This is a Sources Sought notice for information only. This is not a solicitation for proposals and no contract will be awarded from this announcement. In preparation for future procurement for the VISN 15 Network, the Network 15 Contracting Office is performing market research to gain knowledge of potential sources and their size classification (service disabled veteran owned small business, veteran owned small business, hub zone, 8(a), small disadvantage business, woman-owned small business, small business, or large business) relative to NAICS 541611 Administrative Management and General Management Consulting Services that can offer same or similar services listed in the below draft Statement of Work. Capable companies should submit an electronic statement outlining their companies capability and capacity to provide the product Federal Supply Schedule contract or Open Market the specification of the surgical or medical supplies and the delivery time to deliver the products to VISN 15 Medical Centers. Also firms should provide their business size status in accordance with the small business administration website (www.sba.gov) for the above referenced NAICS code that fall within. Please submit statements of capabilities electronically to paul.hemenway@va.gov no later than Thursday September 2 @ 1000 CST. Phone inquiries will not be accepted. FSS Contract Numbers: provide contract numbers and indicate your ability to offer the requested items 100% through a FSS contract on the attached sample statement of requirements. Indicate which Schedules and SINs your contract is listed under and the FSS contract price for the items on contract. 7. Capabilities Statement: provide model numbers, standard features and options of equipment that can be offered. If there are any specifications that your product cannot meet, indicate in separate section. 8. Comments: provide comments regarding the statement of requirements that may be pertinent to the improvement of the statement of requirements. STATEMENT OF WORK KANSAS CITY VETERAN S ADMINISTRATION MEDICAL CENTER for THE JOINT COMMISSION ACCREDITATION SURVEY REVIEW DESCRIPTION OF SERVICES. The contractor shall come on the premises to the Kansas City Veteran s Administration Medical Center (VAMC) to learn what, if anything, it would need to attain Joint Commission (TJC) accreditations. CONTRACTOR RESPONSIBILITIES. The contractor will perform an assessment for the Kansas City VAMC using the latest published TJC (as applicable) guidelines noted in the Comprehensive Accreditation Manual, located at 4801 E. Linwood Blvd, Kansas City, MO for 4 days mid-October 2021. QUALIFICATIONS. Contracted assessor must be familiar with the TJC Comprehensive Accreditation Manual to include: hospital, behavioral health, and home health. EXPERIENCE. Highly recommend that the contractor have served on TJC staff and/or to have conducted at least three TJC inspection on all 3 (three) Hospital, Behavioral Health and Human Services, and Home Care 2021 TJC manuals or most current version. Contractor must be able to identify clinic(s) or section(s) out of compliance. Also highly recommend contractors have either clinical experience (i.e. a licensed nurse or physician) or Health Facilities experience. Experience assessing comprehensive medical treatment centers/organizations with complex programs is a must. ASSESSMENT SCHEDULING: Upon forward of contract, the assessment scheduling will be coordinated with the Executive of the Office of High Reliability (OHR) OBJECTIVES The contractor will assist the Kansas City VAMC to understand gaps between its current state and compliance with TJC accreditation requirements by generating a report within one week, post survey. This report is to be broken out be elements of Performance from 2021 TJC Comprehensive Accreditation Manual for Hospital, Behavioral Health and Human Services, and Home Care or most current version, and identify clinic or section out of compliance. The contractor will aid in the Kansas City VAMC s development of an action plan to close such gaps. The contractor shall collaborate with the Kansas City VAMC to help it structure effective TJC accreditation self-assessment activities. The contractor shall observe, assess, and evaluate current health care operations in relationship in meeting organization goals of quality and patient safety. The contractor shall identify important organizational and system issues related to safety, quality of care, treatment and services, and interfaces between those processes. The contractor shall provide consultative and educational information and summarize during the facility debriefings. METHOD OF ASSESSMENT AND DELIVERABLES: The contractor shall provide comprehensive expertise on-site consultation for a total of 4 (four) days with an MD, Nurse, Pharmacist, and EOC consultant, and then 2 (two) days with a Behavioral Health and Home Care consult. Pre-visit information gathering; and discussion with the Kansas City VAMC about current view of TJC accreditation, any potential timeline for accreditation being attained by the Kansas City VAMC, key organization subject matter-experts, and the facilities current knowledge of TJC 2021 Comprehensive Accreditation for Manual for Hospital, Behavioral Health and Human Services, and Home Care or most current version. The contractor shall provide a realistic on-site experience in scope and breadth, which includes daily debriefs. The contractor shall provide assessment of organizational approach in areas such as leadership, patient safety, infection prevention and control, medication management, emergency management, use of data and medical staff through interactive conferences (as identified in organization-specific agenda). The contractor shall provide utilization of individual and system tracers for evaluation of determined areas of focus and to review direct care processes, across services and disciplines, in key inpatient and outpatient clinical areas and the environment, with focus on understanding gaps between the Kansas City VAMC and Joint Commission requirements. Information will be gathered by making observations, conducting interviews, and reviewing selected documents. The contractor shall provide use of tracer methodology for evaluation of processes and practices as they relate to the organization and its services. The contractor shall review facility processes, focus on identified risk points, and the facilities development of strategies to minimize these risks. The contractor shall provide and overview of the key strengths and challenges that will be shared with the organization leadership. The contractor will use the SAFER Matrix Report and present to the organization that presents each of the standards/Elements Performance as they relate to their impact on patient safety. Strategies for compliance will be discussed at the time of the mock survey. The contractor shall provide a summary of consultative and educational information during the daily debriefings, followed by a written summary of the services, findings, and recommendations. GENERAL INFORMATION. HOURS OF PERFORMANCE: The contractor shall perform the assessment during business duty hours from 0700 16:30, Monday through Friday, excluding Holidays. The primary place of performance is the Kansas City VAMC, located at 4801 E. Linwood Blvd, Kansas City, MO 64128. The contractor shall submit an electronic invoice for the total Professional Fees approximately one to two (1-2) days following the conclusion of the engagement. CONTRACTOR DAMAGE AGREEMENT. The contractor shall take all possible precautions to prevent damage to Government property. All damaged areas shall be repaired at no additional cost to the Government as directed by the contracting Officer. The job site shall, be maintained to provide a reasonably clean and uncluttered appearance at all times. CONTRACTOR EMPLOYEES: The contractor employees shall present a neat appearance and be easily identified as contractor employees. The contractor shall obtain a Facility Visitor Pass for all employees who perform work at the Kansas City VAMC. Contractor personnel are required to wear or prominently display facility identification while visiting or performing work on the campus. The contractor shall not employ persons for work on this contract identified to the contractor by the Contracting Officer as a potential threat to the health, safety, security, general well-being or operational mission of the facility and its population. Each employee will be easily identified by wearing appropriate badges that contain the company name and employee name. The contractor will follow local and facility traffic laws while on any of the Kansas City VAMC campuses. COVID-19 PRECAUTIONS: The contractor shall follow local City, State, and Federal guidance in place regarding COVID-19 while on the Kansas City VAMC campuses to keep the employees and Veterans safe. Accordingly, the contractor will also: Participate in daily checkpoint screening while on campus. Maintain a 6-feet distance from others when feasible. Perform hand hygiene as ordered. Wear face masks and if recommended, personal protective equipment. Follow the directions of the hospital staff. The contractor employees will not be allowed on Kansas City VAMC campus if they have any of the following symptoms to include shortness of breath, high temperature, breathing difficulties, cough, flu-like symptoms, have returned from overseas travel within the past 14 days of arrival, or been in close contact with someone confirmed to have COVID-19.
