Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Regional Health Contracting Office - Pacific, Bldg 339 Clark Road, Ft Shafter, HI 96858, intends to solicit, negotiate, and award a contract for Nihon Kohden EEG System with Windows 10 Upgrade. This is a request for quotation (RFQ). This solicitation number is W81K0221Q0239. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-06. The Standard Industrial Classification Code is 6515, size standard is 1250 employees and the NAICS code of 334510. This procurement is under FAR Part 13.106-1(b)(1) for BRAND NAME ONLY. The provision at 52.212-1, Instructions to Offerors -- Commercial, and its addendum applies to this acquisition. See W81K0221Q0239Att1. The deviation to clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. See W81K0221Q0239Att1 for the FAR clauses that are applicable to this acquisition. The Defense Priorities and Allocations System (DPAS) and assigned rating is not applicable. The Government intends to award a firm fixed-price contract. Contract Requirements - The successful offeror shall be required to provide the Brand Name Only requirement for Nihon Kohden EEG System with Windows 10 Upgrade. Contractors shall quote their unit pricing based on the estimated quantities identified in each Contract Line Item Number.
