Combined Synopsis/Solicitation Notice 9 Page 9 of 9 COMBINED SYNOPSIS/SOLICITATION 36C25021Q1104 Responses are due by 13:00 (EST August 31, 2021) COMBINED SYNOPSIS/SOLICITATION NOTICE: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures; and supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-87, dated March 07, 2016. Solicitation 36C25021Q1104 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. This procurement will be issued as open market, set-aside Service-Disabled Veteran Owned Small Business/Veteran Own Small Business (SDVOSB/VOSB) but does not exclude other vendors, NAICS Code is 336120, Heavy Duty Truck Manufacturing. The Government is seeking a vendor who can provide a brand name or equal Lift-King Rough Terrain Forklift for the Dayton VA Medical Center. (see enclosed price/cost schedule). Place of Performance: Department of Veterans Affairs, Dayton VA Medical Center, 4100 W. Third Street, Dayton, OH 45428. Provisions 52.212-1, Instructions to Offerors Commercial and 52.212-2, Evaluation Commercial Items are applicable to this acquisition. This procurement is being conducted pursuant to FAR Part 13 procedures. In accordance with provision 52.212-2, the Government intends to award a contract resulting from this solicitation to the responsible offeror who offers the lowest price, whose price is found to be reasonable, and whose proposal does not take exception with any of the requirements for this procurement specified in the solicitation.
