- Department of Veterans Affairs, Albany Stratton VA Medical Center, Network Contracting Office 2, is issuing this Notice of Intent to Sole Source in order to identify any potential sources that have the resources and capabilities to provide a comprehensive vehicle for full preventive maintenance, repair and software updates for nine units of Steris Equipment located at the Hudson Valley VAMC Castle Point Campus Sterilization Processing Service (SPS) location. SERVICES REQUIRED: Contractor shall provide annual service for preventive maintenance, repair and software upgrade for nine Steris systems at the Hudson Valley VAMC Castle Point Campus, 41 Wappingers Falls, New York 12590, during the term of October 01, 2021 through September 30, 2022 with provision for four (4) Option Years. Contractor shall provide services for Steris equipment. INTERESTED & CAPABLE RESPONSES: NCO 2 is seeking responses from businesses that are interested in this procurement and consider themselves to have the resources, rights, and capabilities necessary to provide these services. Please provide all of the following in your response: Business name, business type, socio-economic status (e.g., Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), etc.), person of contact (including telephone number & email address), GSA Contract number if applicable, and DUNS number. Please also include the North American Industry Classification System (NAICS) Code for which you propose to provide the service under for verification of your size standard. A Capability Statement which provides detailed, specific information that demonstrates understanding of the tasks required and capability to perform the tasks as stated in the attached Statement of Work. Failure to tailor capability statement to this specific requirement may result in your company being determined non-responsive. This Special Notice of Intent to Sole Source is for planning purposes ONLY. The results of this notice may assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., small business set-aside, full and open competition, etc.). VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted for this notice. STATEMENT OF WORK Steris Equipment Maintenance/ Service Contract BACKGROUND: The Department of Veterans Affairs, Hudson Valley VAMC - Castle Point Campus, is currently seeking full preventive maintenance services for nine units Steris Equipment located in the new Sterilization Processing Service (SPS) location. These units are used to sterilize surgical equipment for the Medical Center to ensure that all instruments and equipment are sterile and clean for use. In order to keep these units in optimal functioning order and to reduce malfunctioning, causing a lapse in functionality, the facility requires full semi-annual and quarterly service for preventive maintenance, repair, and software updates. SCOPE OF WORK: The contractor shall provide all labor, tools, equipment, travel, parts, and services necessary to accomplish the preventive maintenance, repair, and software updates for the nine Steris systems listed The following table displays the serial number, description, and required preventive maintenance frequency for each respective unit. Excluding the annual cleaning, each device s first PM must be completed within the first month of SPS s opening. After the first PM, the contractor must perform scheduled maintenance as frequently as outlined by the below guidelines. Scheduled preventive maintenance shall include, but not be limited to, full preventive maintenance services in accordance with OEM procedure, and annual sterilizer chamber cleanings needed for the applicable systems to ensure proper maintenance. The contractor shall follow the OEM s listed procedure when performing all scheduled maintenance services. This procedure shall be provided to the contracting officer representative (COR), or their designee, upon completion of the maintenance to ensure adherence to OEM procedure. If the unit s scheduled maintenance involves a PM checklist, this completed checklist shall be submitted to the COR or their designee upon PM completion. If parts are required, all parts supplied shall be compatible with existing equipment. The contractor shall use only new or remanufactured parts, approved by the OEM and shall not install used parts. Parts shall be ordered and sent in a timely manner to minimize equipment downtime. All scheduled maintenance shall be completed within one business day as to minimally effect SPS s workflow. These services shall begin during the base year of the contract as units one through nine require scheduled maintenance and the purchased warranty does not cover PM services. These services shall begin within 30 days of SPS s opening of the new location. The contractor performing scheduled maintenance shall submit OEM training documentation to the COR or their designee and the Chief Biomedical engineer. C. OTHER RELATED SERVICES (NOT INCLUDED IN THE STATEMENT OF WORK: Any services not expressly stated in this statement of work are not authorized. Services such as inspection, Original Equipment Manufacturer (OEM) required maintenance, or other maintenance and repair vital to the continued functioning of the systems may be added only after written modification from the contracting officer, unless they are inclusive to the requested services. No other person, including the COR or any other VA personnel are authorized to request additional work be performed by the contractor. Any additional services performed that are not authorized by the contracting officer will be considered inclusive. The contractor shall provide unlimited repair service visits on the Government owned units. The contractor shall respond to emergency requests for service by telephone within two hours of notification. This emergency request will come from the end user, COR or other Biomed personnel. If the issue cannot be corrected over the phone with the FSE providing