Attachment 1: Combined Synopsis-Solicitation for Commercial Items RFQ #36C25021Q1110 /lrg Monitor/Defibrillators & AEDs Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and no further solicitation documents will be issued. This solicitation is issued as an RFQ #36C25021Q1110. This solicitation document, incorporated provisions, and clauses are those in effect through Federal Acquisition Circular (FAC) 2021-06. This solicitation is set-aside for OEM (Original Equipment Manufacturer) of the requested equipment listed under the Federal Supply Schedule only. As it has been determined that the OEM s do not have any authorized SDVOSB or Small Business suppliers of the requested equipment. The associated North American Industrial Classification System (NAICS) code for this procurement is 334510 with a small business size standard of 1,250. The FSC/PSC is 6515. The VA Medical Center located in Cincinnati, Ohio is seeking to purchase Monitor/Defibrillators and AED devices to replace existing equipment that is nearing the end of life. Also, consideration will be needed for credit on the trade-in of a quantity of (43) Philips MRX Biphasic AED devices currently not in use. All interested companies shall provide a quotation following the Salient Characteristics provided in this solicitation for the following supply items: Supplies Line Item Description Quantity Needed Unit of Measure 1 Monitor/Defibrillator (plus any required accessories) 50 Each 2 Power Cord 50 Each 3 Power Adapter 50 Each 4 Lithium Batteries 100 Each 5 Charging Station for Batteries (needed qty based on ports) 2-4 Each 6 Finger Clip Sensors for oxygen saturation 50 Each 7 AEDs (plus any required accessories) 25 Each 8 Peripherals, electrodes, cable, etc. 50 Each 9 Any software and licenses 1 Each 10 Training for Power Users, Bio-Med Engineer, and Manuals 1 Lot Please provide line itemization spreadsheet with the original equipment manufacturer (OEM) name, part number (MPN), and warranty information with your quote. (See spreadsheet provided in this solicitation posting Attachment 4.) Also, supply any literature, technical and warranty information with your proposal. Delivery hours are required during hours of operations (8:00am - 3:00pm, Eastern Standard Time) excluding Federal Holidays. Site Demo Visit All potential bidders or representatives MUST demonstrate their equipment/devices on-site to a newly constituted CPRR evaluation team at the Cincinnati VA Medical Center, 3200 Vine Street, Cincinnati, OH. The available dates and time for the demonstration are listed below and must be scheduled via email or phone to the Contracting Officer (Lillie.grey@va.gov) (765-677-6112) by 5:00pm EST on August 31, 2021. Wednesday, September 1 @ 1:00pm to 3:00pm EST Thursday, September 2, @ 9:00am to 11am EST A warranted Contracting Officer will be available at the demonstration to monitor and to tabulate any questions. The questions will be answered by amendment to this solicitation by 5:00pm on Friday, September 3, 2021 and posted on Contracting Opportunities (beta.SAM). Vendors will not bring any free samples or refreshments to the demonstration. Delivery is required FOB Destination. Delivery hours are required during hours of operations (8:00am - 3:00pm, Eastern Standard Time) excluding Federal Holidays. NOTE: This requirement is an ""All or None"" requirement. Offerors who wish to submit an offer in response to this requirement shall offer pricing on all line items or your offer will not be considered for award. The Government will evaluate all quotes received for technical acceptability and satisfying the required salient characteristics. All available technical information on the product being offered shall be supplied to demonstrate that the contractor fully understands the requirement and is fully capable of providing equipment/devices that conform to the Government's requirements. All responses to this RFQ shall be fixed priced and Freight on Board (FOB) Destination. Documentation that confirms the company is the original manufacturer and/or authorized resellers/distributors of the OEM may quote on the requested items.
