- Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-07 (eff. 08/11/2021). This solicitation is for BRAND NAME ONLY equipment and is unrestricted. Indian School Road Phoenix, AZ 85012 POC: Kevin Jacobson RRT, Chief of Respiratory Therapy Background: ZOLL Transport Ventilator is for Veteran patients that have been intubated and require mechanical ventilation outside of the ICU environment. Our goal is to replace 4 LTV Ventilators with the same make and model ZOLL ventilator purchased by the VISN for emergency use. This will allow us to streamline consumables and establish consistency between ventilator models used for everyday use as well as emergency deployment. Description of how the equipment being requested operates: This ventilation device assists patients with respiratory insufficiency who require mechanical ventilation and transport capability. Where will the equipment being requested be used: ZOLL Z-Vent units will be used to support Intensive Care Unit (ICU) and Emergency Department (ED) when patients requiring mechanical ventilation also require transportation to specialty areas such as radiology (CT, MRI) or possibly movement to another facility. Who will be using the equipment being requested: Mechanical ventilation via ZOLL Z-Ventilator is provided by Respiratory Therapists. 5.0 Description of what specific requirements you have of the equipment: ZOLL Z-Vent meets the manufacture and hospital infection control standards. Secondly the ZOLL Z Vent unit will accommodate ventilatory transportation both inter and intra-facility utilizing 10hr internal battery and supplemental oxygen. Thirdly, PVAMC owns 10 ZOLL Z-Ventilators and utilizes this ventilator in the emergency management plan for disaster and pandemic purposes. Consumables supplies are available and located in the RT OMNI Cell. 7.0 Delivery and Shipping Information: The contractor shall provide the following for shipment and delivery of equipment: All items shall be shipped to the following location: Attention: Kevin Jacobson RRT, Chief of Respiratory Therapy PVAHCS 650 E. Indian School Road Phoenix, AZ 85012-1892 Delivery Schedule: 90 Days ARO 8. Confidentiality and nondisclosure: This equipment does not connect to a VA network. Therefore, in accordance with Appendix A, Handbook 6500.6 Checklist, block 6, the C&A requirements do not apply to this requirement, and a Security Accreditation Package is not required. Device will not be connected to network to enable nurse call functionality. This equipment is not capable of storing patient information. SD cards will not be used in equipment to store data. Therefore, in accordance with Appendix A, Handbook 6500.6 Checklist block 7, Appendix B and C provisions apply. Appendix B: The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract.
