- This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number RFQ 36C25721Q1401 - Storage Reconfiguration is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular. SET-ASIDE: This requirement is a 100% Service-Disabled Small Business set aside under the authority of 38 U.S.C. 8127(d). In order to be eligible for award an offeror must be certified/verified in Vetbiz (www.vip.vetbiz.gov), and small for the applicable NAICS code at the time of the quote submission and at the time of award. Status as a qualified SDVOSB concern is under the authority of 38 CFR Part 74 in accordance with the VA Acquisition Regulation (VAAR) Part 819. The BD Alaris pump module is a large volume infusion pump that continuously or intermittently delivers fluids, medications, blood and blood products to patients. Clinicians can attach up to four infusion modules, allowing four independent infusions on a single BD Alaris PC unit. Description of Work: The contractor shall provide comprehensive services and supports for the VA Alaris system software as ongoing support services, education, infusion data reporting, software upgrade and maintenance, in accordance with the original equipment manufacturer specifications Field service engineers must be licensed or contracted with the original manufacturer in order to work on the equipment. Type of Contract: Firm fixed price Hours of Performance: Work will be completed on weekdays, excluding federal holidays, during the hospital s regular weekday business hours. Normal working hours are 7:00 am 4:30 pm, Monday through Friday. STATEMENT OF WORK The purpose of this project is to reconfigure the existing Spacesaver storage system to allow for more efficient storage space options, including taller/deeper shelves, locking filing drawers, and safer system closers. Certain items must be saved from the current Spacesaver system, including the LCD touchscreens and safety beam sensors to be reused with the new system. Once the existing system is dismantled, the equipment needs to be hauled away for disposal by the technicians. The floor rails need to be refurbished to ensure reliability with the new system. The replacement Spacesaver system will be precisely created for the correct amount of sensitive records to be held in locking file drawers and will include a master key override. The system will allow for small modifications to allow for shelf adjustments that hold banker s boxes and binders. After all work is completed, the contractor will have a final walkthrough with the VA COR or Alternative COR to ensure all work is completed and ready to receive payment. All work shall be done in accordance to the terms, conditions and specifications that meet NFPA 70 standards along with OSHA and EPA rules and regulations. Working Hours Work will be completed on weekdays, excluding federal holidays, during the hospital s regular weekday business hours. Normal working hours are 7:00 am 4:30 pm, Monday through Friday. Federal holidays observed during the execution of work may be exchanged for contractor observed days, only on a one- for-one basis, requested by the contractor and subject to approval by the contracting officer (CO). Scope of Work The contractor shall provide all personnel, services, vehicles, transportation, tools, equipment, materials, supplies, facility supervision, administration supervision and any other item(s) necessary to perform and accomplish this service per the requirements and specifications herein. The contractor shall be familiar with all the terms, conditions, and requirement herein contained. All work must comply with local, state, federal regulations. Period of Performance The contractor shall complete the work required under this SOW in 90 calendar days or less from the Notice to Proceed (NTP), unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor s proposal, the contractor s proposed completion date shall prevail. Contractor shall not work on Federal Holidays unless VA COR deems it necessary. Schedule power outages will be on weekends and cable splices will be on weekend. General Requirements The contractor must be a licensed and certified installer/integrator for Spacesaver Installation technicians must be 2021 calendar year certified by Spacesaver Storage Systems to work on Spacesaver Brand Mobile System Products and certification letter from Spacesaver Storage Systems must be submitted with bid. The contractor shall include a list of current contract vehicles with this bid. Installation technicians must be authorized Spacesaver distributors that can provide any parts needed to refurbish the system to like new condition during relocation, this includes a full system inspection and replacing any worn or broken parts such as but not limited to: drive/guide wheels, wiring, logic boards, control heads, safety sweep devices, drive shafts, guide bearings, electronic distance sensors etc. Structural members shall not be cut or altered, except where noted on