquisition, the Small Business Administration and the Federal Acquisition Regulation have a Limitations on Subcontracting requirement. For all small businesses, to include SDVOSB and VOSB, At least 50 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern. Contractors must be able to quickly identify, screen, and process employees for rapid on-boarding with the healthcare facilities identified. Contractors must be able to quickly adapt to changing needs.
