- SOURCES SOUGHT SYNOPSIS DESCRIPTION This is a SOURCES SOUGHT SYNOPSIS and is for informational / market research purposes only. THIS IS NOT A REQUEST FOR PROPOSAL, QUOTATION OR BID, NOR A SYNOPSIS OF A PROPOSED CONTRACT ACTION UNDER FAR SUBPART 5.2. No reimbursement will be made for any costs associated with providing information in response to this synopsis or on any follow up information. NO SOLICITATION IS CURRENTLY AVAILABLE. The purpose for this synopsis is to conduct market research to help identify capable, potential sources and to establish if this requirement can be set-aside for small business or other socio-economic programs to include: Small Business, 8(a) firms, Historically Underutilized Business Zones (HUBZone), Women-Owned, Economically Disadvantaged Women-Owned Small Business (EDWOSB) Service-Disabled Veteran Owned Small Business (SDVOSB) and Veteran Owned Small Business (VOSB). NAICS Code: 541380 Testing Laboratories Small Business Size Standard: $16.5M PSC/FSC: H266 Equipment & Materials Testing Instruments & Lab equipment GENERAL INFORMATION: The Department of Veterans Affairs intends to procure Sound pressure level assessment for audiometric booths at the Seattle and American Lake VA Medical Centers. See attached Statement of Work for details. INSTRUCTIONS TO RESPOND TO SOURCES SOUGHT SYNOPSIS: Any vendor interested and capable of meeting this requirement should provide a written statement to the Contracting Officer. Vendor response must include at a minimum: Name of Business Point of Contact Name, Phone Number, and E-Mail Address CAGE Code and DUNS Number to show proof of registration in the System for Award Management (SAM) https://www.sam.gov Short Description of Capabilities VIP Verification (if SDVOSB or VOSB) Any response to this notice must show clear and convincing evidence of the Vendor s capability of meeting this requirement. Only authorized representatives of the manufacturer will be considered. Equipment must be new, no used or refurbished equipment accepted. Interested firms shall respond to this synopsis no later than 5:00 PM Mountain Standard Time (MST) on 09/02/2021. All responses shall be submitted electronically to Amy.Kuczajda@va.gov . Statement of Work Sound Pressure Level Assessment for Audiometric Test Booths. Background: The Occupational Safety and Health Administration (OSHA) 1910.95 Appendix D, requires that all Audiometric Test Booths used for employees audiograms be assessed for the Background Sound Pressure Level initially and annually thereafter. Rooms used for audiometric testing shall not have background sound pressure levels exceeding those in Table D-1 when measured by equipment conforming at least to the Type 2 requirements of American National Standard Specification for Sound Level Meters, S1.4-1971 (R1976), and to the Class II requirements of American National Standard Specification for Octave, Half-Octave, and Third-Octave Band Filter Sets, S1.11-1971 (R1976). TABLE D-1 - MAXIMUM ALLOWABLE OCTAVE-BAND SOUND PRESSURE LEVELS FOR AUDIOMETRIC TEST ROOMS _______________________________________________________________ Octave-band center frequency (Hz)............... 500 1000 2000 4000 8000 Sound pressure level (dB) ... 40 40 47 57 62 _______________________________________________________________ This contract is required to assess the Audiometric Booths at the following campuses in the VA Puget Sound Health Care System (VAPSHCS): Seattle Campus (Sea) 1660 South Columbian Way Seattle, WA 98108 American Lake Campus (AmL) 9600 Veterans Drive Tacoma, WA 98493 Contractor will conduct the SPL (dB) assessment using a type I precision Sound Level Meter (SLM) with octave band analyzer in slow response mode. The SPL measurements will be conducted with SLM equipment conforming to the Type 2 requirements of American National Standard Specification for Sound Level Meters, S1.4-1971 (R1976), and to the Class II requirements of American National Standard Specification for Octave, Half-Octave, and Third-Octave Band Filter Sets, S1.11-1971 (R1976). Contractor will obtain SPL measurements inside the booth during normal operational external conditions during activity levels that are representative of anticipated use conditions, including internal conditions (lights, fan and ventilation turned on). During the testing, contractor will document the conditions in the area of testing. Contractor will take recordings at a level determined to be the location of an employee s head, sitting in employee s chair with the SLM held away from the body. Contractor will record results for each required octave band in a frequency range of 500 to 8000 Hertz (Hz) and will compare with OSHA 1910.95 Appendix D, Table D-1. Upon completion of testing, contractor will prepare a brief email report providing the results in tabulated format. Contractor will provide a final written letter report within two to three weeks of the test, including an executive summary, assessment methodology, a table of test results, conclusions, and recommendations, if required.