drawings and/or specifications, without the authorization of the CO. Work remaining in place that is damaged or defaced during this contract shall be restored to the condition existing at the time of award of contract. The contractor shall provide all necessary barriers, safety equipment, and signage for both pedestrian and vehicle traffic impacted while work is in progress. All personnel engaged in the contract activities specified herein must be licensed by the state, local authority, and/or the city local authority in those trades, crafts, or professions that require licensing by such jurisdictions. The license must be of a grade or other level consistent with the requirement of the work being performed and/or as established by said jurisdictions. The contractor shall protect all walls, ceilings, floors, furniture, etc., from damage due to water, staining, or falling debris created during the coring/sawing process. The contractor will repair any damage to the original condition at the contractor s expense. All areas will be cleaned after core drilling. The contractor shall seal all penetrations produced or impacted by this project meeting the fire rating and/or weather barrier to NFPA 99 requirements and/or local fire code, whichever is greater. No unauthorized work shall be initiated without the prior approval of the CO or the COR. The CO will approve any request, or proposal for changes in project scope and/or additional requirements, in writing prior to the contractor proceeding with any related work. Any changes accomplished without prior written approval, will be done at no additional expense to the Government. All measurements and locations are approximate and shall be verified by the contractor. The Government will not be held responsible for any error or variation in any measurements or other data listed, nor will the contractor, because of an error or variation in any measurement listed, be relieved of his responsibility to carry out the contract in accordance with the true intent and meaning of the specifications without additional expense to the Government. Other Considerations The contractor shall be responsible for the verification of all requirements for this project. All field measurements and conditions must be verified by the contractor. Any discrepancies between field conditions, project documents, and this SOW shall be immediately brought to the Contracting Officer s attention. The contractor will be required to submit time schedules for how long a power outage will last if power must out during the project. The contractor must comply with the requirements for Type C/Class II (Lowest Risk) Infection Control Risk Assessment (ICRA). The contractor s supervisor shall always be available to address any needs, the VA COR/Alternative COR may have during the period of performance which is 90 days from start to finish. The contractor shall thoroughly clean inside and outside of project site daily with contractor s own equipment, including vacuums, mops, buckets, wash rags, dust mops, etc. The contractor shall be responsible for the removal of project demo materials/debris /equipment from this facility daily due to nature of the areas working in such as mental ward areas. Contractor personnel will bid by all federal codes and regulations of COVID-19 while working on the government premises. It is the responsible of the contractor to ensure all personnel working in the CLC s have been COVID Test and had their FLU Vaccine prior to starting the work. All paperwork must be submitted 24 hours in advance to the VA COR prior to starting the work. Note: Daily temperature checks will be performed, and masks are required while working in patient care areas. All equipment/tools must be wipe down and sanitized daily if taken off the facility premises. SPECIFICATIONS Provide turnkey dismantling, track refurbishment and new installation services for the Spacesaver Eclipse LCD Pushbutton Electric storage systems outfitted with Spacesaver carriages with quantity 420 modular locking drawers to accommodate hanging pendaflex filing letter size. Removal of all equipment that is not reusable by our service is ground floor to parking lot access. LCD Push Button Assist operation with Audit Trail and Access Control capability. Full height laminated end panels (Steel not acceptable as they dent and show wear over time). One single faced stationary unit, 15 0 long x 16 deep x 94 1/4"" high. Ten double faced moving units, High-Capacity Push Button System, each 15 0 long x 32 deep x 94 1/8"" high. All shelving will be provided with 2 adjustable openings per face, 4 12"" drawers for left to right letter sized files. All drawers keyed differently with a master key. (A total of 420 drawers) System utilizes existing and refurbished recessed structural Spacesaver brand rails. One touch powered carrier with safety recessed shelving retainer and in aisle safety. Exposed ends of the units will be finished with a laminate