This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2020-04 (effective 1-15-2020). The associated North American Industrial Classification System (NAICS) code for this procurement is 339112 with a small business size standard of 1000 EMP The Jackson, MS Veteran Affairs Medical Center (VAMC) needs ultrasound tables. The delivery address is: Jackson VAMC (586) 1500 E. Woodrow Wilson Drive Jackson, MS. 39216. All interested companies shall provide quotations for the following: Supplies Line Item Description Quantity Unit Price Total Price 0001 Ultrasound Table 4 0002 Folding Side Rails 4 0003 Ergonomic sonography chair with foot ring 4 0004 Retractable Stirrups 2 Supply STATEMENT OF WORK INCLUDED IN GLOBO BELOW. All delivery shall be IAW attached Statement of Work. Place of Performance Address: Jackson, MS VAMC Postal Code: 39216 Country: UNITED STATES Award shall be made to the quoter whose quotation offers the specified product at the lowest price. The government will evaluate information based on the following evaluation criteria: lowest price technically acceptable. The SF1449 is included below and contains all applicable provisions for this solicitation. All interested vendors shall submit one copy of the quotation. All quotations must be submitted electronically to luke.miles@va.gov. This is an open-market combined synopsis/solicitation for supplies as defined herein. The government intends to award a single award, firm-fixed price contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition.""