technical support, the FSE shall commence on-site repairs within the next day of the emergency service request. D. OTHER REQUIREMENTS: The contractor shall: Be a firm regularly engaged in the repair, maintenance, and servicing of Steris equipment described in this Statement of Work. Have expertise, experience, and regular business to include the repair, maintenance of the equipment. Possess all the manpower, equipment, tools, and ready sources of parts as necessary to carry out the conditions of the requirement promptly, skillfully, and expertly If an unforeseen event occurs during scheduled or unscheduled maintenance that leads to equipment downtime, the following escalation plan will be triggered: Escalation Plan Level 1: If the unforeseen equipment defect cannot be repaired within two hours, the contractor shall contact the COR, their designee, or any other Biomed staff member. The contractor must provide a description of the issue, any parts needed to repair the issue and an expected resolution timeframe. Level 2: If the repair cannot be completed, the COR or their designee, or Biomed staff member shall notify the Biomed Chief Engineer. Level 3: If the repair is going to affect scheduling of patient care, the Biomed Chief Engineer shall convey this to the Hospital Director and the Front Office. SAFETY REQUIRMENTS: Upon completion of maintenance, the contractor is required to complete any safety or functional verification checks required by the OEM to ensure the device is operated according to OEM standards. If these checks require the use of test equipment, this test equipment shall be calibrated according to the respective equipment s standards. A copy of this equipment s calibration certification shall be provided to the COR upon request. The contractor performing scheduled maintenance must submit OEM training documentation to the COR or their designee and the Chief Biomedical engineer. The Contracting Officer or his/her designee will notify the contractor of any non-compliance with, VA Safety and Security Regulations and the foregoing provisions and the action to be taken. The Contractor shall, after receipt of such notice, immediately correct the conditions to which attention has been directed. Such notice, when served on the Contractor or his/her representative at the site of the work, shall be deemed sufficient for the purpose aforesaid. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all and any part of the work and hold the Contractor in default. SERVICE REPORTS: Upon completion of any work, the contractor shall provide a copy of the service report to the COR or their designee. These service reports at a minimum shall outline a description of the issue provided by the COR or their designee or the end user, a description of the device defect observed by the FSE, an itemized description of the performed services, an itemized description of the parts/materials/software used or replaced, the contractor s information, the FSE s information, the date and time of the beginning and end of the service, equipment downtime, hours on site, device name, serial number, model name and signature of the FSE who performed the work. The service report shall be submitted to the COR or their designee within twenty-four hours of repair completion. If the service report is not received, we will report this to Contracting and hold up payment to the contractor. SAFETY REQUIRMENTS: The contractor performing the work must be certified by the OEM to perform preventive maintenance, repairs, and all required safety and functional verification checks on Steris equipment according to OEM standards. A copy of this contractor s valid and current certification must be provided to the COR or their designee upon request. The contractor must ensure that the equipment functions in conformance with the latest editions of NFPA-99, OSHA, JACHO as well as OEM standards. If the contractor acts out of compliance with the foregoing requirements, the Contracting Officer or their designee will notify the contractor of any noncompliance and action to be taken. The contractor shall, upon receipt of such notice, immediately correct the conditions which led to the act of noncompliance. Such notice, when served on the contractor or their representative at the work site, shall be deemed sufficient for the purpose aforesaid. If the contractor fails or refuses to comply promptly, the contracting officer (CO) may issue an order stopping all and any part of the work and hold the contractor in default. INSPECTION: After scheduled or unscheduled maintenance is completed, the contractor shall provide a copy of the service report to the COR or their designee and the affected end user. The COR and affected end user must acknowledge the work performed by the contractor by signing the contractor s field service report. The COR shall perform quality assurance inspections to verify the performed work. These inspections may not occur every service date. If the COR completes an inspection on a service date it will be in the presence of the contractor prior to the contractor s site departure. The contractor shall inform the end user and COR when any maintenance is completed. The COR may appoint a designee to be notified in the event that the COR is not present during the service day. NATIONAL HOLIDAYS: Federal Holidays which are observed by the Federal Government are annotated below: - New Year s Day - Martin Luther King Jr. Day - President s Day - Memorial Day - Juneteenth - Independence Day - Labor Day - Columbus Day - Veteran s Day - Thanksgiving Day - Christmas Day And any other day specifically declared by the President of the United States to be a national holiday. PERIOD OF PERFORMANCE: All service shall be performed during normal business hours of coverage unless proper alternate arrangements have been coordinated beforehand with the COR. Work hours will be from 8:00 AM - 5:00 PM, Monday-Friday, excluding Federal Holidays.