face panel, your choice of manufacturer s standard colors. Remove and trash existing carriages with shelving. Shelving must be empty prior to tech's arrival or additional charges will apply. Pricing does not include media move. Reuse most electrical components. Carriages shall have a 1,000 lb. per carriage ft load capacity with a ¼ lip that provides a housing for the shelving to be attached to. C channel systems are not approved as shelving can shift over time. All shelving and carriages to be powder coated. Manufacturer to be GreenGuard Certified and can demonstrate they meet Executive Order 13514. All work must be done in accordance with Spacesaver Storage System guidelines to maintain existing warranty on carriages, electric motors, and all electronics. All Spacesaver equipment holds a Limited Lifetime Warranty on all shelving and mobile carriages (structural frames). All work must be done in accordance with Spacesaver Storage System guidelines to maintain existing warranty on carriages, electric motors, shelving, drawers, and all electronics. Any work performed by non Spacesaver certified workers will make null and void remaining warranty. Scope includes relocating and re-assembly of all wiring and electronics including safety sweep devices, logic control boards, end cap panels, electronic control heads, and individual parts and pieces. Our team will update all logic control boards to the newest software version using FOB keys, PCD device and Spacesaver Diagnostic Software. Newest software version will be made available during install. Run full diagnostic test on the system using PCD Device and Spacesaver Diagnostic Software and reprogram all unit s distance sensors and movement speeds and start/stop points. Notification of Work Prior to the start of the work, contractor must notify VA COR 72 hours in advance to schedule a time to start working. Also, contractor will need to notify the COR 72 hours in advance to schedule a power outage too. The contractor shall contact government COR no later than 9 a.m. if scheduled work for that day will not be performed. Site Conditions The work covered by these specifications is an occupied facility. All work must be coordinated with the COR. The contractor shall leave no area unsecured or unsafe at the end of the day. The contractor shall always keep the premises as clean as possible and free from an accumulation of waste materials and rubbish. It shall be the contractor s responsibility to field verify all existing conditions and dimensions prior to bid. All materials removed shall be disposed of offsite by the contractor. Documentation of Services Performed The contractor shall provide at a minimum the following data: All manufacturer warranty information for each piece of equipment installed The contractor shall provide a database (excel spreadsheet) card reader serial number, card reader type, location, floor, and room number (if applicable). The contractor shall provide two (2) full sets of O&M printed or electronic manuals. Manuals will include a Compact Disk (CD) with electronic versions of all documents, as well as any viewing, management, or configuration software on CD. Contractor shall include programming guides, configuration manuals, and user guides. Submit the proper documentation and reports to receive payment on this service. See information below: Payment to be made monthly, in arrears, following satisfactory performance by Contractor s personnel and upon receipt of a properly prepared invoice. Contractor shall be required to invoice through the Tungsten System at http://www.tungsten- network.com/us/en/veterans-affairs/. Additional information regarding Tungsten shall be provided upon award. The contract shall have current registration in the System for Award Management (SAM) for this solicitation; URL: www.sam.gov. The invoice MUST be itemized to include the following information. The VA COR will coordinate with the facility managers on power outages. The VA COR will let the contractor know of any hazards or other harmful conditions. The VA COR will provide emergency response teams members if needed. VA personal will keep any unqualified personal from the work area being worked. Codes and Standards The project shall be in accordance with the latest Uniform Building Code (UBC), National Fire Protection Associate (NFPA) standard, Uniform Federal Accessibility Standard (UFAS), VA Specifications and Standards, and local facility requirements. In the event there is a conflict between any of the standards/codes, the more stringent standard/code shall apply. The contractor shall provide all applicable safety signage and barricades applicable to OSHA, VHA and DOD regulations. Contractor shall comply with VHA DIRECTIVE 2004-012 and all VA Center Policy Memorandums, including but not limited to the following: Protection of patients, visitors, and employees from traumatic injury, as well as occupational and facility associated infections. Compliance