Description
1. Description of Agreement a. This is a Blanket Purchase Agreement (BPA) for Occupational Health Services as described in Federal Acquisition Regulation (FAR) Part 13. This BPA establishes the terms and conditions for the services to be provided. b. Service requests made under this BPA will be referred to as Calls. c. These services are to be performed primarily in Tarrant County, Texas d. The duration of this BPA shall be five years from award. Calls can be placed up to the date of the BPA expiration. The period of performance of any Call placed prior to the BPA expiration can extend past the date of the BPA expiration. e. Price Schedules will be included in this BPA, one for each year. By signing this BPA the contractor agrees to accept the prices on the appropriate Price Schedule for services ordered and not to charge any additional fees. 2. Extent of Obligation: The Government is obligated only to the extent of authorized purchases actually made under the BPA. The establishment of the BPA does not imply any agreement on the part of the Government to place future Calls with the BPA holder. The Government makes no representation as to the number of Calls or actual amount of work to be ordered. 3. Purchase limitation: The maximum amount for each individual Call is $1,000. 4. Individuals authorized to purchase under the BPA: A list of individuals authorized to purchase under this BPA will be provided after award by the contracting officer. Do not accept calls unless it is signed by an authorized individual. 5. Call Procedures: USACE may have multiple BPAs in place with various Contractors for the same services. USACE intends to rotate calls among the BPA holders, considering firms� capacity and quality of performance on previous calls. 6. Delivery Tickets: All deliverables provided under the agreement shall be identified by the following minimum information: See attached solicitation for full details. AMENDMENT 0001 -�The purpose of this amendment is to provide responses to prospective vendor inquiries.
