Amendment #4 Incorporate the attached form ""Attachment L-2"" Offeror's Request for Clarification dated August 5, 2021 as a result of questions/clarifications and request raised by one of the Prospective and Interested Vendors as described in Items #5 - #8 of the attachment.� Also extends the RFP Due date from August 30, 2021 to September 6, 2021. Amendment #3 Incorporate the attached form ""Attachment L-2"" Offeror's Request for Clarification dated August 3, 2021 as a result of questions/clarifications raised by one of the Prospective and Interested Vendors as described in Items #2 - #4 of the attachment. Amendment #2 Incorporate the attached form ""Attachment L-2"" Offeror's Request for Clarification dated July 28, 2021 as a result of questions/clarifications raised by one of the Prospective and Interested Vendors as described in Item #1 of the attachment Amendment #1 is being issued to extend the Proposal Due date from Friday, July 30, 2021 to Monday, August 30, 2021.� All other original Terms and Conditions of the RFP remains the same.� Please refer to Amendment #1 attached herewith. You are invited to submit a firm-fixed-price proposal in accordance with Line Items in Section B of this RFP and the proposal preparation requirements and other applicable provided documents. Your proposal must be received no later than Friday, July 30, 2021, 5:00 PM Pacific, and be valid for 90 days after submission. Submissions can be provided by electronic files in PDF format.� Please refer to the attached RFP for complete list and details.