with OSHA and State construction safety regulations. Construction contracts require that the general contractor s on-site competent person have completed the OSHA 30-hour construction worker course and that all other general and sub-contractor s workers have completed the OSHA 10-hour construction worker course or other relevant competency training. The determination for training is based on the project hazards and complexity, State and Federal regulations and VA requirements. Verification of completion of training will be required prior to performing work. Compliance with EPA and State environmental regulations. Safety Plan Requirements Prior to performing any work, the contractor will be required to submit a safety plan in accordance with OSHA and VA Regulations. The safety plan shall include, but not limited to the following: Safety and Health Policy (i.e. company policy) Safety and Health Objectives (i.e. company goals, procedures, etc.) Job Site Inspections (i.e. safety inspection/potential hazards/ mishap prevention, etc.) Contractor Safety Administrator (i.e. competent person/safety officer) Personal Protective Equipment (i.e. head, hearing, and eye protection, footwear, clothing, gloves, etc.) Safety Rules (i.e. rules applicable to the work/task being performed, equipment, ladders, confined spaces, open flame, etc.) Job Safety Training (i.e. OSHA, safety meetings, etc.) 30 Hour OSHA Construction Safety Course competent person 10 Hour OSHA Construction Safety Course all others Safety Discipline (i.e. correction of violations, etc.) Emergency Procedures (i.e. responsibilities, procedures, etc.) Lockout/Tagout procedures but be known by the technician Safety Inspections On a weekly basis, the VA will conduct a safety inspection of the work area. This inspection includes, but is not limited to the following: Means of Egress Fire/Smoke Doors unlocked, free of obstructions. Means of Egress (includes Exits) free of obstructions and clearly marked. Housekeeping and Traffic Control Trash and debris are removed promptly and transported outside the construction area. Floor mats and dust tack mats are located at the entrance to construction area. Tools and equipment are properly stored. Signs installed to restrict patient access (doors locked as needed). Dust control measures are in place. Barriers are in place and properly sealed. Clothing PPE is available and properly worn. Clothing is relatively dust free with the construction area and patient areas. Warranty Period Contractor must provide one (1) year warranty on labor and (at least) 90-day warranty on parts. Warranty period does not start until VA COR signs off on the project. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 60 days. This includes options under FAR 52.217-8, Option to Extend Services, which applies to this solicitation. Evaluation of options under FAR 52.217-8 will be accomplished by using the prices offered for the last option period to determine the price for a 6-month option period, which will be added to the base and other option years to arrive at the total price. Evaluation of options will not obligate the Government to exercise the option(s). (End of Provision) VAAR 852.233-70 PROTEST CONTENT/ALTERNATIVE DISPUTE RESOLUTION (OCT 2018) (a) Any protest filed by an interested party shall (1) Include the name, address, fax number, email and telephone number of the protester; (2) Identify the solicitation and/or contract number; (3) Include an original signed by the protester or the protester s representative and at least one copy; (4) Set forth a detailed statement of the legal and factual grounds of the protest, including a description of resulting prejudice to the protester, and provide copies of relevant documents; (5) Specifically request a ruling of the individual upon whom the protest is served; (6) State the form of relief requested; and (7) Provide all information establishing the timeliness of the protest. (b) Failure to comply with the above may result in dismissal of the protest without further consideration. (c) Bidders/offerors and Contracting Officers are encouraged to use alternative dispute resolution (ADR) procedures to resolve protests at any stage in the protest process. If ADR is used, the Department of Veterans Affairs will not furnish any documentation in an ADR proceeding beyond what is allowed by the Federal Acquisition Regulation. (End of Provision) VAAR 852.233-71 ALTERNATE PROTEST PROCEDURE (OCT 2018) (a) As an alternative to filing a protest with the Contracting Officer, an interested party may file a protest by mail or electronically with: Executive Director, Office of Acquisition and Logistics, Risk Management and Compliance Service (003A2C), Department of Veterans Affairs, 810 Vermont Avenue NW, Washington, DC 20420 or Email: EDProtests@va.gov. (b) The protest will not be considered if the interested party has a protest on the same or similar issue(s) pending with the Contracting Officer.
